跑步运动损伤的发生率很高,其中髌骨疼痛综合征是最常见的膝关节损伤疾病。通过改变跑步时跑者的落地模式和步频来修正异常的下肢生物力学目前受到越来越多学者的关注。研究目的为针对跑步再训练后跑者髌股关节应力的变化,综述跑步再训练后跑者髌股关节生物力学变化的研究和进展。目前研究已证实通过改变跑步落地模式和步频能够对髌股关节生物力学产生影响,但是针对患有髌骨疼痛综合征跑者的即刻效应的研究较少,且对于同时改变步频和落地模式的效果存在一定争议,未来需进一步探讨髌骨疼痛综合征跑者的跑步再训练效果。  相似文献

There is conflicting evidence on the association between lower limb alignment characteristics and the incidence of running-related injury (RRI). Therefore, the primary aim of this study was to investigate the association between lower limb alignment characteristics and the incidence proportion of RRI in a convenience sample of recreational runners. A total of 89 recreational runners were included in this prospective cohort study. These participants had been running for at least six months and were injury-free at baseline. Lower limb alignment measurements were conducted in order to calculate lower limb discrepancy, Q-angle, subtalar angle and plantar index. All participants also answered a baseline and biweekly online surveys about their running routine, history of RRI and newly developed RRI over a period of 12 weeks. The prevalence of previous RRI and the 12-week incidence proportion of new RRI were calculated. Logistic regression analysis was performed to estimate the association between lower limb length discrepancy, Q-angle, subtalar angle and plantar ach index with the incidence proportion of RRI. The prevalence of previous RRI was 55.1% (n?=?49). The 12-week incidence proportion of new RRI was 27.0% (n?=?24). Muscle injuries and tendinopathies were the main types of RRI identified. The lower leg and the knee were the main anatomical regions affected. We did not find significant associations between lower limb length discrepancy, Q-angle, subtalar angle and plantar arch index and injury occurrence.  相似文献

聚焦跑步时髌股关节生物力学特征,探究穿着不同极简指数(MI)跑鞋对髌股关节接触力、应力等的即刻影响。选取15名习惯后跟着地的健康男性跑者,分别穿着两种MI跑鞋(MI 86%极简跑鞋和MI 26%缓冲跑鞋),使用Vicon红外运动捕捉系统、Kistler三维测力台同步采集3.33 m/s(速度变化范围±5%)跑速下的膝、踝关节运动学和地面反作用力,通过逆向动力学等计算股四头肌肌力、髌股关节接触力、髌股关节接触面积以及髌股关节接触应力。结果显示:两种跑鞋条件下的冲击力峰值和蹬地力峰值均无明显差异。与缓冲跑鞋相比,穿着极简跑鞋跑步时,膝关节最大屈曲角度显著降低(P<0.01);髌股关节接触面积显著减小(P<0.01);膝关节伸肌峰值力矩显著下降(P<0.01);髌股关节接触力和应力峰值均显著减小(P<0.05)。研究表明,相比缓冲跑鞋,穿着极简跑鞋在未影响触地后冲击力峰值的同时,通过降低伸膝力矩大幅度减少髌股关节接触力(下降17.02%)、降低髌股关节接触应力,从而有效改善支撑期髌股关节负荷,为进一步减小髌股关节疼痛综合征风险提供可能。  相似文献

ABSTRACT

The capacity of foot-strike running patterns to influence the functional properties of the Achilles tendon is controversial. This study used transmission-mode ultrasound to investigate the influence of habitual running foot-strike pattern on Achilles tendon properties during barefoot walking and running. Fifteen runners with rearfoot (RFS) and 10 with a forefoot (FFS) foot-strike running pattern had ultrasound transmission velocity measured in the right Achilles tendon during barefoot walking (≈1.1 ms?1) and running (≈2.0 ms?1). Temporospatial gait parameters, ankle kinematics and vertical ground reaction force were simultaneously recorded. Statistical comparisons between foot-strike patterns were made using repeated measure ANOVAs. FFS was characterised by a significantly shorter stance duration (?4%), greater ankle dorsiflexion (+2°), and higher peak vertical ground reaction force (+20% bodyweight) than RFS running (P < .05). Both groups adopted a RFS pattern during walking, with only the relative timing of peak dorsiflexion (3%), ground reaction force (1–2%) and peak vertical force loading rates (22–23%) differing between groups (P < .05). Peak ultrasound transmission velocity in the Achilles tendon was significantly higher in FFS during walking (≈100 ms?1) and running (≈130 ms?1) than RFS (P < .05). Functional Achilles tendon properties differ with habitual footfall patterns in recreational runners.  相似文献

ABSTRACT

This study aimed to examine the characteristics of electromyography (EMG) and kinematics of the supporting leg affecting energy cost while running at incline, level, and decline slopes. Twelve male Japanese middle- and long-distance runners volunteered for this study. The subjects were asked to run at 13.5 km·h?1 on a treadmill under three slope conditions. Sagittal plane kinematics and the EMG of the lower limb muscles, respiratory gases were recorded. Energy cost differed significantly between slopes, being the lowest in decline slope and the greatest in incline slope. Integrated EMG (iEMG) of leg extensor muscles was greater in the incline slope than in the decline slope, and iEMG of the gastrocnemius and soleus muscles correlated positively with energy cost. The knee and ankle joint kinematics were associated with energy cost during running. In incline slope, the knee and ankle joints were more extended (plantarflexed) to lift the body. These movements may disturb the coordination between the ankle and knee joints. The gastrocnemius muscle would do greater mechanical work to plantarflex the ankle joint rather than transfer mechanical energy as well as greater mechanical work of mono-articular muscles. These muscular activities would increase energy cost.  相似文献

BackgroundRunning is becoming an increasingly popular activity among Americans with over 50 million participants. Running shoe research and technology has continued to advance with no decrease in overall running injury rates. A growing group of runners are making the choice to try the minimal or barefoot running styles of the pre-modern running shoe era. There is some evidence of decreased forces and torques on the lower extremities with barefoot running, but no clear data regarding how this corresponds with injuries. The purpose of this survey study was to examine factors related to performance and injury in runners who have tried barefoot running.MethodsThe University of Virginia Center for Endurance Sport created a 10-question survey regarding barefoot running that was posted on a variety of running blogs and Facebook pages. Percentages were calculated for each question across all surveys. Five hundred and nine participants responded with over 93% of them incorporating some type of barefoot running into their weekly mileage.ResultsA majority of the participants (53%) viewed barefoot running as a training tool to improve specific aspects of their running. However, close to half (46%) viewed barefoot training as a viable alternative to shoes for logging their miles. A large portion of runners initially tried barefoot running due to the promise of improved efficiency (60%), an attempt to get past injury (53%) and/or the recent media hype around the practice (52%). A large majority (68%) of runners participating in the study experienced no new injuries after starting barefoot running. In fact, most respondents (69%) actually had their previous injuries go away after starting barefoot running. Runners responded that their previous knee (46%), foot (19%), ankle (17%), hip (14%), and low back (14%) injuries all proceeded to improve after starting barefoot running.ConclusionPrior studies have found that barefoot running often changes biomechanics compared to shod running with a hypothesized relationship of decreased injuries. This paper reports the result of a survey of 509 runners. The results suggest that a large percentage of this sample of runners experienced benefits or no serious harm from transitioning to barefoot or minimal shoe running.  相似文献

ABSTRACT

This study aimed to explore the plantar loading variables between habitual rearfoot strike (RFS) and non-rearfoot strike (NRFS) during running. 78 healthy males participated in this study (41 RFS, 37 NRFS). In-shoe pressure sensors were used to measure plantar loading while the participants were running on a 15 m indoor runway with their preferred foot strike pattern (FSP) at 12.0 ± 5% km/h. Results indicate that force and pressure parameters were much higher in the rearfoot and midfoot regions during RFS running and relatively greater in forefoot region during NRFS running. However, compared with NRFS running, the contact area, maximum force and force-time-integrals during RFS running on total foot were 21.44% (P < 0.001, ES = 2.29), 13.99% (P = 0.006, ES = 0.64) and 21.27% (P < 0.001, ES = 0.85) higher, respectively. Total foot peak pressure and pressure-time-integral between two FSPs were similar. Higher loads in the rearfoot region may transmit to the knee joint and result in patellofemoral joint injuries. NRFS runners’ higher loads in forefoot seem to be ralated to metatarsal stress fractures and compensatory damage to the Achilles tendon. Therefore, runners should choose proper FSPs according to their unique physical conditions.  相似文献

ABSTRACT

Aiming to evaluate the prevalence of unilateral/bilateral patellofemoral pain (PFP) among young dancers, and to investigate whether different factors are associated with PFP in young dancers, 132 dancers aged 12–14 years were assessed for PFP. Anthropometric parameters, proprioception ability, dynamic postural balance (DPB), and muscle strength were measured. PFP was found in 64.1% of the dancers. No significant differences in the prevalence of dancers with no, unilateral, or bilateral PFP at different ages were found. Significant age effects were found for anthropometric and developmental measurements, and for intensity of training. PFP effect was found for DPB asymmetry, ankle proprioception, and leg-length %height. A higher hip abductor/adductor ratio was associated with PFP in 14-year-old dancers. Binomial logistic regression showed that increased number of hours per day (h/day) and decreased number of hours per week (h/week), low proprioception scores, greater leg length as %height, and more anterior DPB asymmetry were significant predictors of PFP. In conclusion: unilateral/bilateral PFP is common among young dancers. Body morphology, reduced ankle proprioception ability, DPB asymmetry, and increased h/day of practice are associated with PFP. Dance teachers should start monitoring the impact of training and implement injury modification/prevention strategies when their students are at a young age.  相似文献

Abstract

Chronic knee disorders, such as patellofemoral pain syndrome and patellar tendinosis, are common injuries in volleyball players. Using a randomized clinical trial, the aim of the present study was to determine the effect of a 4-month in-season intervention programme on the prevalence of anterior knee pain in volleyball players. No significant differences were observed between the intervention and control group for the prevalence of anterior knee pain after the intervention programme (odds ratio =1.58 [95% confidence interval: 0.60–4.20]). Also, no significant differences were observed for individuals with previously sustained anterior knee pain (OR =0.81 [95% CI: 0.33–2.20]). It was revealed that the risk for recurrent anterior knee pain is significantly higher than the risk for newly sustained anterior knee pain (OR =5.79 [95% CI: 1.59–21.00]). In conclusion, we were unable to confirm any preventive effect of the intervention programme on the prevalence of anterior knee pain. However, a previous study using the same intervention and measuring the effect on the intrinsic risk factors of anterior knee pain showed a positive effect.  相似文献

Abstract

Cartilage oligomeric matrix protein is a structural protein of the extracellular matrix, while thrombospondin-2 is a matricellular protein involved in cell–matrix interactions. Recent studies have shown that genetic variation is a significant risk factor for Achilles tendinopathy, and the genes encoding cartilage oligomeric matrix protein (COMP) and thrombospondin-2 (THBS2) were identified as good candidate genes for association with Achilles tendinopathy. This study aimed to test the association of sequence variants within these candidate genes with the risk of Achilles tendinopathy in participants from South Africa (SA) and Australia (AUS). Three-hundred and forty (133 SA; 207 AUS) control participants with no history of Achilles tendinopathy and 178 (94 SA; 84 AUS) participants clinically diagnosed with Achilles tendinopathy were genotyped for five single nucleotide polymorphisms within the COMP and THBS2 genes in this case-control study. There was no difference in genotype distributions between control and tendinopathy groups for either the THBS2 variants rs9505888, rs6422747 and rs9283850, or the COMP variants rs730079 and rs28494505 in the SA and AUS populations. As the selection of COMP and THBS2 as candidate genes was hypothesis driven, based on biological function, the possibility that other variants within these genes are associated with Achilles tendinopathy cannot be excluded.  相似文献

This study examined the kinematic effects of orthoses in participants with a history of chronic Achilles tendon injury. Twelve participants ran at self-selected speeds on a treadmill with and without customized orthoses. Joint and segment angles including leg abduction, calcaneal, eversion, ankle dorsiflexion, and knee flexion angles were calculated from three-dimensional data throughout stance. Five footfalls were obtained for each participant and condition. Statistical tests revealed an increase in maximum eversion with orthoses (P < 0.001, eta(p)2 = 0.642). In the individual participant analysis, this was evident in 9 of 12 participants. Trends towards increased eversion range of motion and decreased ankle dorsiflexion maximum and range of motion angles were also observed. Increased eversion was unexpected as all devices were designed to provide pronation control as deemed necessary by the podiatrist. Despite this, participants reported between 50 and 100% (average 92%) relief from symptoms with the use of orthoses. Further analysis of the angle-time curves and coordination between angular measures is recommended.  相似文献

Abstract

The objective of this study was to compare the three-dimensional lower extremity running kinematics of young adult runners and elderly runners. Seventeen elderly adults (age 67–73 years) and 17 young adults (age 26–36 years) ran at 3.1 m · s?1 on a treadmill while the movements of the lower extremity during the stance phase were recorded at 120 Hz using three-dimensional video. The three-dimensional kinematics of the lower limb segments and of the ankle and knee joints were determined, and selected variables were calculated to describe the movement. Our results suggest that elderly runners have a different movement pattern of the lower extremity from that of young adults during the stance phase of running. Compared with the young adults, the elderly runners had a substantial decrease in stride length (1.97 vs. 2.23 m; P = 0.01), an increase in stride frequency (1.58 vs. 1.37 Hz; P = 0.002), less knee flexion/extension range of motion (26 vs. 33°; P = 0.002), less tibial internal/external rotation range of motion (9 vs. 12°; P < 0.001), larger external rotation angle of the foot segment (toe-out angle) at the heel strike (?5.8 vs. ?1.0°; P = 0.009), and greater asynchronies between the ankle and knee movements during running. These results may help to explain why elderly individuals could be more susceptible to running-related injuries.  相似文献

Abstract

The aim of this study was to analyse lower limb joint moments, powers and electromyography patterns in elite race walking. Twenty international male and female race walkers performed at their competitive pace in a laboratory setting. The collection of ground reaction forces (1000 Hz) was synchronised with two-dimensional high-speed videography (100 Hz) and electromyography of seven lower limb muscles (1000 Hz). As well as measuring key performance variables such as speed and stride length, normalised joint moments and powers were calculated. The rule in race walking which requires the knee to be extended from initial contact to midstance effectively made the knee redundant during stance with regard to energy generation. Instead, the leg functioned as a rigid lever which affected the role of the hip and ankle joints. The main contributors to energy generation were the hip extensors during late swing and early stance, and the ankle plantarflexors during late stance. The restricted functioning of the knee during stance meant that the importance of the swing leg in contributing to forward momentum was increased. The knee flexors underwent a phase of great energy absorption during the swing phase and this could increase the risk of injury to the hamstring muscles.  相似文献

The coronal and sagittal plane leg movements of 24 experienced male cyclists were assessed using video analysis while cycling on a Kingcycle windload simulator. The cyclists were grouped into those with a history of injury and an asymptomatic group on the basis of self-reported injury status. The ages, cycling experience, competition distances and competition speeds of the two groups were compared using Student's t-test. No significant differences (P < 0.05) were found for any of these variables. The maximum and minimum shank adduction, shank adduction velocities, knee flexion and ankle dorsiflexion values were also compared using Student's t-test. Significant differences were found at the point of maximum adduction (1.9 degrees; P = 0.019) and minimum dorsiflexion (4.9 degrees; P = 0.014). These differences indicated more dorsiflexion and greater abduction on the part of the symptomatic cyclists, supporting previous research that found that cyclists with a history of injury differ from those without a history of injury in the coronal plane leg movement patterns they adopt. Also, the most extreme medial position of the knee relative to the ankle occurred during knee extension. This supports the potential injury mechanism proposed by Francis (1986), which had previously only been examined using coronal plane kinematics.  相似文献

BackgroundWhen non-operative management fails to improve symptoms in patients with non-insertional Achilles tendinopathy, surgery may be required. Various open and endoscopic techniques have been proposed, and platelet-rich plasma (PRP) injections have been proposed as an adjunct to aid tendon healing.MethodsThirty-six patients with mid-portion Achilles tendinopathy were randomized to undergo endoscopic debridement alone (n = 19) or in combination with intraoperative PRP application (n = 17). Clinical outcome measures included the Visual Analogue Scale for pain, function, and satisfaction and the Victorian Institute of Sports Assessment–Achilles (VISA-A) questionnaire. Patients were followed-up at 6 weeks, 3 months, 6 months, and 12 months after surgery. An MRI examination at 3 and 12 months was used to assess signal alterations within the tendon.ResultsBoth groups showed significant clinical improvement (p < 0.05) after surgery, with no difference between the 2 groups. Tendon diameter increased at 3 months and decreased at 12 months. The tendinopathy area increased at 3 months and decreased at 12 months below baseline level in both groups. There was no significant difference between the groups regarding the MRI parameters. Nodular thickening and MRI-detected signal alteration persisted after surgery, with no association between imaging and clinical outcome. Five minor complications were reported: 2 in the PRP group and 3 in the control group.ConclusionEndoscopic debridement of the Achilles tendon improved clinical outcomes in patients with mid-portion tendinopathy. The addition of PRP did not improve outcomes compared to debridement alone. MRI parameters showed no association with clinical outcomes.  相似文献

Abstract

Sixty-two elite adolescent (age: 16.39 ± 1.62 yrs) female track and field athletes volunteered to be measured isokinetically for peak torque of the leg flexors and extensors. The sample included 16 throwers, 11 jumpers, 12 middle-distance runners (> 400 m), and 23 sprinters. A Cybex II isokinetic dynamometer was used to measure dominant leg strength at 180°/sec. One-way ANOVA revealed significant between group differences for absolute flexion and extension (Nm), extension/body weight (Nm/Kg), and extension/lean body weight (Nm/Kg). Scheffe post-hoc comparisons showed that, for absolute leg extension strength, throwers were stronger than middle-distance runners (by 54.4%), sprinters (35.6%), and jumpers (23.6%). For absolute leg flexion movements, throwers were stronger than middle distance runners (37.4%) and sprinters (32.1%). Jumpers had greater extension peak torque values than middle-distance runners relative to body weight (14.0%), and throwers were stronger than middle-distance runners for extension/lean body weight (14.9%)). No other comparisons were significant. However, when strength was expressed relative to body weight or lean body weight, there were few differences between events for flexion or extension. Given that absolute strength scores were correlated with body weight and lean body weight, peak torque/body weight or peak torque/lean body weight may be more sensitive than absolute values as indicators of underlying factors (such as training status or fiber type composition) responsible for the differences in muscular strength between groups.  相似文献

Abstract

The aim of this study was to test the correlation between knee-to-hip flexion ratio during a single leg landing task and hip and knee strength, and ankle range of motion. Twenty-four male participants from a professional soccer team performed a continuous single leg jump-landing test during 10s, while lower limb kinematics data were collected using a motion analysis system. After biomechanical testing, maximal isometric hip (abduction, extension, external rotation), knee extension and flexion strength were measured. Maximum ankle dorsiflexion range of motion was assessed statically using the weight bearing lunge test. Pearson correlation coefficients were calculated to determine the associations between the predictor variables (knee and hip strength, and ankle ROM) and the main outcome measure (knee-to-hip flexion ratio). Correlation between knee-to-hip flexion ratio and hip abductors strength was significant (r = ?0.47; p = 0.019). No other significant correlations were observed among the variables (p > 0.05). These results demonstrated that a lower hip abductors strength in male soccer players was correlated with a high knee-to-hip flexion ratio during landing from a single leg jump, potentially increasing knee overload by decreasing energy absorption at the hip. The results provide a novel proposal for the functioning of hip muscles to control knee overload.  相似文献

The purpose of this study was to investigate the interaction of foot strike and common speeds on sagittal plane ankle and knee joint kinetics in competitive rear foot strike (RFS) runners when running with a RFS pattern and an imposed forefoot strike (FFS) pattern. Sixteen competitive habitual male RFS runners ran at two different speeds (i.e. 8 and 6?min?mile?1) using their habitual RFS and an imposed FFS pattern. A repeated measures analysis of variance was used to assess a potential interaction between strike pattern and speed for selected ground reaction force (GRF) variables and, sagittal plane ankle and knee kinematic and kinetic variables. No foot strike and speed interaction was observed for any of the kinetic variables. Habitual RFS yielded a greater loading rate of the vertical GRF, peak ankle dorsiflexor moment, peak knee extensor moment, peak knee eccentric extensor power, peak dorsiflexion and sagittal plane knee range of motion compared to imposed FFS. Imposed FFS yielded greater maximum vertical GRF, peak ankle plantarflexor moment, peak ankle eccentric plantarflexor power and sagittal plane ankle ROM compared to habitual RFS. Consistent with previous literature, imposed FFS in habitual RFS reduces eccentric knee extensor and ankle dorsiflexor involvement but produce greater eccentric ankle plantarflexor action compared to RFS. These acute differences between strike patterns were independent of running speeds equivalent to typical easy and hard training runs in competitive male runners. Current findings along with previous literature suggest differences in lower extremity kinetics between habitual RFS and imposed FFS running are consistent among a variety of runner populations.  相似文献

Matrix metalloproteinase-3 (MMP3) is a mediator of matrix remodelling and a proposed susceptibility locus in the genetic profile of musculoskeletal soft tissue injuries. Therefore, this study aimed to validate the MMP3 gene as a risk marker for these injuries by conducting a case control genetic association study in two independent samples groups. Three previously investigated MMP3 variants (rs679620, rs591058 and rs650108) in addition to the functional promoter variant (rs3025058) were genotyped in 195 Australian control participants and 79 Australian individuals with chronic Achilles tendinopathy. Similarly, 234 South African individuals with acute anterior cruciate ligament ruptures and 232 matched control participants were also analysed. Based on high linkage with the previously associated MMP3 variant rs679620, rs3025058 was inferred and found to be associated with increased risk for Achilles tendinopathy within the South African group (= 0.012; OR: 2.88; 95% CI: 1.4 to 6.1). Lastly, the 6A-G-C-G haplotype, constructed from the investigated variants, was significantly associated with reduced risk for Achilles tendinopathy (29% CON vs. 20% TEN, = 0.037) in the Australian group. In conclusion, a signal surrounding MMP3 is apparent with respect to Achilles tendinopathy. However, whether the investigated variants are contributing to injury susceptibility or whether they are merely linked to the risk conferring variants mapping elsewhere within the MMP gene cluster on chromosome 11, still requires refining.  相似文献
