The benefits of breastfeeding for infant, mother, family, and community are well recognized, and increasing breastfeeding rates is considered an important health-promotion strategy. Improving breastfeeding knowledge and practice among individuals caring for breastfeeding women is considered an important aspect of this strategy. The practice-development initiative described in this article aimed to improve hospital-based breastfeeding rates through the implementation of The Ten Steps to Successful Breastfeeding. The initiative included the development and implementation of an education program aimed at changing and improving breastfeeding practices. The program was evaluated in three ways: changes in breastfeeding rates at hospital discharge; client preparation for breastfeeding and satisfaction during the postnatal period; and staff knowledge and skills.  相似文献

In spite of the efforts of numerous organizations and individuals to offer mother-friendly birth information and care in the United States, the nation remains a highly technical, low-touch birth culture with a decline in positive maternal-fetal outcomes. A number of organizations and individuals came together to form the Coalition for Improving Maternity Services and to create the Mother-Friendly Childbirth Initiative (MFCI). The MFCI is a wellness model of maternity care that offers 10 evidence-based steps of care that will improve birth outcomes and reduce costs. In this column, the author answers the question, "Is choosing to give birth naturally a realistic option in today's birth environment?" Women's choices of health care provider and place of birth are limited by the general belief that birth is safe only in the hospital and when managed by obstetric medicine. In the typical hospital environment, women rarely have access to the wide variety of comfort measures and the continuous emotional and physical support required to give birth naturally. Routine care practices further limit women's birthing options. The Coalition for Improving Maternity Services' document, the "Mother-Friendly Childbirth Initiative," offers direction for creating birth environments that ensure options for women who choose to give birth naturally.  相似文献

Substantial evidence documents the superiority of breastfeeding for mothers and breastmilk for babies. Although the American Academy of Pediatrics and the U.S. Healthy People 2010 initiative promote breastfeeding, current breastfeeding rates often fall short of recommendations. This study determined factors associated with exclusive breastfeeding 2 to 4 weeks following discharge from a large, urban, academic medical center striving for Baby-Friendly designation. Results indicated that mothers who breastfed within the first hour of birth (61%) were significantly more likely to be exclusively breastfeeding 2 to 4 weeks after discharge. Incorporating care practices that include a number of the "Ten Steps to Successful Breastfeeding," as recommended by the Baby-Friendly Hospital Initiative, may increase the duration of exclusive breastfeeding after discharge.  相似文献

The Healthy Steps for Young Children Program (HS) is designed to provide support for new parents through the pediatrician's office. We present findings of the effect of HS on the quality of mother–child interaction, security of attachment, and child behavior using observational data from two of the HS sites that utilized a randomized treatment design. Results indicated that mothers participating in Healthy Steps were more likely to interact sensitively and appropriately than mothers in the comparison group at the second assessment point (age 34–37 months) but not at the first assessment point (age 16–18 months). There were no differences in child outcomes at either time point when the cross-sectional data were analyzed. However, the results of the longitudinal analysis (which included families who participated in the home observations at both Time 1 and Time 2) indicated that Healthy Steps participation was associated with greater security of attachment and fewer child behavior problems.  相似文献

新农村建设与农村城镇化是紧密相关的两个层面,前者能够有力地促进后者。在"旧三农"必须变成"新三农"并且经历"三步走"的过程中会遇到诸多障碍因素,包括分散的自然村落与城镇集中发展之间的矛盾、不同的行政村之间进行合并的困扰、有些新农村建设只做表面文章、农村经济市场化程度较低等。变"旧三农"为"新三农"过程中需要农业工业化、农民知识化、农村现代化,在新农村建设与农村城镇化互动中改变农村面貌。结合不同农村的具体特点,可以通过发展工业企业、依托城镇职能以及政府行政造城等方式突出城镇作用推进农村城镇化,也可以通过完善农业产业、开发旅游服务和重用能人贤士等方式突出农村作用推进农村城镇化。  相似文献
