Forty-five abusive parents (40 mothers, 5 fathers) in treatment at a large, urban medical center's child abuse program were compared on 22 parent, child, and treatment variables posited to bear on reabuse. While no one variable by itself was strongly associated with reabuse, the data indicated that interactions involving several variables (e.g., income source, marital status, and the abuser's personal abuse history) significantly differentiated between reabusers and nonreabusers. Implications of the findings for research and clinical intervention are discussed.  相似文献

OBJECTIVE: This exploratory study investigated how attributes from multiple domains influence retention in a voluntary home-visiting child abuse prevention program. METHOD: A sample of 1093 "at-risk" families participated in a home-visiting child abuse prevention program. A total of 71 Family Support Workers (FSWs) provided home visitation services in 12 different communities. Hierarchical general linear modeling (HGLM) was used to examine the community, home visitor, and maternal attributes that predicted retention in the program beyond 1 year. RESULTS: Multi-level analyses (HGLM) revealed significant community, home visitor, and maternal level effects. Families were less likely to remain in a home-visiting child abuse prevention program for at least 1 year if they lived in an area with high community violence. Families were more likely to remain when the home visitor received more hours of direct supervision. Older mothers were more likely to remain for at least 1 year than were younger mothers. Hispanic mothers were more likely to remain than were White non-Hispanic mothers. CONCLUSIONS: These findings demonstrate the utility of looking across multiple levels of influence when examining retention in home-visiting child abuse prevention programs. To increase retention rates home visitors will need to be adaptable to fit the needs of families in violent communities. Supervisors can influence retention rates by providing more hours of direct supervision. Other strategies that may contribute to program retention include meeting the scheduling needs of younger mothers and involving the families of younger mothers more effectively.  相似文献

A three-year study examined the effects of two prevention programs for sole-support mothers with preschool-aged children. A life-skills, esteem-building group program was compared with a group program teaching life skills combined with parent training. A non-randomized, repeated-measures design was employed with measures occurring before, after, and one year after each program for participants in the two intervention groups and for a group of matched comparison subjects. The lack of competency enhancement effects appeared to be a function of reduced exposure of subjects to the interventions. In addition, there was no support for the connection between short-term competency enhancement and long-term prevention of disorder. The negative results serve, in this report, as a cautionary tale highlighting the difficulties in conducting evaluations of primary prevention. It would appear that program planners are placed in a Catch-22 situation by trying to justify the effectiveness of primary prevention through short-term, low-intensity approaches.  相似文献

OBJECTIVE: The purpose of this paper is to review literature related to home visitation support for new families using volunteer visitors and to describe characteristics of volunteers of the Kempe Community Caring Program, a home visitor support program for first time parents. Suggestions for effectively utilizing volunteers in family support programs are made. METHOD: Program data was collected and reviewed on 592 volunteer home visitors and the 1,144 families they served. Several evaluations and assessments were performed in order to attempt to measure the success of the intervention. Motivation for volunteering, longevity with program, and the extent of sociodemographic match between visitor and client were reviewed and discussed. RESULTS: Information on matched volunteer-client pairs was limited; however, speculations were made that neither motivation to participate nor sociodemographic match were critical factors in determining success of the intervention. Duration of volunteer participation was influenced by marital status, but not by age. Married volunteers stayed with the program significantly longer than unmarried volunteers. Low risk families demonstrated slightly improved confidence in parental function after receiving home visitation by a volunteer. CONCLUSION: This program suggests that new mothers may benefit from receiving the services of a volunteer home visitor; specifically, their confidence as parents and ability to establish positive relationships show slight improvement. Likewise, volunteers and families felt satisfied with the supportive relationships generated. In addition, a volunteer profile emerged which may be beneficial to similar programs' recruiting efforts.  相似文献

Head Start centers were randomly assigned to intervention (parent training) or control conditions, and the role of maternal mental health risk factors on participation in and benefit from parent training was examined. Parenting was measured by parent report and independent observation in 3 domains: harsh/negative, supportive/positive, inconsistent/ineffective parenting. Structural equation modeling showed that parent engagement training was associated with improved parenting in a dose-response fashion. Mothers with mental health risk factors (i.e., depression, anger, history of abuse as a child, and substance abuse) exhibited poorer parenting than mothers without these risk factors. However, mothers with risk factors were engaged in and benefited from the parenting training program at levels that were comparable to mothers without these risk factors.  相似文献

Objective

This study set out to carry out a feasible, real-world, randomized clinical trial to examine the benefits of home-based paraprofessional parent aide services in reducing physical abuse and neglect risk in high-risk parents.

Methods

Families were randomly assigned to receive either parent aide plus case management services (n = 73) or case management services only (n = 65), collecting in-home data on physical child abuse and neglect and proximal risk and protective factors, just prior to service initiation, and again after six months of services.

Results

Mothers receiving parent aide and case management services reported significant improvements from baseline to six-month follow-up in self-reported indicators of physical child abuse risk, as well as improvements on parental stress, mastery, depression, and anxiety, whereas mothers receiving only case management services did not. The slopes of such observed changes across groups, however, were not found to be statistically significantly different. No discernable improvements were found with regard to indicators of risk for child neglect.

Conclusions

As the first randomized clinical trial examining the effectiveness of parent aide services, this study provides the first controlled evidence examining the potential benefits of this service modality. This study suggests promising trends regarding the benefit of parent aide services with respect to physical child abuse risk reduction and related predictors, but evidence does not appear to suggest that such services, as they are presently delivered, reduce child neglect.

Practice implications

These findings support the continued use of parent aide services in cases of physical child abuse and also suggest careful consideration of the ways such services may be better configured to extend their impact, particularly with respect to child neglect risk.  相似文献

Maltreatment of children is a key predictor of a range of problematic health and developmental outcomes. Not only are affected children at high risk for recurrence of maltreatment, but effective interventions with known long term impact are few and limited. While home visiting is one of the most tested secondary prevention models for improving parenting, its primary focus on young primiparous mothers underemphasizes one of the most important risk groups: child welfare involved multiparous mothers. This study's focus is a randomized controlled trial of Healthy Families New York that included a subgroup of mothers (n = 104) who had at least one substantiated child protective services (CPS) report before enrolling in the program. By the child’s seventh birthday, mothers in the home visited group were as half as likely as mothers in the control group to be confirmed subjects for physical abuse or neglect (AOR = .46, p = .08). The number of substantiated reports for mothers in the control group was twice as high as for those in the home visited group (1.59 vs. 79 p = .02, ES = .44). Group differences were only observed after the child's third birthday, suggesting the possible effect of surveillance in early years. Post-hoc analyses indicate that home visited mothers had fewer subsequent births that may have contributed to less parenting stress and improved life course development for mothers. In light of our findings, we suggest considering and further testing home visiting programs as a tertiary prevention strategy for child welfare-involved mothers.  相似文献

Research Findings: This study simultaneously examined parental depression and parent involvement as predictors of satisfaction with an early childhood intervention program. Parents (N = 203) of Head Start children participated in this short-term longitudinal study. Measures of parent involvement and satisfaction assessed multiple dimensions of these constructs. Nearly 40% of low-income mothers reported being sometimes or chronically depressed over the course of 1 year of the Head Start program. Compared with mothers who were never depressed, those who were sometimes depressed reported less involvement in home- and school-based activities as well as fewer interactions with their child's teacher. Never depressed parents were more likely to be satisfied with their child's teacher compared with either group of depressed mothers. Higher levels of parent involvement and parent–teacher interaction predicted optimal satisfaction with Head Start services. Practice or Policy: Implications of results for practice are considered in terms of teacher training to recognize unique needs involved in working to establish a home–school connection with mothers experiencing depression. Strategies for building community partnerships to assist with mental health needs are discussed.  相似文献

Implementing a program of supplemental instruction (SI) in the present climate of transition in South Africa requires attention to involving students in planning and conducting the new program. Involved in this process are political opportunities and constraints. This paper recounts the steps (and missteps) taken in beginning an SI program in high risk courses in two academic departments at Border Technikon: Accounting and Management. It documents the steps taken to enlist support of the Student Representative Council (SRC) in setting policy, selecting tutors, and maintaining the program's funding base. We maintain that a successful SI program must draw upon the energies of tertiary level students, both in roles as learners in the high risk courses and as student leaders at an advanced level. Some students may experience both roles. For this process to be more than an imposition from the outside, the program must be a partnership with faculty and students sharing a stake in the outcomes. Both groups must see that they benefit from the smooth working of the SI system. Our results to date indicate that such a joint ownership is occurring. Objective: To describe the annual operational costs of a mature Experience Corps® program in elementary schools in the Baltimore City Public School System. Methods: Systematic records of expenditures kept by the community partner, Greater Homewood Community Corporation, to be reported to funders were made available for analysis. Expenditures were aggregated to the level of fiscal year and grouped in categories in consultation with the community partner. Results: Expenditures to provide reimbursement to volunteers and to administer this complex volunteer program in an environment in which the school district needed additional management resources for such a program dominate the total expenditures. In the 2008 fiscal year, the program provided 158,305 hours of older adult volunteer service to public elementary schools in Baltimore City and the cost per volunteer hour was $9.79; in the 2009 fiscal year the program provided 157,238 hours of older adult volunteer service and the cost per volunteer hour was $9.60. This suggests consistency and may also suggest that the program is becoming more efficient over time. Conclusion: The costs presented represent the costs of running a mature program after an initial investment has been made. These costs would need to be compared with the short-term and long-term benefits to the older adult volunteers, the school children, other school personnel, and the schools' and volunteers' communities to perform a comprehensive benefit-cost analysis to inform public policy.  相似文献

Learning to parent sensitively and safely can be challenging for adults with childhood abuse and neglect experiences. Such childhood experiences are prevalent among incarcerated parents whose ability to parent their own children is also limited by separation from them. Several prisons have developed programs to foster pro-social parenting skills among incarcerated mothers and fathers to assist them on release. This paper reports a qualitative research study that explored the factors affecting the delivery and outcomes of parenting programs in correctional facilities in New South Wales Australia from the perspective of individuals involved in developing and implementing the programs. Thematic analysis of 19 interviews identified two main themes: supporting parents’ learning in correctional settings and providers’ learning about parent education in correctional settings. Respondents reported the benefits of providing creative learning opportunities enabling parents to build on their strengths and to develop relationships. These factors contributed to changing prisoners’ attitudes and supporting them to consider alternative parenting approaches. The co-productive approach to parent education supported enhanced parenting knowledge among parents and greater insights among educators. Parenting education can be successfully delivered in correctional settings and can assist incarcerated parents to build on existing knowledge and adapt it to their own needs.  相似文献

OBJECTIVE: To share the experiences of the first clinical multidisciplinary child abuse and neglect (CAN) team in Turkey with international child abuse community. METHODS: The authors established the first Turkish CAN follow-up team at Dr. Behcet Uz Children's Hospital. Following a training program in five teaching hospitals in Izmir, the authors kept a record of every case diagnosed with CAN from these hospitals between 1996 and 1998. The demographic, epidemiologic, and medical features of this case series are summarized. RESULTS: Fifty cases were diagnosed and followed-up. Seventy-six percent of patients were reported from Dr. Behcet Uz Children's Hospital. Age and sex distribution was 9.2 +/- 6.7 years and 46% male, 54% female, respectively. The offenders were only father in 38%, only mother in 28%, and multiple in 34%. More than three CAN risk factors were present in 94%. Of the children reported 44% survived, 14% died, and 42% were lost to follow-up. Sixteen percent were free of reabuse, and 42% survived with handicaps. CONCLUSIONS: Child abuse and neglect is a reality in Turkey. The team's work increased pediatricians' awareness of CAN. Reporting these cases to Social Affairs Bureau was established for the first time in Turkey. Physicians in Turkey need training to diagnose and properly report CAN. The implemen tation of a national CAN prevention program is an urgent need for Turkey.  相似文献

This study of physical abuse to children under the age of 2 addresses the prognosis of the family, compliance with treatment recommendations and the long term outcome of the child. Data were collected from a multidisciplinary evaluation team and from the Department of Human Services' records on 71 children. Nearly all families were referred for mental health counseling and parent training, but only 33% complied. Half of the children were abused more than once; 30% were abused after referral for counseling. The incidence of reabuse was unrelated to compliance with counseling. The majority of children had some out-of-home placement with a mean time in foster care of 11 months and in care of relatives for 24.4 months. In 69% of families, more than one child was abused. At the time of follow-up, 41% of children and 47% of siblings were not living with either parent. Half of the families were judged to be functioning poorly and only nine had made good progress in counseling.  相似文献

OBJECTIVE: To examine the relationship between physical abuse of adolescents and parenting by mothers and fathers and whether the association differs by gender. METHODS: Subjects were adolescents, 51 girls and 45 boys, documented by Child Protective Services (CPS) as physically abused during adolescence. Comparison subjects were non-abused adolescents, 47 girls and 48 boys, from the same suburban communities. Subjects completed the following: Family Adaptability and Cohesion Evaluation Scale, Parental Bonding Instrument, modified Conflict Tactics Scale (assessing physical abuse/punishment by each parent). RESULTS: Although CPS generally cited fathers as the abuse perpetrators, abused boys and girls often reported experiencing physical maltreatment from both parents. Not surprisingly, comparison subjects rated parents more positively than abused subjects. For both groups, mothers were perceived as more caring and less controlling, were reported to have closer relationships with their adolescents, and were less likely to use abuse/harsh punishment than were fathers. Differences between the adolescents' perceptions of mothers and fathers were more pronounced for abused than for comparison subjects. Boys' and girls' perceptions of parenting were generally similar except that girls, especially the abused girls, reported feeling less close to fathers. Abused girls also viewed mothers as less caring than the other groups viewed mothers. Abused girls were also less likely than abused boys to perceive that either parent, but particularly fathers, had provided them with an optimum style of parenting. CONCLUSIONS: Adolescents who experienced relatively mild physical abuse reported dysfunctional family relationships, which may place them at risk of poor adult outcomes. Adolescents' reports suggest that CPS reports may underestimate physical maltreatment by mothers.  相似文献

The purposes of this study were to determine the extent and types of parent involvement in Head Start programs, and to examine the relations between parent participation and family, teacher and classroom characteristics. Parents (n = 1131) and teachers (n = 59) from four Head Start programs participated. Data were gathered through volunteer logs, parent interviews, teacher questionnaires, and classroom observations. The most frequent type of parent involvement activity was helping out in the classroom, followed by attendance at parent meetings. This pattern was consistent across the year (fall or spring), and across total amount of participation (i.e. parents participating one, two, three or more times in the year). Parent employment was the strongest predictor of parent involvement compared to other parent characteristics. Among teacher and classroom characteristics, classroom quality was the strongest predictor of parent involvement. Also, teachers with more years of experience in Head Start had more total hours of volunteering in their classrooms and had volunteers returning more times. Teachers’ reports of the involvement of parents in their classrooms were moderately correlated with volunteer logs, while parent self-reports of their involvement were only modestly correlated with volunteer logs, indicating that teachers may be more accurate than parents when reporting parent involvement activities.  相似文献

Shared book reading in families is strongly linked with successful school literacy and thus with identity, belonging and participation in literate societies. From an ‘emergent’ perspective, literacy is recognised as beginning from birth. However, despite exceptions such as research into the UK program, ‘Bookstart’, most research into shared book reading does not focus on babies. In this paper, I report on interactions during shared book reading between 10 babies aged 8–12 months, and their mothers. A detailed examination of linguistic and paralinguistic features of the interactions in the ten mother–baby dyads showed varying individual patterns of language use, discourse structure and paralinguistic interaction. There were also differences in children's dispositions to engage in the activity. The findings from this study have important implications for parent support, prevention/intervention programs, and early childhood settings that include infants and toddlers.  相似文献

The verbal interaction of 12 mothers and 12 fathers with their 19-month-old girls and boys in 2 situations was assessed. In 1 situation, all 3 family members were together, while in the other, each parent was alone with the child. Analyses indicated that when all 3 family members were together, fathers spoke less and took fewer conversational turns than mothers. Other than these significant differences, fathers' speech to their children was not different from mothers' speech on a range of quantitative and qualitative measures.  相似文献

South Korean mothers’ reactions to preschoolers’ aggression and social withdrawal were examined within a cultural framework. With recent social, political, and cultural changes in South Korea, there appears to be a conflict between traditional values and the Confucian heritage on one hand, and Western influence on the other. Mothers (N = 81) of preschoolers from Seoul reported their emotional reactions, causal attributions, socialization strategies, and socialization goals that were endorsed in response to these behaviors. Mothers reacted with negative emotions to both child aggression and social withdrawal, with more anger, disgust and embarrassment for aggression. Mothers also believed that aggression was due to external causes, and less stable than social withdrawal. High-powered and directive strategies were provided more often in response to aggression than social withdrawal, whereas lower-powered/indirect strategies were favored for social withdrawal over aggression. Finally, mothers were more likely to suggest parent- and social-centered goals for aggression, and child-centered goals for social withdrawal, indicating the increasing influence of westernized child developmental information on South Korean mothers. The significance of changing cultural norms and conventions in South Korean mothers’ perceptions and evaluations of maladaptive behaviors in children was supported.  相似文献
