目的:探讨糖尿病合并肺部感染的临床治疗特点。方法:对2009年9月至2012年11月收入本院的81例糖尿病患者合并肺部感染进行回顾性的分析。结果:显效66例(81.48%),缓解9例(11.11%);无效6例(7.4%)。结论:糖尿病患者易发生肺部感染,而血糖控制情况与糖尿病患者合并肺部感染关系极为密切。应监测患者血糖定期做胸部X线检查,以便达到早诊断早治疗的目的。  相似文献

血镁与2型糖尿病视网膜病变的相关研究   总被引:1,自引:0,他引:1  
为了探讨血镁在糖尿病视网膜病变发生中的作用及糖尿病视网膜病变的相关危险因素,测定了30例正常对照组,30例糖尿病无视网膜病变级(NDR组),30例糖尿病合并视网膜病变组(DR组)病人血清镁浓度,并对糖尿病病人行空腹血糖、糖化血红蛋白(HbAlc)、尿微量白蛋白排泄量的测定,记录病程,行眼底检查,结果发现,与正常相比较,血镁降低在NDR组P<0.05,DR组P<0.01。NDR组与DR组比较血镁降低亦有显著性意义(P<0.01),且两组比较病程、空腹血糖、糖化血红蛋白和尿白蛋白排泄是均有显著性差异(P<0.01)。作者认为糖尿病病人有缺镁的倾向,且糖尿病视网膜病变者的血镁比糖尿病无视网膜病变的患者更低,低镁与病程、空腹血糖、糖化血红蛋白、尿白蛋白排量一样,是糖尿病视网膜病变的危险因素。  相似文献

1 临床资料。1.1 一般资料 60例患者中男29例,女31例;年龄32-84年,平均年龄59例;坏疽发生至就诊时间最早3天,最晚32天,平均17天;其中6例并发肾病,5例脑血栓偏瘫,37例心肌缺血。本组患者糖尿病史10年以内者23例,10-20年者31例,发生坏疽后才查出有糖尿病者6例。诱因:修脚感染13例,擦伤感染17例,取暖烫伤感染4例,无明显诱因者26例。对35例湿性坏疽伤口分泌物做细菌培养均为阳性,包括需氧菌和厌氧菌。需氧菌中以肠杆菌属最为多见,其次是柠檬酸杆菌、埃希菌属;厌氧菌中常见有拟杆菌属、嗜纤菌属及梭菌属。实验室检查:血糖均在10.6-27.3mmol/L范围内,尿糖( )23例,( )32例,( )5例。  相似文献

目的:探讨糖尿病足临床治疗方法,为糖尿病足患者提供专业正确治疗和护理。方法:对广西玉林市第一人民医院内分泌科一例成人隐匿性糖尿病足并低蛋白血症进行护理,给予抗感染,控制血糖同时做好心理护理、饮食指导、支持治疗及皮肤护理,落实VSD负压封闭引流技术。结果:经过有效治疗及护理取得较好疗效,治愈出院。结论:细致落实各项基础护理及专科护理,有利于糖尿病足创口愈合,缩短糖尿病足患者住院时间,提高患者生活质量。  相似文献

为了解亲式剖宫拉特点,对我院1999年1月至7月月17例因不同原因行新式剖宫产术患者进行回顾性分析。结果:患者术后疼痛时间平均为14.2h,术后6-8h基本可以下床活动,术中出血量平均141.1ml,术后进食时间12h,术后第5d伤口全部拆线,均甲级愈合,除3例因纠正低蛋白血症、腹水、黄疸而住院时间延长至7d外,其余全部术后第5d出院。结论:新式剖宫产术患者进食早、活动早、疼痛轻、恢复快,既减轻了  相似文献

目的:探讨脑梗死恢复期患者肺炎的危险因素。方法:回顾性分析广西壮族自治区工人医院161例脑梗死恢复期患者的临床资料,对常见暴露因素进行logistic回归统计分析。结果:危险因素鼻饲的OR值为3.740,留置气管切开造口的OR值为4.801,中心静脉置管的OR值为3.635,使用PPI药物的OR值为2.261,低蛋白血症的OR值为3.471。结论:鼻饲、留置气管切开造口、中心静脉置管、使用PPI药物、低蛋白血症是脑梗死恢复期患者发生肺炎的危险因素。  相似文献

2 型糠尿病患者的高胰岛素血症为众多学者所关注。本文对148 例2 型糠尿病患者进行研究,结果显示高胰岛素血症者(A 组)42 例,占28-4 % ,空腹胰岛素为28-7 ±10-2mIu/L,胰岛素/ 血糖比为3-9 ±0-5 ,较对照组(B组) 明显增高,二者有显著性差异;且高胰岛素血症组高血压、冠心病、高血脂、肥胖的发生率明显增高。提示:2 型糖尿病高胰岛素血症患者存在胰岛素抵抗,其X综合症的发生率明显增高  相似文献

目的:探讨2型糖尿病肾病患者血清IGF-1、PEDF、IL-18水平变化及其与不同尿微量白蛋白/肌酐比值的相关性及临床意义。方法:选取在我院住院的2型糖尿病患者223例,依据尿微量白蛋白/尿肌酐比值(ACR)水平分为单纯糖尿病组(A组)82例、微量蛋白尿组(B组)70例、临床蛋白尿组(C组)71例,选取我院体检中心健康体检者30例为对照(NC)组.采用ELISA法检测血清IGF-1、PEDF、IL-18水平,检测ACR、FPG、FINS、HbA1c、BUN、CREA、UA、TG、CHOL、HDL、LDL、Cys-C,计算胰岛素抵抗指数(HOMA-IR)。结果:4组患者在年龄、病程、BMI、ACR、Cys-C、FPG、FINS、HbA1c、BUN、CREA、UA、TG、CHOL、LDL比较,差异有统计学意义(P0.05),HDL比较,差异无统计学意义(P0.05)。2型糖尿病患者血清IGF-1、PEDF、VEGF、IL-18水平明显高于对照组,差异有统计学意义(P0.05)。Pearson相关分析显示,ACR与病程、血糖、HbA1c、CHOL、LDL、Cys-C、IGF-1、PEDF、IL-18、VEGF、BMI呈正相关(P0.05);与年龄、FINS、TG呈负相关(P0.05)。多元线性回归分析显示,IGF-1、PEDF、VEGF、CR EA、LDL、IL-18、HOMA-IR是ACR的独立影响因素(P0.05)。结论:2型糖尿病及2型糖尿病肾病患者早期就存在IGF-1、PEDF、IL-18水平异常,IGF-1、PEDF、IL-18水平增加2型糖尿病及早期肾脏损伤风险。IGF-1、PEDF、IL-18可作为2型糖尿病患者血糖控制情况及2型糖尿病肾病损伤早期发现的有价值的监测指标,预防2型糖尿病患者肾脏损伤,改善患者生活质量。  相似文献

目的:研究低蛋白血症对继发性肺结核免疫相关性影响,实现对继发性肺结核的预防和临床治疗指导。方法:以107例患者作为研究对象,随机将患者分为4组,进行为期8周的测试,每周通过肺部X射线检查方法进行不同康复指数的测定,利用Kinase-Glo Luminescent Kinase Assay Kit测定蛋白脂质体中磷脂和蛋白含量,随后用表面活性剂溶解蛋白脂质体,提取的肺功能测定样本中,用紫外分光光度计测定405 nm波长的吸光度,测试不同负荷下不同温度条件下的调节纤维激酶、甲基氢生成值,分析对继发性肺结核的抑制性抗体。结果:通过相关性分析,提高人体的肺疾病免疫能力,感染特性随年龄的变化与低蛋白血症的恶化程度相同,且男性感染比例与女性比例基本相同,这是由于一般人的免疫系统可以承受和抗击低蛋白血症的恶化,但当低蛋白血症加剧时,免疫力变差,肺部受感染结果相当,产生对继发性肺部疾病的免疫能力。结论:相关性影响分析准确,研究结果在对继发性肺结核的预防和临床实践中具有积极的指导意义。  相似文献

In the present study the efficacies of therapy with insulin, sulphonylurea or insulin + metformin on NIDDM patients are compared. One group which was on a definite dose of insulin therapy, but with uncontrolled diabetes was treated by doubling the insulin dose, a second group whose diabetes was not controlled by glibenclamide was switched over to another sulphonylurea viz; glimepiride and a third group whose diabetes was not controlled by insulin therapy was switched over to a combination therapy with insulin +metformin. After recording their initial blood parameters all the groups were treated as above for 3 months, and the parameters were again determined. The fasting blood sugar and serum lipids of the first group were controlled significantly, but the values were far above normal range. However HDL Cholesterol and atherogenic index were near normal range. In glimepiride treated group, none of the parameters showed any amelioration. In the combined therapy group, control of blood sugar and atherogenic index was more or less the same as for group 1, but hyperlipidemia remained slightly above that of the same. From the findings we can infer that in long term diabetes treatment higher doses of insulin and combined therapy with insulin and metformin may be more beneficial than with low doses of insulin or sulfonyl urea alone  相似文献

Non-insulin dependent diabetes mellitus (NIDDM) is the most rapidly growing chronic metabolic disorder in the world. With advancement in the age and duration of diabetes there is a gradual tendency for the level of blood sugar to rise along with a subsequent increase in the HbA1c as well as in the fasting insulin level. Whether this is an aging process or increased frequency of diabetes is still controversial. The correlation between glucose and insulin sensitivity is consistent with the idea that the degree of chronic hyperglycemia is a cause of excessive insulin resistance in type 2 diabetes, i.e. the insulin resistance which characterizes type 2 diabetes but not nondiabetic subjects matched for age, gender, family history and duration of diabetes. The study comprised a total of 76 subjects out of which 30 were normal, non-diabetic persons and the rest 46 were diabetics with different duration of time in years, after being diagnosed diabetic. Data was analyzed after dividing the subjects into four groups—Group 1 comprised of one year old diabetics, Group 2 was made up of those, who had diabetes, for the past 2–5 years, Group 3 included patients who were diabetic since more than 5 years and Group 4 included non-diabetics as the normal control group. The results obtained indicated that the HbA1c levels showed a significant increase with the duration of diabetes as well as the insulin level showed a significant correlation after adjustment for age, sex and duration of diabetes.  相似文献

The study was designed to find out the correlation between lipid peroxidation, lipoprotein levels to severity and complication of diabetes mellitus. Degree of lipid peroxidation was measured in terms of malondialdehyde (MDA) along with lipid profile and blood glucose in diabetes mellitus. It is categorised into insulin dependent diabetes mellitus (IDDM), non insulin dependent diabetes mellitus (NIDDM) and diabetes mellitus(DM) with complication. Total 112 known diabetic patients and 52 non-diabetic controls were studied. These patients were grouped as per the concentration of fasting blood glucose level i.e. controlled, poorly controlled, and uncontrolled group. There are significant increase in the lipid peroxide (MDA) and lipid profile except HDL cholesterol which is decreased, has been found in all groups as compared to controls In NIDDM group lipid peroxidation was markedly increased than IDDM group and it was higher in DM with complications. Other finding observed was that the level of lipid peroxide increased as per the increase in concentration of blood glucose. The increase lipid peroxidation in the hyperglycemic condition may be explained, as the superoxide dismutase enzyme which is antioxidant becomes inactive due the formation of superoxide radical within the cell. Maximum lipid peroxidation leads to the damage of the tissue and organs which results into complication in diabetic patients. High levels of total cholesterol appear due to increased cholesterol synthesis. The triglyceride levels changes according to the glycemic, control. The increase may be due to overproduction of VLDL-TG. It is concluded that good metabolic control of hyperglycemia will prevent in alteration in peroxidation and the lipid metabolism, which may help in good prognosis and preventing manifestation of vascular and secondary complication in diabetes mellitus.  相似文献

In diabetes, persistent hyperglycemia results in increased production of free radicals especially oxygen free radicals, which can cause cell destruction and tissue injury resulting in cell dysfunction. With the premise that oxidative stress is a major cause of diabetic complications, we conducted a controlled laboratory based investigation on level of lipid peroxide levels in the serum of Type 1 and Type 2 diabetic patients attending Muhimbili National Hospital. From our clinical data it was observed that majority of the patients had higher waist to hip ration and body mass index, which suggests that the patients were either overweight or obese. The enrolled diabetic patients had higher lipid peroxide levels than controls and also Type 2 patients had higher lipid peroxide levels than Type 1 patients. Moreover, patients with known complications had higher lipid peroxide levels than patients without complications. The lipid peroxide levels in the diabetic patients were significantly different from that of the control subjects enrolled in the study. A majority of the diabetic patients had a poorly controlled blood sugar. Our finding hints that despite the fact that diabetic patients in our clinic are on follow up, they are at a risk of developing coronary heart diseases, neuropathy and other secondary diabetic complications.  相似文献

The role of treadmill exercise on blood glucose homeostasis in noninsulin dependent diabetes mellitus (NIDDM) were studied using males between age of 45 and 60 years (X-52), who were clinically and biochemically-confirmed cases of NIDDM were taken into study group. Control group comprised of 10 males between age group of 45 to 60 (X-53) years. All the subjects were assessed by physician and were investigated to confirm diabetic status. The whole study period was extended for 6 weeks. The significant decrease in postprandial blood sugar (44.4 mg% for the study group and 32.2mg% for the control group) with a significant inter group difference (P<0.05) was observed. The mean decrease in fasting blood sugar (39.4mg% for the study group and 27.4mg% for the control group), with a marginal inter group difference (P<0.05) was observed. The treadmill exercise was found to be a definite tool in addition to drug and diet in glycemic control.  相似文献

InAyurvedic system of medicine in India, not only extracts of one plant or the other but also a combination of plant extracts are used for the treatment of diabetes mellitus. The present paper reports the combined effect ofAbroma augusta andCoccinia indica known to be useful for the treatment of diabetes in Ayurveda on the fasting blood sugar, glucose tolerance and lipid profile of Streptozotocin (STZ) induced albino rats. 300mg of water extract of the mixture of dried powdered roots ofA. augusta and leaves ofC. indica in equal proportions was given once daily for 8 weeks. After 8 weeks of treatment of Streptozotocin (STZ) diabetic rats, the fasting blood sugar came down to almost normal value and improvement in glucose tolerance and serum lipid profile were also observed.  相似文献

Neural tube defects are congenital structural abnormalities of the brain and vertebral column resulting from improper or non-timely closure of the neural tube. Prevalence of neural tube defects is reported to be higher among women with diabetes mellitus and obesity. This study was designed to investigate the relation between the presence of dyslipidemia in antenatal patients and the risk of fetal neural tube defects. The present study was an observational, cross-sectional study involving 129 pregnant women in 16 to 18 weeks gestation period. Of these, 80 women had normal pregnancies and 49 were clinically high-risk cases for neural tube defects. Fasting blood samples were analyzed for blood sugar and lipid profile by enzymatic assay and alpha-fetoprotein levels using Enzyme Immunoassay. Alpha-fetoprotein (AFP) values were converted to Multiples of Median (MoM) appropriate for the gestational age. Based on AFP values, women were labeled as screen negative (AFP <2 MoM, n= 102) and screen positive (AFP > 2 MoM, n =27). Screen positive women were further evaluated by ultrasound and 21 women were found to carry a neural tube defects positive pregnancy. Statistical analysis was done on SPSS software. Body weight of the women showed a significant positive correlation with serum triglycerides, plasma sugar and AFP MoM values. A significant difference was observed in serum cholesterol levels (p= 0.038), triglycerides (p=0.001) and plasma sugar levels (p=0.002) between normal women and those with neural tube defects positive pregnancy. The Odds ratio for neural tube defects risk in dyslipidemic cases was 24.23 (CI 4.73 − 148.60) with a relative risk of 12.12. Dyslipidemia especially hypertriglyceridemia was found to be significantly associated with fetal neural tube defects.  相似文献

Microalbuminuria is an established cardiovascular risk indicator in diabetes, hypertension and the general population. There is lack of information on MAU in healthy obese Indian adults and an ongoing debate whether obese adults deserve targeted identification and clinical intervention for MAU and prediabetes. We aimed to screen the healthy obese, young (group I) and middle aged (group II) adults for prevalence of MAU and prediabetes and study its association with Framingham risk score. The study included 50 healthy obese young (20–30 years) and middle aged adults (31–50 years), attending the outpatient clinic of Dept. of Medicine for a duration of 2 months (July–August). The patients were screened for fasting blood sugar, lipid profile and MAU. Of the total patients 28 % had MAU, 32.14 % of which had prediabetes and 33.33 % had diabetes whereas 10 % were normoglycemic. The group I patients had 50 % cases of MAU and group II had 25 % patients with MAU. Group II 63.63 % pre-diabetics. The values of MAU obtained were correlated with age, gender, body mass index, systolic and diastolic blood pressure, FBS, waist to hip ratio using Pearson’s Coefficient (p < 0.05). The 10 year CVD risk calculated using FRS in subjects with MAU was higher as compared to those without MAU. Thus we conclude that Indian, young and middle aged obese adults to be at a risk of prediabetes, MAU and CV risk warranting their routine screening for better clinical outcomes.  相似文献
