ABSTRACT

Approximately 15% of New Zealand children will experience a traumatic brain injury during their school years. Given that educators play an important role in facilitating the successful school re-entry and recovery of these students, it is paramount that educators have adequate knowledge of childhood traumatic brain injury. This study used an online questionnaire to assess knowledge traumatic brain injury among 20 New Zealand educators. It also identified what services educators were aware of within their schools and communities that could assist with the management of students with brain injury, and what further training or information educators required to feel confident in supporting these students in the school environment. Results indicated that although educators possessed sound general understanding of traumatic brain injury, they had some misconceptions regarding the implications and recovery trajectories of these injuries during childhood. Educators reported the need for training relating to diagnostic criteria and strategies for supporting students with traumatic brain injury in the classroom. Providing educators with information and training on how to identify traumatic brain injury and manage the needs of students with traumatic brain injury will allow them to facilitate inclusive and supportive educational opportunities, leading to improved outcomes for these students.  相似文献

目的:探讨大鼠脑损伤后脑微循环障碍、脑缺血及神经组织超微结构的变化规律,为临床改善脑损伤后脑微循环障碍、治疗脑缺血、促进神经功能恢复提供理论依据。方法:采用Feeney's自由落体撞击法建立急性局灶性脑挫裂伤模型,共81只大鼠,随机均分为9组,每组6只行内源性过氧化物酶(EGPO)组织化学染色、脑含水量测定,并进行图象分析。每组3只电镜观察微血管内皮细胞和神经组织超微结构改变。结果:①脑损伤后30min伤区可见出血灶,伤区内无血管染色,伤区周围存在"微无血管区"。"微无血管区"的存在持续至伤后72h。②脑损伤后30min微血管面密度明显下降,伤后48h达到高峰,直到伤后168h才有所恢复,但仍未达到正常水平。③脑损伤后30 min微血管平均光密度明显下降,伤后24h、48h回升,72h再次下降,至168h仍未恢复正常。④脑损伤后30 min,微血管内皮细胞有轻度受损迹象,伤后2h毛细血管腔内有微绒毛形成,伤后6h微绒毛增多。伤后12~72h毛细血管腔明显狭窄。⑤脑损伤后30min,神经细胞超微结构改变不明显,随时间的延长,神经细胞超微结构逐渐恶化,至伤后168h细胞结构完全丧失。结论:EGPO组织化学染色方法能准确反应脑损伤后脑微循环的改变;脑损伤后即发生脑缺血改变,神经细胞、毛细血管内皮细胞的损害继发于脑损伤后的缺血性改变,而脑缺血的发生源于脑损伤后脑微血管结构的破坏和微循环灌注的不足。  相似文献

The purpose of this study is to determine knowledge of traumatic brain injury among educators. Few studies have examined knowledge of traumatic brain injury in this population and fewer still have included a substantial proportion of general education teachers. Examining knowledge of traumatic brain injury in educators is important as the vast majority of children with traumatic brain injury will return to school and many are likely to require support in the short term and some will require special education services in the long term. The Common Misconceptions of Traumatic Brain Injury questionnaire was completed by 94 educators. Participants demonstrated accurate knowledge on a number of questionnaire items associated with the identification of individuals with traumatic brain injury, common socio-emotional effects, and recovery from traumatic brain injury; however, a considerable degree of uncertainly was indicated on several items relevant to the school setting. In addition, substantial misconception rates were noted on only four items including one pertaining to recovery and another concerning the likelihood of additional brain injuries. Knowing someone with traumatic brain injury, participating in training and having taught a student with traumatic brain injury resulted in greater knowledge. Implications of these findings for educators are discussed.  相似文献

OBJECTIVE: To describe physicians' initial forensic impressions of hypothetical cases of pediatric traumatic brain injury (TBI) and to compare the responses of pathologists and pediatricians. METHOD: A survey was administered to physicians who attended workshops on pediatric TBI; were members of two national internet list serves; and were members of the Section on Child Abuse and Neglect of the American Academy of Pediatrics (N=522) and the National Association of Medical Examiners (N=815). The survey included 16 hypothetical case scenarios depicting a pediatric TBI. Participants were asked to categorize their initial forensic impressions of each scenario on a seven-point scale from definitive unintentional to definitive inflicted injury. RESULTS: A total of 570 surveys were completed, including 465 of 1337 surveys (35%) distributed through the regular mail. In 8 of 16 hypothetical cases, a majority of respondents (range, 60-98%) and a majority of the more experienced respondents (range, 59-97%) categorized the hypothetical TBI as either unintentional or inflicted. Pathologists were less likely than the pediatric participants to view these scenarios as inflicted TBI. CONCLUSION: Certain features of TBI cases result in a majority of clinicians categorizing them as inflicted or intentional. In the absence of a confession of inflicted injury, witnessed or verifiable events, severe injuries without explanation or injuries that can be linked clearly to a perinatal period, clinicians were unable to agree on the etiology of the injury. Our results will be helpful to other researchers so that consistent, validated research definitions are used to classify pediatric TBI for research purposes.  相似文献

Objective: To investigate the relationship between the expression of endothelial nitric oxide synthase (eNOS), vascular endothelial growth factor (VEGF) and angiogenesis in primary astrocytoma. Methods: Thirty-seven primary astrocytomas and 4 astrocytic hyperplasia samples were collected and divided into three groups according to histological grade. The expression of eNOS, VEGF and factorⅧrelated antigen (FⅧRAg) were assayed by immunohistochemistry. Microvascular density was assessed by FⅧRAg immunoreactivity. The intensity of immunoreactivity was graded according to the percentage of positive tumor cells. Results: No eNOS and VEGF were expressed in the astrocytes and vascular endothelium in astrocytic hyperplasia. The expression of eNOS or VEGF was light in low-grade astrocytoma and strong in glioblastoma. eNOS expression in astrocytoma was very positively correlated with VEGF. eNOS and VEGF expression in anaplastic astrocytoma was median in contrast to the low grade astrocytoma and glioblastoma. Lower microvascular density was found in low grade astrocytoma than that in higher grade malignant ones. The expressions of eNOS and VEGF were correlated with microvascular density and tumor malignancy. Conclusion: This finding suggests that eNOS and VEGF may have cooperative effect in tumor angiogenesis and play an important role in the pathogenesis of primary astrocytoma.  相似文献

选用32只狗,以钟摆式打击器制成脑挫裂伤模型。并随机分四组,对照组、常规组、常规 低右组和IHD组进行治疗。伤前、治疗前后,分别对颅内压、血及脑脊液乳酸进行监测,实验结束对损伤狗脑进行病理学检查及半定量计分。结果显示:IHD治疗后,ICP及CSF乳酸较其它各组均明显下降,损伤灶周围脑水肿及损伤反应亦明显轻于其它组。本研究揭示,IHD疗法可减轻创伤性脑水肿,降低颅内压。  相似文献

Objectives: To explore the mechanism of development and aggressiveness in gastric carcinomas by investigating the expression and role of CD97 and its cellular ligand CD55 in gastric carcinomas. Methods: Tumor and corresponding normal mucosal tissue, collected from 39 gastric carcinoma patients, were examined by immunohistochemistry and RT-PCR for the expression of CD97 and CD55. Results: CD97stalk was strongly stained on scattered tumor cells or small tumor cell clusters at the invasion front of gastric carcinomas. The expression of CD97stalk was frequently observed in tumors of stage Ⅰ and T1 gastric carcinoma patients. The expression of CD97stalk between Stage Ⅰ and Stage Ⅱ, Ⅲ, Ⅳ specimens showed significant difference (P<0.05), between T1 and T2, T3, T4 specimens also showed significant difference (P<0.05). Specimens with tumor invasion depth limited in mucosa of T 1 specimens showed higher positive CD55 expression than specimens with the same tumor invasion depth in T2, T3, T4 specimens, the expression of CD55 between T1 and T2, T3, T4 specimens was significantly different (P<0.05).There was strong correlation between the distribution patterns of CD97stalk and CD55 on tumor tissues (r=0.73, P<0.05). Signet ring cell carcinomas frequently contained strong CD97stalk and CD55-staining. Conclusions: Our results suggest that CD97stalk is probably involved in the growth, invasion and aggressiveness of gastric carcinomas by binding its cellular ligand CD55. CD97stalk and CD55 could be useful as molecular markers for prognosis and therapy of gastric carcinoma patients.  相似文献

Every year in the UK, a significant number of children sustain a moderate or severe traumatic brain injury (TBI). Children who sustain a moderate or severe TBI have been shown to experience continuing neurological decline in two or more domains up to five years post-injury. Environmental enrichment (EE)—which involves supplementing an environment with appropriately reinforced and rewarded, frequently changing, novel experiences—has been shown to effectively improve social and cognitive functioning in animals with experimental TBI. A growing body of evidence shows EE is also beneficial in the recovery of humans following a TBI. Children who have sustained a moderate or severe TBI often return to school and the educational setting is well-suited for the implementation of EE interventions during the later stages of TBI recovery. This paper reviews a variety of factors pertinent to moderate and severe TBI, neurorehabilitation and EE. It also offers EE considerations for educational professionals who are involved in the development and implementation of school-based interventions for children who have sustained a significant brain injury.  相似文献

In recent years, there has been a growing interest in white matter anatomy of the human brain. With advances in brain imaging techniques, the significance of white matter integrity for brain function has been demonstrated in various neurological and psychiatric disorders. As the demand for interpretation of clinical and imaging data on white matter increases, the needs for white matter anatomy education are changing. Because cross‐sectional images and formalin‐fixed brain specimens are often insufficient in visualizing the complexity of three‐dimensional (3D) white matter anatomy, obtaining a comprehensible conception of fiber tract morphology can be difficult. Fiber dissection is a technique that allows isolation of whole fiber pathways, revealing 3D structural and functional relationships of white matter in the human brain. In this study, we describe the use of fiber dissection in combination with plastination to obtain durable and easy to use 3D white matter specimens that do not require special care or conditions. The specimens can be used as a tool in teaching white matter anatomy and structural connectivity. We included four human brains and show a series of white matter specimens of both cerebrum and cerebellum focusing on the cerebellar nuclei and associated white matter tracts, as these are especially difficult to visualize in two‐dimensional specimens and demonstrate preservation of detailed human anatomy. Finally, we describe how the integration of white matter specimens with radiological information of new brain imaging techniques such as diffusion tensor imaging tractography can be used in teaching modern neuroanatomy with emphasis on structural connectivity. The researchers performed a survey study to determine the effectiveness of collegiate programmes in dispelling common misconceptions about traumatic brain injury (TBI) while preparing undergraduate and graduate students for special education (SpEd) careers. Respondents included 136 undergraduate and 147 graduate SpEd students in their final semesters before obtaining degrees. Each completed an 18‐item true/false survey about TBI and the associated recovery process. Results were compared with survey responses from 318 lay public respondents who participated in a previous study. Two major findings emerged: (a) no significant differences existed in misconception endorsement between SpEd students completing Bachelor's versus Master's degrees; and (b) graduating students in SpEd teacher preparation programmes endorsed similar misconceptions as lay public respondents; hence, these programmes do not appear effective in dispelling common TBI misconceptions. Improving academic preparation for special educators regarding TBI is imperative for effectively identifying, assessing and serving student survivors.  相似文献

The aim of this study was to explore the protective effect of basic fibroblast growth factor (bFGF) on brain injury following global ischemia reperfusion and its mechanisms. Brain injury following global ischemia was induced by four vessels occlusion and systemic hypotension. Twenty-four rabbits were randomized into three groups: group A, only dissection of vessels; group B, intravenous infusion of normal saline after reperfusion for 6 h; group C, 30 μg/kg bFGF injected intravenously at the onset of reperfusion, then infused with 10 μg/(kg·h) for 6 h. Serum neuron specific enolase (NSE), S-100B, tumor necrosis factor-α(TNF-α), interleukin-1 (IL-1), interleukin-8 (IL-8) were measured before ischemia, 30 min after ischemia, 0.5, 1, 3, 6 h after reperfusion. Brain water content was determined and cerebral histopathological damages were compared. NSE and S-100B were increased 1 h after reperfusion and reached their peaks 6 h after reperfusion, but were much higher in group B than those in group C 3, 6 h after reperfusion. In groups B and C, TNF-a was increased after ischemia and IL-1 and IL-8 were increased significantly 0.5 h after reperfusion, then reached their peaks 6 h, 3 h, 6 h after reperfusion respectively. TNF-a and IL-8 at the time points of 1 h and 3 h and IL-1 at 3 h and 6 h in group C were correspondingly lower than those in group B. These indices in group A were nearly unchanged. There were less severe cerebral histopathological damages in group C compared with group B, but no difference in brain water content. It could be concluded that bFGF alleviates brain injury following global ischemia and reperfusion by down-regulating expression of inflammatory factors and inhibiting their activities.  相似文献

目的:探讨颅面部骨折合并颅脑损伤的临床表现及救治要点。方法:回顾性分析60例颅面部骨折合并颅脑损伤的临床资料。结果:入院时已脑疝形成15例,经神经外科手术治疗后,死亡5例,其余患颅脑损伤者治愈。颅面部骨折中上颌骨和下颌骨骨折共27例,20例行小型或微型接骨板内固定,2例行牙弓夹板颌间牵引,5例行可吸收内固定材料固定,效果良好,其他骨折自行愈合。结论:颅面部骨折合并颅脑损伤的临床救治需多学科综合治疗,早期全面系统的检查,减少漏诊率,从而获得满意的治疗效果。  相似文献

To screen and evaluate protein biomarkers for the detection of gliomas (Astrocytoma grade Ⅰ-Ⅳ) from healthy individuals and gliomas from brain benign tumors by using surface enhanced laser desorption/ionization time of flight mass spectrometry (SELDI-TOF-MS) coupled with an artificial neural network (ANN) algorithm. SELDI-TOF-MS protein fingerprinting of serum from 105 brain tumor patients and healthy individuals, included 28 patients with glioma (Astrocytoma Ⅰ-Ⅳ), 37 patients with brain benign tumor, and 40 age-matched healthy individuals. Two thirds of the total samples of every compared pair as training set were used to set up discriminating patterns, and one third of total samples of every compared pair as test set were used to cross-validate; simultaneously, discriminate-cluster analysis derived SPSS 10.0 software was used to compare Astrocytoma grade Ⅰ-Ⅱ with grade Ⅲ-Ⅳ ones. An accuracy of 95.7%, sensitivity of 88.9%, specificity of 100%, positive predictive value of 90% and negative predictive value of 100% were obtained in a blinded test set comparing gliomas patients with healthy individuals; an accuracy of 86.4%, sensitivity of 88.9%, specificity of 84.6%, positive predictive value of 90% and negative predictive value of 85.7% were obtained when patient's gliomas was compared with benign brain tumor. Total accuracy of 85.7%, accuracy of grade Ⅰ-Ⅱ Astrocytoma was 86.7%, accuracy ofⅢ-Ⅳ Astrocytoma was 84.6% were obtained when grade Ⅰ-Ⅱ Astrocytoma was compared with grade Ⅲ-Ⅳ ones (discriminant analysis). SELDI-TOF-MS combined with bioinformatics tools, could greatly facilitate the discovery of better biomarkers. The high sensitivity and specificity achieved by the use of selected biomarkers showed great potential application for the discrimination of gliomas patients from healthy individuals and glioma from brain benign tumors.  相似文献
