Background: Point of care resources make it easier for clinicians to find answers to questions that arise during a clinical encounter. In order to make informed purchase decisions in times of tight budgets, librarians need to have a better understanding of which resources will meet their patrons’ clinical information needs. Objectives: The goal of this study was to assess the content, interface and usability of three point‐of‐care tools: BMJ Point‐of‐Care, Clin‐eguide and Nursing Reference Centre. Methods: A questionnaire designed to gather quantitative and qualitative data was created using Survey Monkey. The survey was distributed to healthcare practitioners in Alberta’s two largest health regions, and the data were analysed for emergent themes. Results: The themes that arose – ease of use, validated content, relevancy to practice – generally echoed those stated in the literature. No one database fared significantly better, due to differing features, content and client preference. Conclusions: Despite the limitations of the survey, the themes that emerged provide a springboard for future research on the efficacy of information resources used at the point of care, and the need for deeper analysis of these recent additions to the medical information market.  相似文献

AIM OF STUDY: The primary focus of this pilot study was to gain a better understanding of the information needs of emergency-care clinicians. The secondary focus was to compare the traditional current practice of information provision within other emergency departments in Greek hospitals, with the new model of clinical librarianship (CL). RATIONALE: Clinical staff in the emergency department deal with a variety of cases, they have no time to visit the library, but need information instantly in their place of work. Clinical decision making in the emergency department setting frequently requires the clinician to obtain additional sources of information and clinical librarians may facilitate this. STUDY DESIGN: The present study focused on two professional groups: medical librarians (group A) and clinicians (consultants, senior registrars, registrars, nurses (group B), working in the emergency departments of two Greek hospitals. The study was organized through a questionnaire survey and some in-depth interviews. RESULTS: This study showed that, for 100% of the clinicians in daily practice, the main information needs arise while treating patients, and that information would help in making patient-care decisions. Clinicians made little use of hospital libraries because they are usually under tremendous time pressure. The main outcome of the study was the use of a clinical librarian as an information provider. Clinical librarians supply information to assist decisions, based on this model. This extends the librarian's role in evidence-based medicine, giving much stronger attention to the relevant evidence in clinical practice. CONCLUSIONS: Nowadays, health services are facing organizational change. The introduction of new technology, and rapid growth of medical knowledge creates a demand for new ways of providing information. Clinical librarian programmes may deliver patients specific information in a timely manner. The mission of the clinical librarian is to facilitate access to quality information which is necessary for improving health, and to act as an informationist in the emergency department.  相似文献

Background: A librarian consultation service was offered to 88 primary care clinicians during office hours. This included a streamlined evidence‐based process to answer questions in fewer than 20 min. This included a contact centre accessed through a Web‐based platform and using hand‐held devices and computers with Web access. Librarians were given technical training in evidence‐based medicine, including how to summarise evidence. Objectives: To describe the process and lessons learned from developing and operating a rapid response librarian consultation service for primary care clinicians. Methods: Evaluation included librarian interviews and a clinician exit satisfaction survey. Results: Clinicians were positive about its impact on their clinical practice and decision making. The project revealed some important ‘lessons learned’ in the clinical use of hand‐held devices, knowledge translation and training for clinicians and librarians. Conclusions: The Just‐in‐Time Librarian Consultation Service showed that it was possible to provide evidence‐based answers to clinical questions in 15 min or less. The project overcame a number of barriers using innovative solutions. There are many opportunities to build on this experience for future joint projects of librarians and healthcare providers.  相似文献

BACKGROUND: The scope of primary care makes knowledge management in this context very challenging, especially as access to information increases and the role of primary care within the NHS expands. OBJECTIVES: This paper reviews the literature on the information needs of primary care clinicians to enable evidence-based decision making. Drawing on the literature, and using a specific example of a clinical informaticist service, some lessons are drawn on the role of information professionals in facilitating evidence-based health care. METHODS: Literature review. Case study of clinical informaticist service. CONCLUSIONS: There are numerous barriers to clinicians engaging with evidence-based practice. However, providing evidence-based information to clinicians' questions in a user-friendly format has the potential to facilitate changes in practice. The successful implementation of information services requires attention to both academic and service dimensions, fostering closer working relationships between clinicians and information professionals.  相似文献

Objectives:Access to high-quality information improves the quality of patient care, but lack of time and sufficient skills in information seeking can prevent access to information by clinicians. To solve this problem, clinical informationists can provide high-quality, filtered information for clinical team members. This study identified the core competencies that clinical informationists need to effectively fulfill their roles on clinical teams.Methods:Participants were selected purposefully from clinicians and medical librarians. Data were collected through semi-structured interviews and analyzed using qualitative content analysis.Results:The authors identified six competencies—communication, research, education and training, domain knowledge, information services, and technology—which together were used to develop a “CREDIT” model of core competencies for clinical informationists.Conclusions:The CREDIT model can be used as criteria for evaluating the performance of clinical informationists as well as for developing and assessing clinical informationist educational programs and curriculums.  相似文献

Background and objectives: Clinicians report difficulties using research in their practices. The aim of the study was to describe needs and preferences for a mental health clinical question‐answering service designed to assist this process. Method: Multi‐disciplinary clinicians participated in a focus group; users of the service supplied feedback. Results: Fifty‐four clinicians received answers to 84 questions about mental health treatments. User ratings showed that the answers had multiple uses: informing health care (43), education (22), staff development (28) and research (12), and were considered useful, clear, relevant and helpful. Focus group participants appreciated critically appraised summaries of evidence and stressed the time‐saving benefit of the service. Clinicians without a medical training were least confident in applying evidence. Attitudes to research were positive, but concern was expressed about its potential misuse for political purposes. This appeared to arise from an ambiguity around the term ‘insufficient evidence’, which participants felt is widely misinterpreted as ‘evidence of no effect’. Conclusions: A highly valued, responsive service has been developed. A range of clinicians find critically appraised summaries of research useful. Education about the use of research may help clinicians to be more evidence based.  相似文献

Background: Hospital pharmacists need access to high‐quality information in order to constantly update their knowledge and improve their skills. In their modern role, they are expected to address three types of challenges: scientific, organizational and administrative, thus having an increased need for adequate information and library services. Objectives: This study investigates the information‐seeking behaviour of public hospital pharmacists providing evidence from Greece that could be used to encourage the development of effective information hospital services and study the links between the information seeking behaviour of hospital pharmacists and their modern scientific and professional role. Method: An empirical research was conducted between January and February 2010 with the development and distribution of a structured questionnaire. The questionnaire was filled in and returned by 88 public hospital pharmacists from a total of 286 working in all Greek public hospitals, providing a response rate of 31%. Results: The hospital pharmacists in Greece are in search of scientific information and, more particularly, pharmaceutical information (e.g., drug indications, storage, dosage and prices). The Internet and the National Organization of Medicines are their main information sources, while the lack of time and organized information are the main obstacles they have to face when seeking information. Conclusions: The modern professional role of hospital pharmacists as invaluable contributors to efficient and safer healthcare services may be further supported through the development of specialized libraries and information services within Greek public hospitals.  相似文献

Objectives: To investigate the extent to which junior doctors in their first clinical positions retained information literacy skills taught as part of their undergraduate education. Method: Participants drawn from different training cohorts were interviewed about their recall of the instruction they had received, and their confidence in retrieving and evaluating information for clinical decision making. They completed a search based on a scenario related to their speciality. Their self‐assessment of their competency in conducting and evaluating a search was compared with an evaluation of their skills by an experienced observer. Results: Most participants recalled the training they received but had not retained high‐level search skills, and lacked skills in identifying and applying best evidence. There was no apparent link between the type of training given and subsequent skill level. Those whose postgraduate education required these skills were more successful in retrieving and appraising information. Conclusion: Commitment to evidence‐based medicine from clinicians at all levels in the profession is needed to increase the information seeking skills of clinicians entering the work force.  相似文献

Clinical medical journals have not been effective in meeting the information needs of practitioners and bridging the gap between clinical research and practice. The slow adoption of results of clinical research is at least partly due to the failure of clinical journals to disseminate information in a way that would motivate practitioners to change practice. Although implementation is primarily a local process, medical journals are in a unique position to advance implementation by modifying their focus and adjusting their contents. Strategies that may be useful include publication of pre‐appraised evidence summaries and ‘clinical bottom‐lines’ and giving importance to systematic reviews and large evaluative research articles as they represent higher levels of evidence and have greater potential to change practice. Clinical journals should encourage researchers to consider how and by whom the findings will be used and provide information on implications for implementation such as possible strategies that may work, cost‐effectiveness, side‐effects and potential barriers to implementation. Medical journal publishers should explore ways to cooperate so that findings of landmark clinical trials could be shared thus reducing the ‘scatter’ of medical information. Electronic media offers numerous advantages such as quick accessibility and linking of information, and medical journals should capitalize on such innovations. There is a paradigm shift in health care practice as evidence is consciously and explicitly incorporated into individual patient care. Medical journals need to change to reflect this change in practice and provide practitioners with valid and relevant information.  相似文献

Background: The Government's nursing recruitment campaign has widened access for those wishing to enter the profession, resulting in some students entering university with clinical experience but little experience of academic writing or using libraries. For these students, the library and study advice staff have an important role to play in helping them acquire appropriate information literacy and study skills. Objective: To evaluate the impact of workshops supplementing online instruction in study skills on students’ study and writing skills, and level of information literacy. Methods: A small group of pre‐registration nursing students identified as needing support with information literacy and study skills participated in focus groups to identify areas of concern, and were offered four remedial workshops. A follow‐up focus group explored whether the students’ needs had been addressed. Further evaluation of the workshops’ impact was completed by diagnostic testing within the University's information literacy programme, Skills Plus. Outcomes: The results showed that all students who attended at least one workshop improved their academic grade in their next assignment. Qualitative data indicated that the students’ confidence level and information literacy, including referencing skills, had improved. Conclusion: Evaluating the impact of this intervention has provided the evidence to demonstrate the value of this additional support.  相似文献

OBJECTIVE: To determine the information needs and support required by AHPs whilst building services based on clinical effectiveness. METHODS: A qualitative approach using action research in co-operation with a Rehabilitation Department Clinical Effectiveness group, leading to interviews with eight therapists. The sampling strategy was purposive. RESULTS: Therapists have well-developed professional networks, but are unsure how to access traditional library areas. Library training can stop AHPs in asking for help or services, whilst the library is thought to be very different to their everyday work, but confidence in using the library service can be acquired by getting to know a librarian. A departmental-based service was thought to provide better access to, and understanding of, information through mentorship, whilst also providing up-to-date evidence so that clinicians can concentrate on their clinical work whilst being more clinically effective. CONCLUSIONS: When seeking information, AHPs are more likely to use professional networks than libraries. However, when working alongside an AHP, a librarian can upgrade information-seeking skills through mentorship, whilst named library contacts can make it easier to access the library for help.  相似文献

The purpose of this study was to examine the relationship between college students’ communication motives (i.e., relational, functional, participatory, excuse making, sycophantic) and their use of information‐seeking strategies (i.e., overt, indirect, third party, testing, observing). Participants were 149 students enrolled in an introductory communication course at a Mid‐Atlantic university. Results indicated that (a) students who communicate for the sycophantic, relational, and participatory motives use the indirect and observing information‐seeking strategies and (b) students who communicate for the functional communication motive use the overt information‐seeking strategy, but do not use the testing information‐seeking strategy. Future research should examine whether college students use information‐seeking strategies with their classmates and the impact of the use of these strategies on their learning experience.  相似文献

Hospital health sciences libraries represent, for the vast majority of health professionals, the most accessible source for library information and services. Most health professionals do not have available the specialized services of a clinical medical librarian, and rely instead upon general information services for their case-related information needs. The ability of the hospital library to meet these needs and the impact of the information on quality patient care have not been previously examined. A study was conducted in eight hospitals in the Chicago area as a quality assurance project. A total of 176 physicians, nurses, and other health professionals requested information from their hospital libraries related to a current case or clinical situation. They then assessed the quality of information received, its cognitive value, its contribution to patient care, and its impact on case management. Nearly two-thirds of the respondents asserted that they would definitely or probably handle their cases differently as a result of the information provided by the library. Almost all rated the libraries' performance and response highly. An overview of the context and purpose of the study, its methods, selected results, limitations, and conclusions are presented here, as is a review of selected earlier research.  相似文献

This study investigates attitudes to ‘lay’ or ‘plain‐English’ summaries of open access (OA) journal articles in the context of engaging the public with medical research. It places lay summaries in the wider contexts of patients' information‐seeking behaviour and OA publishing activities. It reports the results of qualitative research involving two stakeholder groups: employees of organizations with a stake in communicating OA medical research to the public, and members of the public who have experience of accessing online medical research. It shows that patient access to the research literature is seen as one of a number of important sources of information that can help them manage their health conditions as ‘informed patients‘. However, accessing the literature was reported to be problematical, particularly because of paywalls, and there were also difficulties in using it, including language barriers. Lay summaries were seen to make a helpful contribution to improving patient access to information. There is, however, a clear need to gather more evidence about the costs and benefits of such an approach and also on the potential ways in which OA can create benefits for the general public.  相似文献

Purpose: To characterise the training needs of those providing clinical question answering services (CQAS). Participants: Seventeen specialist UK staff working in CQAS, 21 from general health library UK staff and eight international respondents. Methodology: A literature review examined documented training needs for CQAS staff. A follow‐up questionnaire examined prior training and experience and identified training needs for the surveyed staff. Results/outcomes: Ninety per cent of CQAS staff had worked in health services for 3 years or longer. Training received in preparation for the CQAS role comprised literature searching (including the PRECEPT/ADEPT and cochrane library courses) and critical appraisal. Skills considered ‘essential’ for clinical question answering were ‘literature searching’ (100%), ‘understanding the context of clinical questions’, ‘bibliographic databases’, ‘evidence‐based sources’ and ‘the Internet’ (all 93%). Main training needs for specialist CQAS staff include management and organisation of CQAS and technical skills in interpretation and presentation. Discussion/conclusion: CQAS staff require a formal training programme. Most CQAS staff considered that this should be a mandatory requirement.  相似文献

Medical librarians typically contribute to patient care from the confines of the library in response to practitioners' requests for literature searches, information pearls, and evidence-based information. While this model has long served the needs of active clinicians, the learning environment of residency programs is ripe for innovative librarian involvement. This paper illustrates how the librarian can become part of a clinical team outside of the library and provide real-time reference services while teaching by example. Benefits of the alliance include the integration of disciplines to provide enhanced resident learning and improved patient care.  相似文献

This article investigates the information needs and seeking habits of lawyers in England in the United Kingdom. Specifically, it seeks to ascertain the type of information lawyers require to meet their needs; the reasons prompting them to seek information; to establish from where lawyers obtain information; and to determine the factors that influence their information needs and seeking habits. This study was carried out as part of a wider research project for a doctorate programme. Data collection was carried out by semi-structured interviews. The participants comprised lawyers, essentially solicitors, in London and the Midlands.It was found that lawyers' information needs are greatly influenced by the nature of the work they do. It was noted that experience has a considerable influence on their needs: it is generally assumed that experienced lawyers do not require as much information support as newly qualified lawyers. The lawyers' needs for information in turn influences their information seeking habits. It was noted that the card catalogue as an information retrieval tool is fast disappearing from law libraries in the UK. Its ultimate future is likely to be determined by the needs of small firms which are presently unable to afford automated systems.The study confirms the validity of the findings of earlier studies conducted in Canada, the United Kingdom and the United States. It appears that very little has changed since these studies were published. The legal profession continues to rely heavily on printed media. Electronic media is used only as a last resort. Practising lawyers continue to rely on their junior colleagues for legal research. Many are apathetic to legal research. Despite the fact that law is a dynamic subject, the legal discipline remains a conservative profession.  相似文献
