We report a case of a diabetic, heterozygote with near normal hematology, marginally low level of hemoglobin A2(HbA2) having an increased level of hemoglobin F(HbF) that was pancellularly distributed among the red cells. BioRad DiaSTAT measurements
gave a high glycated hemoglobin A1c(HbA1c) of 31.5% and the BioRad Variant analyzer recorded an HbA1c value which was very
low, in discordance with the detected blood glucose levels. Flow cytometry and polymerization chain reaction (PCR) based studies
were carried out which revealed the case to be that of the common hereditary persistence of fetal hemoglobin (HPFH)-3, an
Asian Indian mutation. Fructosamine estimation and HbA1c by Boronate affinity chromatography were able to resolve the discordant
value detected and was able to confirm the diabetes status. The case would have been a diagnostic dilemma, if reported without

Thyroid function tests are very important for the diagnosis and monitoring of patients with thyroid dysfunction. The guidelines
recommend serum thyroid stimulating hormone (TSH) as the single most reliable test to diagnose all common forms of hypothyroidism
and hyperthyroidism. The aim of this study was to analyze the ordering pattern for thyroid function tests by physicians and
the analysis of results based on the clinical history. The mean age of the patients was 32.5 ± 6.5 years. Majority of samples
(87.7% of total) were received from the departments of Medicine and Gynae. Thyroid profiles (47.5%) were ordered more frequently
as compared to TSH only (46%). There was no significant difference in the percentage of normal reports for both types of tests.
77.8% of TFT and 76.6% of TSH samples had results within the reference range. The percentage of abnormal results was 13.7%
in the patients who were screened for thyroid disorders. There is a need to redefine the case definition for thyroid dysfunction

Ensuring quality of laboratory services is the need of the hour in the field of health care. Keeping in mind the revolution
ushered by six sigma concept in corporate world, health care sector may reap the benefits of the same. Six sigma provides
a general methodology to describe performance on sigma scale. We aimed to gauge our laboratory performance by sigma metrics.
Internal quality control (QC) data was analyzed retrospectively over a period of 6 months from July 2009 to December 2009.
Laboratory mean, standard deviation and coefficient of variation were calculated for all the parameters. Sigma was calculated
for both the levels of internal QC. Satisfactory sigma values (>6) were elicited for creatinine, triglycerides, SGOT, CPK-Total
and Amylase. Blood urea performed poorly on the sigma scale with sigma <3. The findings of our exercise emphasize the need
for detailed evaluation and adoption of ameliorative measures in order to effectuate six sigma standards for all the analytical

This paper presents a simple and efficient design method for cosine-modulated filter banks with prescribed stopband attenuation, passband ripple, and channel overlap. The method casts the design problem as a linear minimization of filter coefficients such that their value at ω=π/2M is 0.707, which results in a simpler, more direct design procedure. The weighted constrained least squares technique is exploited for designing the prototype filter for cosine modulation (CM) filter banks. Several design examples are included to show the increased efficiency and flexibility of the proposed method over the exiting methods. An application of the proposed method is considered in the area of sub-band coding of the ECG and speech signals. 相似文献

Surface features such as uneven playing surfaces, low impact absorption capacity and inappropriate friction/traction characteristics are connected with injury prevalence whereas force impact during foot strike has been suggested to be an important mechanism of intravascular haemolysis during running. We aimed to evaluate intravascular haemolysis during running and compare the effect of running on two different types of surfaces on haemolysis. We selected two surfaces (asphalt and grass) on which these athletes usually run. Participants were randomly assigned to group A (asphalt) or group B (grass) with 10 athletes in each group. Each athlete completed one hour of running at the calculated target heart rate (60-70%). Venous blood samples were collected before and immediately after running. We measured unconjugated bilirubin (UBR) (mg · dl(-1)), lactate dehydrogenase (LDH) (μ · ml(-1)), haemoglobin (g · l(-1)) and serum ferritin (ng · ml(-1)) as indicators of haemolysis. Athletes who ran on grass demonstrated an increase in the haematological parameters (UBR: P < 0.01, LDH: P < 0.05) when compared to athletes who ran on asphalt (UBR: P < 0.05, LDH: P = 0.241). Our findings indicate that intravascular haemolysis occurs significantly after prolonged running. Furthermore, we conclude that uneven grass surface results in greater haemolysis compared to asphalt road. 相似文献

To study oxidative stress in systemic lupus erythematosus (SLE) by estimating serum oxidised LDL (OxLDL), 8-hydroxy-2′-deoxyguanosine (8-OHdG), malondialdehyde (MDA), and total anti-oxidant status and to correlate with SLE disease activity and disease damage. Eighty SLE patients satisfying the Systemic Lupus International Collaborating Clinics/American College of Rheumatology (SLICC/ACR) 2012 criteria and 80 healthy controls were studied. Exclusion criteria were infections, renal insufficiency, other connective tissue diseases, drug-induced lupus, smoking, alcohol consumption. Disease activity was measured by SLE disease activity index-2 K (SLEDAI), disease damage was quantified by SLICC-Damage Index (SDI). Sera was tested for OxLDL, 8-OHdG, and total antioxidant status (TAS) by double-antibody sandwich ELISA; MDA measured by Colorimetric assay. Oxidative stress markers were compared between group1- controls, group 2-mildly active SLE (SLEDAI ≤ 5), group 3- moderate to highly active SLE (SLEDAI ≥ 6). SLE patients had significantly higher MDA, 8-OHdG and lower TAS when compared to healthy controls, while OxLDL was similar in the three groups. MDA, 8-OHdG were significantly higher, TAS lower in group 3 compared to group 2. MDA had positive correlation with SLEDAI, TAS negatively correlated with SLEDAI. SLE with neuropsychiatric manifestations, vasculitis, anti-sdDNA antibodies had higher MDA, MDA/TAS ratio. SLE patients with thrombocytopenia, and vasculitis had higher OxLDL. Only OxLDL was significantly higher in those patients who have SDI > 1. SLE patients have increased oxidative stress measured by increases in MDA, 8-OHdG, and lower total antioxidant status that was associated with disease activity and some disease manifestations. However only OxLDL was associated with damage. 相似文献

Methionine synthase reductase (MTRR) is an important enzyme of the folate/homocysteine pathway. It is responsible for regulation of methionine enzyme by reductive methylation. A common variant A66G is reported in the FMN-binding domain of the MTRR gene, which leads to substitution of isoleucine by methionine (I22M) in MTRR enzyme with reduced activity. Reduced catalytic activity of enzyme leads to high homocysteine concentration in blood and increases risk for numerous diseases. The frequency of A66G polymorphism varies in different ethnic groups. The present study has been designed to evaluate the frequency of MTRR A66G gene polymorphism in the Eastern UP population by PCR–RFLP method. Along with this we also performed a meta-analysis to evaluate the global prevalence of this polymorphism. Databases were screened to identified the eligible studies. The prevalence of the G allele and GG genotype was determined by the use of prevalence proportion with 95% CI. Open meta-analyst software was used for the meta-analysis. Total 1000 blood samples were analyzed, the frequencies of A and G alleles were 0.35 and 0.65 respectively. Meta-analysis results revealed that the prevalence of G allele and GG genotype were 49.4% (95% CI 40.6–58.1, p ≤ 0.001) and 24.3% (95% CI 17.8–30.9, p ≤ 0.001) respectively. In sub-group meta-analysis, the lowest frequency of G allele was found in South America (32.7%; 95% CI 14.1–51.3, p ≤ 0.001), and highest in Asia (56.4%; 95% CI 39.5–73.3, p ≤ 0.001). The results of the meta-analysis showed that the Asian population has the highest frequency of G allele and highest frequency of the GG genotype was found in the European population. 相似文献

In this study, a simple roller burnishing tool was made to operate burnishing processes on A356/5%SIC metal matrix composite fabricated by electromagnetic stir casting under different parameters. The effects of burnishing speed, burnishing force and number of burnishing passes on the surface roughness and tribological properties were measured. Scanning electron microscopy (SEM) graphs of the machined surface with PCD (insert-10) tool and roller burnished surface with tungsten carbide (WC) roller were taken into consider- ation to observe the surface finish of metal matrix composites. The mechanical properties (tensile strength, hardness, duc- tility) of A356/5%SIC metal matrix composites were studied for both unburnished samples and burnished samples. The results revealed that the roller burnished samples of A356/ 5%SIC led to the improvement in tensile strength, hardness and ductility. In order to find out the effects of roller bur- nishing process parameters on the surface roughness of A356/ 5%SIC metal matrix composite, response surface methodol- ogy (RSM) (Box-Behnken design) was used and a prediction model was developed relevant to average surface roughness using experimental data. In the range of process parameters, the result shows that roller burnishing speed increases, and surface roughness decreases, but on the other hand roller burnishing force and number of passes increase, and surface roughness increases. Optimum values of burnishing speed (1.5 m/s), burnishing force (50 N) and number of passes (2) during roller burnishing of A356/5%SIC metal matrix com- posite to minimize the surface roughness (predicted 1.232 μm) have been found out. This pilot study set out to examine the impact of a regular practice of yoga within schooling curricular hours on the well-being and behaviors of atypically developing children. The design of this study was based on a qualitative hermeneutic-phenomenology approach. The intervention consisted of 5 weekly sessions of yoga of 15 minutes each and composed of breathing techniques (pranayama), postures (pranayama), and lying down relaxation exercises (yoga nidra). The research site was a special needs school in South Auckland, New Zealand in an area characterized by poor socio-economic living conditions with a composite range of different ethnic groups. The findings of this study suggest that a regular practice of yoga can have numerous benefits for children on the Autism Spectrum Disorder (ASD). They found yoga to be a fun-filled and engaging experience. They improved significantly in their flexibility levels and general physical health. Emotionally, the children reported that the breathing techniques allowed them to feel calmer and more relaxed. The children felt that incorporating a regular practice of yoga within formal curriculum will be another effective tool to enhancing their overall health and well-being. 相似文献
