Vitamin B12 deficiency can present with neurologic and psychiatric symptoms without macrocytic anaemia. We describe a case of late-onset cobalamin C deficiency which typically presents with normal serum vitamin B12 concentrations, posing an additional diagnostic challenge. A 23-year-old woman with decreased muscle strength and hallucinations was diagnosed with ‘catatonic depression’ and admitted to a residential mental health facility. She was referred to our hospital for further investigation 3 months later. Heteroanamnesis revealed that the symptoms had been evolving progressively over several months. Magnetic resonance imaging (MRI) of the brain showed diffuse symmetrical white matter lesions in both hemispheres. Routine laboratory tests including vitamin B12 and folic acid were normal except for a slight normocytic, normochromic anaemia. Over the next 6 weeks her symptoms deteriorated, and she became unresponsive to stimuli. A new MRI scan showed progression of the white matter lesions. The neurologist requested plasma homocysteine (Hcys) which was more than 8 times the upper limit of normal. Further testing revealed increased methylmalonic acid and the patient was diagnosed with adult-onset cobalamin C deficiency. This case illustrates that Hcys and/or methylmalonic acid should be determined in patients presenting with neuropsychiatric symptoms suggestive of vitamin B12 deficiency with a normal serum vitamin B12 to rule out a late-onset cobalamin C deficiency.  相似文献

One of the main shortcomings of multimedia in open learning environments is the use of monitors. The monitor as currently supplied with the personal computer originates from the world of television and video. Television equipment is extremely suitable for the transmission of one-way messages and also for the presentation of film, video and speakers ('talking heads'). It is also suitable for giving instruction and showing other educational programmes. But the monitor is built as a one-way message medium.
Pictures and images are presented but disappear as soon as other pictures are presented. Thus, a certain measure of short and long memory is required as well as the interpretation of the data and so it remains a linear medium. The organisation of the monitor display is a constant worry in open learning and working environments, because in practice it is continually changing.
This article analyses these shortcomings and describes some solutions according to the concepts and theories of Min, Koopal, Gritter, Struyker Boudier, Coleman, Miltenburg and Van Schaick Zillesen, developed over the last decade (Min, 1992).  相似文献

Abstract

Some of the basic findings of Piaget's study on spatial knowledge in Genevan children are contrasted to Navajo spatial knowledge: there is no unique spatial «primitive» in Navajo knowledge and each and every notion is constituted in a different way than its Genevan correlate. The total system of Navajo knowledge shows a structure of interrelated and codetermining notions which differs markedly from the unilinear logical-deductive structure in the Piagetian model. In the second half of the paper the investigation turns to education and schooling. Here a position is taken against the (Piaget-based) rationalistic education policy one finds in the New Math-movement. Instead, some of the principles of Freudenthal are explored in a non-western setting: children should reinvent geometry on their own, in the terms of their culture. To that end, a curriculum for geometry teaching in a Navajo bicultural school is presented, focusing on the schooling in geometric concepts within the Navajo language and with full respect for the Navajo spatial representations in pre-school children.  相似文献

Prior to the exhibition Portrait-making, Rodin and his models (2009), the Rodin museum wanted to restore two busts of Hanako and Clemenceau. Interestingly, these two sculptures contain pieces of modern modeling materials (MMMs) invented at the end of the nineteenth century as an alternative to clay or waxes. The poor state of conservation of the two portraits made any handling and exhibition impossible. Accordingly, the purpose of this article is twofold: to contribute to technical art history and conservation. Elemental and chemical analyses were done on samples from 12 sculptures (SEM–EDX, FTIR, GC–MS, GC–FID, XRD, synchrotron-based µXRF, µXANES, and µFTIR) aimed at identifying the composition of MMMs used by Rodin on plaster sculptures and establishing hypotheses about the origins of their degradation. This thorough study of their composition and degradation was necessary to implement an appropriate restoration plan. The development of conservation protocols adapted to such materials is rarely documented. Different tests were performed on mock-ups (pH, solubility, adhesion, consolidation, and cleaning). In particular, a protocol based on laser cleaning was developed and successfully applied to remove superficial dust and crusts so that the sculptures regained their original aspect.  相似文献

ABSTRACT

Two Spiritelli attributed to Donatello have been extensively studied from a technical point of view, including X-radiography, alloy analysis, characterization of the core, and visual observations on the interior and exterior of the statues. The results of this technical study reveal how the two statues were cast and document the artist’s technical choices. These new insights offer additional information to address questions regarding their attribution, production context, and conservation history. First, the technical study confirmed that the statues were made by the same workshop and argues against the prior proposal of different attributions for the two figures. Second, the results strongly suggest that the statues have never been separated since they were taken off Luca Della Robbia’s Cantoria. Finally, study of the two Spiritelli offers further evidence of the heterogeneity and ingeniousness of Donatellos bronze artistic production, and contributes to the ongoing debate whether the artist himself was involved in the technical aspects of his sculpture.  相似文献

The history of education and schooling is characterised by not only the attainment of results but also ongoing critique of the existing situation in education and schooling. Developers and researchers are, as a result, constantly seeking new strategies and methods for the improved implementation of innovations in actual educational practice. The central question is which factors and which conditions facilitate the effects of innovations. In an attempt to answer this question, many educators will not so much be concerned with objective structural factors such as available funds, classmates, et cetera. They will be concerned, rather, with the questions of which socially constructed realities and which particular forms of involvement on the part of teachers, school administrators, and counsellors facilitate successful innovation. In our search for an answer to these questions, moreover, we will be guided by a cultural-individual perspective on innovation. Traditional innovative strategies – and, in particular, large scale strategies – seem not always suitable to deal with today's educational problems. Alternative scenario's for the management of innovations are considered.  相似文献
