Vitamin B12 deficiency can present with neurologic and psychiatric symptoms without macrocytic anaemia. We describe a case of late-onset cobalamin C deficiency which typically presents with normal serum vitamin B12 concentrations, posing an additional diagnostic challenge. A 23-year-old woman with decreased muscle strength and hallucinations was diagnosed with ‘catatonic depression’ and admitted to a residential mental health facility. She was referred to our hospital for further investigation 3 months later. Heteroanamnesis revealed that the symptoms had been evolving progressively over several months. Magnetic resonance imaging (MRI) of the brain showed diffuse symmetrical white matter lesions in both hemispheres. Routine laboratory tests including vitamin B12 and folic acid were normal except for a slight normocytic, normochromic anaemia. Over the next 6 weeks her symptoms deteriorated, and she became unresponsive to stimuli. A new MRI scan showed progression of the white matter lesions. The neurologist requested plasma homocysteine (Hcys) which was more than 8 times the upper limit of normal. Further testing revealed increased methylmalonic acid and the patient was diagnosed with adult-onset cobalamin C deficiency. This case illustrates that Hcys and/or methylmalonic acid should be determined in patients presenting with neuropsychiatric symptoms suggestive of vitamin B12 deficiency with a normal serum vitamin B12 to rule out a late-onset cobalamin C deficiency.  相似文献

This article provides an overview of an innovative development programme in the field of second chance education for young Maori people who for various social and cultural reasons have failed at school and who have consequently low aspirations and little expectations for the future. In order to improve their disadvantaged position within the European-dominated society of New Zealand the Community Training Centre discussed in this article has developed a new approach to education and training by attempting to empower trainees so that they become aware of alternatives and can make choices about their future.  相似文献

Abstract This paper discusses issues raised by research into people's views of science and scientists, and the implications for interpretative forms in museums. The principles proposed here are based on a series of meetings that looked at the use of narratives in science and the responses of potential visitors from different cultural groups to ideas for narrative signage. Signage design can help people connect to science content by relating practical and theoretical knowledge, crafting explanations, understanding the nature of the medium, and conveying a message about science. The use of narrative form to design experiential guides opens up the possibility of changing a visitor's relationship to the traditional text encountered in museums.  相似文献

This article focuses on the effectiveness of Flemish (Belgian) public libraries in reaching a large and socially diverse public. A statistical model is developed which incorporates unique data gathered through a large-scale visitor survey, a survey of librarians and municipal demographic information. By taking into account both characteristics of the public library and its surrounding community, the impact public libraries themselves have on the number and composition of their visitors can be clearly assessed. Factors such as the percentage of CDs, videos, and DVDs in the collection, and the opening hours are positively associated with the overall number of visitors and also the amount of male visitors. However, the model reveals a rather limited impact of the library on the percentage of lower-educated visitors; this percentage seems to be largely determined by municipality characteristics. The results hope to contribute to a discussion on library effectiveness and the current demands being placed on libraries and their staff.  相似文献

The study investigated: (a) the congruency hypothesis, and (b) the member characteristics hypotheses relating to ability and gender, of Chelladurai's (1978) Multidimensional Model of Leadership. One hundred forty-eight tennis players (77 women) competing at the NCAA Division I and II Tennis Championship level participated in the study. Results indicated athlete satisfaction (Athlete Satisfaction Scale; Riemer & Chelladurai, 1998) was not dependent on the congruence between preferred and perceived leadership behavior. Other results indicated that an athlete's level of ability did affect preferences for leadership behavior. Further, while athlete gender was responsible for some variance in preferences for autocratic behavior and positive feedback behavior, the gender of the athlete's coach had a significant effect on the athlete's preferences for social support behavior.  相似文献
