This article draws on a notion of the hospital as a complex service provider and healthcare system hub in order to examine the question of innovation in hospitals. It puts forward a relatively simple framework for analysing hospital output, in which the medical function is incorporated into a complex system of other constituent services that make use of different types of technologies. This analytical framework is used to reveal the multiple sources of innovation and the main organising principles driving innovation within hospitals.  相似文献

This paper discusses the role of public demand, in the form of a government procurement policy, in generating innovative solutions for healthcare infrastructure. It considers the effects of the project delivery system (planning, finance, construction and operation) for new hospitals on design innovation. It focuses on hospitals built under the UK's private finance initiative (PFI), which was partly introduced to inject increased innovation into hospital delivery. We use case studies of six early PFI hospitals to argue that the introduction of PFI has increased the complexity at the interface between project delivery and hospital operational functions. The result is a project delivery model which yields less innovative outcomes and produces facilities that might not be able to cope with future changes in demand. The paper suggests that new public procurement models do not automatically provide efficiency and innovation benefits.  相似文献

This study examines how healthcare communication professionals (HCPs) cope with workplace stress while working in an inherently stressful industry. In-depth, semi-structured interviews were conducted with 17 HCPs (14 females, 3 males) from seven nonprofit and for-profit hospitals in the midwestern and eastern regions of the United States. Communication positions held by participants included marketing, media/community relations, public information, public relations, corporate communication, and social media. Interviews revealed HCPs use four strategies to cope with workplace stress: prioritizing, seeking colleague support, keeping perspective, and being active. HCPs primarily relied on employee-centered coping strategies, even though all sources of stress were organizationally-based (i.e., workload, lack of resources, client confidentiality). The majority of HCPs emphasized the benefits of using problem-centered (i.e., prioritizing) and emotion-centered (i.e., seeking colleague support) coping strategies. Although mentioned less often, HCPs utilized more avoidance-centered coping strategies (i.e., keeping perspective, being active). More importantly, they highlighted the benefits of using avoidance-centered strategies to help them mentally and physically disengage from workplace stress. Hospital administration should reassess if, and how, they help HCPs effectively cope with workplace stress (i.e., outlets, programs, professional development opportunities) in order to improve the well-being of HCPs and increase the likelihood of organizational success.  相似文献

This paper evaluates the impact of varying implementation of electronic lab order entry management (eLAB) system strategies on hospitals’ productivity in the short run. Using the American Hospital Association's Annual Surveys for 2005–2008, we developed hospital productivity measures to assess facilities’ relative performances upon implementing eLAB systems. The results indicate that different eLAB system implementation strategies were systematically related to changes in hospitals’ relative productivity levels over the years studied. Hospitals that partially implemented an eLAB system without completing the roll-out experienced negative impacts on productivity. The greatest loss in short-term productivity was experienced by facilities that moved from having no eLAB system to a complete implantation in one year—a strategy called the “Big Bang”. The hybrid approach of a limited introduction in one period followed by complete roll-out in the next year was the only eLAB system implementation strategy associated with significant productivity gains. Our findings support a very specific strategy for eLAB system implementation where facilities began with a one-year pilot program immediately followed by an organization-wide implementation effort in the next period.  相似文献

This study examines judgments made by hospital-based child protection teams (CPTs) when determining if there is reasonable suspicion that a child has been maltreated, and whether to report the case to a community welfare agency, to child protective services (CPS) and/or to the police. A prospective multi-center study of all 968 consecutive cases referred to CPTs during 2010–2011 in six medical centers in Israel. Centers were purposefully selected to represent the heterogeneity of medical centers in Israel in terms of size, geographical location and population characteristics. A structured questionnaire was designed to capture relevant information and judgments on each child referred to the team. Bivariate associations and multivariate multinomial logistic regressions were conducted to predict whether the decisions would be (a) to close the case, (b) to refer the case to community welfare services, or (c) to report it to CPS and/or the police. Bivariate and multivariate analyses identified a large number of case characteristics associated with higher probability of reporting to CPS/police or of referral to community welfare services. Case characteristics associated with the decisions include socio-demographic (e.g., ethnicity and financial status), parental functioning (e.g., mental health), previous contacts with authorities and hospital, current referral characteristics (e.g., parental referral vs. child referral), physical findings, and suspicious behaviors of child and parent. Most of the findings suggest that decisions of CPTs are based on indices that have strong support in the professional literature. Existing heterogeneity between cases, practitioners and medical centers had an impact on the overall predictability of the decision to report. Attending to collaboration between hospitals and community agencies is suggested to support learning and quality improvement.  相似文献
