| 摘 要： | 目的探讨血浆总同型半胱氨酸（tHCY）含量在肝硬化疾病的临床价值。方法测定并分析47例肝硬化患者及55例健康者血浆tHCY含量,以及肝硬化患者血清丙氨酸转氨酶（ALT）、门冬氨酸转氨酶（AST）、总胆红素（TBIL）、胆碱酯酶（CHE）、白蛋白（ALB）、前白蛋白（PALB）、葡萄糖（GLU）、尿素氮（BUN）、肌酐（Cr）含量、血浆中20种氨基酸水平。结果肝硬化患者血浆tHCY水平（12.98±3.73μmol/L）与正常对照组（10.29±1.96）μmol/L相比有所升高,差异有统计学意义,肝硬化Child-pugh C级患者中HCY水平明显升高（14.96±4.74μmol/L）,与对照组、肝硬化Child-pugh A级患者、肝硬化Child-pugh B级患者相比差异具有统计学意义。tHCY变化与血清ALT、AST、ALB、TBIL、PALB、BUN、Cr、BCAA/AAA无相关性,而与氨基酸分析中蛋氨酸（MET）成正相关,且具有统计学意义。结论 HCY可作为评价肝硬化病情严重程度的一项新的生化指标。

| Abstract: | Objective To investigate the clinical significance of plasma total homocysteine（tHcy） in cirrhosis.Methods Plasma total homocysteine level were measured in 47 cirrhosis patients and 55 healthy controls during 2007/20-2009/10.Serum ALT,AST,TBIL,CHE,ALB,PALB,GLU,BUN,Cr and amino acid levels in cirrhosis patients were also detected.Results Plasma tHcy levels in cirrhosis patients（12.98±3.73 μmol/L） were higher than healthy controls（10.29±1.96）μmol/L（P0.05）.Plasma tHcy levels in Child-pugh C patients（14.96±4.74）μmol/L was significantly higher than Child-pugh A and B patients（P0.05）.There were no correlation between plasma tHcy levels and ALT,AST,ALB,TBIL,PALB,BUN,Cr,BCAA/AAA,but there was significantly positive correlation between tHcy and Methionine（P0.05）.Conclusions Plasma tHcy levels in cirrhosis disease may be a new biochemistry measurement. |
