| 摘 要： | 目的:探讨鼻内镜手术失败的原因及修正性鼻内镜手术的方法,避免并发症,提高治疗效果.方法:回顾性分析我科39例复发性鼻窦炎、鼻息肉患者的病例资料.结果:通过术后一年以上的随访,治愈29例(74.35%),有效8例(20.51%),无效2例(5.1%).术中并发症大出血3例,眶周损伤2例,术后鼻腔粘连4例.结论:病变去除不彻底,鼻腔粘连、息肉复发及鼻窦口阻塞是鼻内镜手术失败的常见原因;修正术中辨认残存解剖标志,熟练的技术操作,适当的术式、术后处理以及长期随访是提高疗效防止并发症的关键.

| Abstract: | Objective The aim of this study was to discuss the failing reason of cndoscopic sinus surgery and the techniques of the revision endoscopic sinus surgery, to avoid the compolica-tions,to improve the effect of treatment. Methods: In this presentation we review indication for revision endoscopic sinus surgery in our 39 patients with recur chronic rhinosinusitis. Results: After 6 months to one year, 29 patients were cured (74. 35%), 8 patients were efficiency (20. 51%),2 patients were nullity (5. 1 %). complications in the surgery :serious bleeding 3 patients, hematoma around the ophthalmic 2 patients, conglutination of nasal cavity after surgery 4 patients. Conclusion: the failing reasons of endoscopic sinus surgery were no enouth removing disease, conglutination of nasal cavity after surgery, recurrent polyposis,and obostructive sinus osti-um. In revision surgery identifying the anatomic landmarks, skillful surgery techniques, proper methods and dealing with after surgery are impotant to improve treatment and prevent complications. |
