胃癌和癌旁粘膜组织中bcl-2和c-myc基因蛋白表达的研究

摘    要:目的:研究bcl-2、c-myc在胃癌发生、发展过程中的变化。方法:应用SP免疫组织化学方法,对80例胃癌及癌旁组织进行bcl-2、c-myc基因蛋白的检测,并对两种基因的生物学和临床病理意义进行研究。结果:胃癌组织bcl-2阳性表达率为46.3%,癌旁组织为40.0%。光镜下对癌旁组织依据有无癌前病变进行分组,其中56例伴癌前病变者bcl-2阳性表达率为57.1%,24例不伴癌前病变者无阳性表达,伴癌前病变癌旁粘膜与癌相比无统计学意义(P>0.05)。对53例带有癌的癌旁组织根据癌旁粘膜和癌的关系,分为移行粘膜25例,受侵粘膜28例,其中移行粘膜均伴癌前病变。bcl-2阳性表达率在移行粘膜(68.0%)和胃癌组织均明显高于受侵粘膜(3.57%)(P<0.01)。bcl-2的表达强度在癌组织和伴癌前病变癌旁粘膜明显高于不伴癌前病变癌旁粘膜(P<0.01),癌和移行粘膜显著高于受侵粘膜(P<0.05)。c-myc阳性表达率在胃癌为67.5%,癌旁组织为57.5%。伴癌前病变癌旁粘膜(82.1%)和胃癌组织均与不伴癌前病变癌旁粘膜(41.7%)有明显差异(P<0.01、P<0.05)。移行粘膜(84.0%)和受侵粘膜(67.9%)与胃癌组织比较,无明显差异(P>0.05)。c-myc表达强度在胃癌和伴癌前病变癌旁粘膜显著高于不伴癌前病变癌旁粘膜(P<0.05);而胃癌、移行粘膜、受侵粘膜三组间无明显差异(P>

Abstract:Purpose: To study the changes of bcl- 2 and c-my concoprotein in the occurrence and development of gastric carcmoma(GC). Methods:The cellular oncoprotein expressions of bcl - 2 and c - myc were studied with streptavidin peroxidase immunohistochemical method. Specimens taken from 80 gastric carcinoma(GC) and mucosa tissues adjacent to the primary cancer (MA) were examined in this series. Results: The positive rates of bcl-2 oncoprotein were 46. 3% , 40.0% in GC and MA. MA were further devided into two groups on the basis of precancerous lesions(PL) under light microscope. The posi tive expression rate of bcl - 2 was 57. 1 % for the 56 cases MA with PL(MAWPL) , while no positive expression of bcl-2 was found in the other 24 cases MA without PL( MAWOPL) . Yet statistical significance was not found between MAWPL and GC(P>0.05). According to the relationship between MA and GC, 53 cases MA with rancer were subdivided into transitional mucosa(TM,25 cases) and invaded mucosa (IM,28 cases). All the TM were with PL The rate of expression of bcl - 2 in TM (68.0%) and GC were significantly higher than that in IM(3.57%)(P<0.01). Immunostaining intensity of bcl - 2 was stronger in GC and MAWPL than in MAWQPL (P<0.01), while those in GC'and TM were stronger than IM (P<0.05). The 57. 5% in GC and MA respectively . The c - myc expression in MAWPL(82.1%) and GC were significantly higher than that in MAWOPL (41.7%) (P<0.01, 0.05 respectively) . No significant differences were found among TM(84.0%), IM(67.9%) and GC(P>0.05 ). The immunostaining intensity of c-myc was stronger in GC and MAWPL, but weaker in MAWOPL (p<0.05), the statistical significance was not found among GC. TM and IM (P>0.05). In this study, the coexpression rates of bcl - 2 and c - myc oncoproteins were Significant differences between MAWPL (57.1%) and MAWOPL (0 %) , TM (68. 0%) and 1M(3.57%)(P<0.01), among GC, MAWPL and MAWOPL, and among GC. TM and IM (P<0.05). Conclusion: the expressions of bcl-2 and c - myc in GC were observed. The mutation of bcl - 2 and c - myc genes may be an early molecular marker of gastric carcinogenesis. Therefore, detection of both bcl - 2 and c-myc expression is of use in the early diagnosis and prognosis of patients with GC.
