| 摘 要： | 目的：探讨无创呼吸机联合纳洛酮治疗慢性阻塞性肺疾病急性加重期（AECOPD）合并Ⅱ型呼吸衰竭的临床应用价值。方法：选取2011年3月-2013年9月收治的78例AECOPD合并Ⅱ型呼吸衰竭患者,随机分为对照组和观察组,两组患者均给予常规治疗措施及无创呼吸机辅助呼吸,观察组在此基础上再给予静滴纳洛酮治疗,对两组患者治疗效果、血气指标改善情况进行综合比较。结果：观察组治疗总有效率为90.00%,高于对照组72.05%（P〈0.05）;观察组患者p H、Pa O2、Pa CO2三项血气指标改善情况均优于对照组（P〈0.05）。结论：在呼吸机辅助通气基础上联合应用盐酸纳洛酮治疗AECOPD合并Ⅱ型呼吸衰竭效果显著,可显著改善患者血气结果。

| Abstract: | Objective： To investigate the clinical value of the non - invasive ventilator combined with naloxone therapy chron ic obstruc- tive pulmonary disease with acute exacerbation （AECOPD） and type II respiratory failure. Methods： A total of 78 cases with chronic obstructive pulmonary disease with acute exacerbation （AECOPD） and type Ⅱ respiratory failure were randomly divided ilto control and observation group on march 2011 to September 2013, Two groups were given conventional treatment and non -invasive inechanical ventilation, simultaneously, the observation group was given naloxone on intravenous injection. Two groups were compared on clinical symptoms and blood gas analysis. Results： Total effective rate of observation group was 90. 00%, higher than 72.05% of co itrol group （ P 〈 0. 05） ; Improvement of blood gas analysis of pH, PaO2 , PaCO2 on observation group were better than the control gr mp （ P 〈 0. 05） . Conclusions： It is significant effect on the non - invasive ventilator combined with naloxone therapy chronic obstr tctive pul- monary disease with acute exacerbation （AECOPD） and type Ⅱ respiratory failure, and can significantly improve outcones of blood gas analysis. |
