围绝经期运动人群与非运动人群激素水平与骨密度研究

摘    要:目的:以重庆市27所中小学182名健康围绝经期女性教师为研究对象,欲了解长期运动的围绝经期女性激素水平对骨密度的影响。方法:根据研究对象职业特点分为运动人群和非运动人群两类,每类人群又根据绝经时间分为绝经前期,绝经期和绝经时间小于2年三组。利用双能X线(DXA)骨密度仪对研究对象的腰椎(L2-4)前后位、左股骨颈(L-FN)、大转子(L-TR)和华氏三角区(Wards)正侧位进行测量,同时检测血清特异性碱性磷酸酶(BAP)、血清雌二醇(E2)、甲状旁腺激素(PTH)及血钙(Ca2+)浓度。结果绝经前期,两类人群BAP、E2、PTH及Ca2+浓度无显著性差异(P>0.05),运动人群L-FN、L-TR、Wards骨密度显著高于非运动人群;绝经期和绝经后2年内运动人群BAP、PTH水平显著低于非运动人群(P<0.05),L2-4、L-FN、L-TR、Wards骨密度则显著高于非运动人群(P<0.05);绝经期运动人群E2含量显著高于非运动人群(P<0.05)。结论:长期、持续运动可维持围绝经期女性骨密度水平,从而有效预防围绝经期妇女因激素缺乏导致的骨质疏松。

Abstract:The purpose of the paper is to learn about the influence of sportive perimenopausal women’s hormone level on their BMD(based on a survey of 182 healthy women teachers from 27 primary and high schools in Chongqing).Methods: the subjects are divided into sportive group and non-sportive group according to their professions,with the people in each group further divided into three sub-groups according to their menopause time(pre-menopause,menopause and the menopause time shorter than two years).DXA is used to test their lumbar spine(L2-4),left femoral neck(L-FN),great trochanter(L-TR) and Wards.At the same time,the BAP in serum,the serum estradiol(E2),the parathyroid hormone(PTH) and the concentration of blood calcium(Ca2 +) are tested,too.The results show that early menopause can not witness any remarkable differences(P > 0.05)in the two groups of people’s E2,PTH BAP and Ca2 + concentration.The BMD of L-FN,L-TR,Wards in sportive people is significantly higher than that in the non-sportive people while the BMD level of BAP,PTH,Wards for sportive people(menopause and menopause within 2 years) is obviously lower than that in the non-sportive group(P < 0.05),but their BMD of L2-4,L-FN,L-TR,Wards is significantly higher than that of the non-sportive group(P<0.05).The E2 in menopause sportive people is higher than that in the non-sportive group(P<0.05).Conclusion: long and continuous exercise can help maintain the BMD of perimenopausal women so as to effectively prevent these women from suffering from osteoporosis because of the lack of hormone.
