单克隆抗体MC3用于大肠癌和腺瘤的免疫组织化学研究及其预后意义

摘    要:用单克隆抗体MC_3在福尔马林固定的48例大肠癌、31例癌变腺瘤、42例伴有不典型增生腺瘤、40例腺瘤、13例息肉以及48例正常肠粘膜组织切片上用免疫组化(PAP)法检测,结果MC_3相应抗原的表达阳性率分别为93.8%(45/48)、80.65% (25/31)、47.62% (20/42)、37.5%(15/40),其阳性率差异有非常显著性(P<0.001).48例正常肠粘膜均为阴性.MC_3在大肠癌组织中的反应程度与患者3年生存率有关,差异有显著意义(P<0.05).MC_3反应强度在高分化肠癌组与中~低分化肠癌组比较差异有显著意义(P<0.05).在绒毛状腺瘤、混合性腺瘤和管状腺瘤中MC_3相应抗原表达阳性率分别为71.4%(25/35)、60.0%(9/15)、41.3(26/63),差异有显著性(p<0.05).绒毛状腺瘤、混合性腺瘤和管状腺瘤其癌变率分别为45.7%(16/35)、13.3%(2/15)、20.6%(13/63),其癌变率比较差异有显著性(P<0.05).

Abstract:Formalin fixed tissue sections obtained from 48 cases of large bowel carcinoma, 31 cases of adenoma with malignant transformation, 42 cases of adenoma accompanied with dysplasia,40 cases of adenoma,13 cases of polyp and 48 cases of normal colonic mucosa were examined for the degree of immunoreaction by immunohis- tochemical (PAP) methods using anti-human colonic carcinoma monoclonal antibodies MC_3. The MC_3 positive staining rates were 93. 8%(45/48),80. 65%(25/31),47. 62%(20/42),37. 5%(15/40)respectively. The dif- ferences were very significant (P<0. 001). Normal colonic mucosa of 48 cases did not show any staining. It was found that in the group of patients surviving less than 3 years after operations the MC_3 reactions of can- cer were markedly (P<0. 05) stronger than that of those patients surviving more than 3 years. The group of well differentiated carcinomas demonstrated less MC_3 reaction as compared with the group of moderately to poorly differentiated ones and the differences between them were significant (P<0. 05). The MC_3 positive staining rates of villous adenoma, mixed adenoma and tubular adenoma were 71. 4% (25/35), 60. 0% (9/15) and 41.3% (26/63) respectively. The differences were significant (P<0. 05). The rates of malignant transfor- mation of villous adenoma, mixed adenoma and tubular adenoma were 45.7%(16/35),13. 3%(2/15),20. 6% (13/63) respectively. The differences were significant (P<0.05).
