合成了4种不同取代基的1,3-双(芳基丁二烯基酮)丙烷类新型双共轭链衍生物,并对其合成条件进行了优化,实施了其在可见光诱导Ru(bipy)3Cl2催化条件下发生的[4+4]和[2+2]环加成反应,合成得到了4个四环并五环及4个八环并五环双酮取代化合物.该结果为合成特殊八员环及四员环类药物分子和多共轭有机光电材料化合物,提供了很好的前体化合物.  相似文献

Serum cardiac troponin T (cTnT) and CKMB (mass) were analysed in three groups of patients. The first group (n=32) were patients with acute coronary syndromes including myocardial infarction. The second group (n=35)were patients with hypertension. The third group (n=24) were patients who had succumbed to non cardiac diseases. In all 3 groups, cardiac troponin T was elevated when compared with controls (p<0.001). However, CKMB elevation was not significant in all groups. CKMB levels correlated well with troponin T levels only when CKMB was greater than 50 ng/ml (r=1.00). Small elevations of troponin T identifies minimal cardiac necrosis and patients can benefit from early invasive therapy.  相似文献

Bone metastases are a serious problem in patients with advanced cancer disease and their presence usually signifies serious morbidity prior to the patient’s death. In breast cancer patients the incidence of bone metastasis is observed to be very high at 70 %, as seen during post-mortem examination. Bone metastasis is difficult to diagnose, treat or follow clinically without radiological tools. This study was designed to evaluate the utility of a novel bone resorption marker–serum tartrate-resistant acid phosphatase 5b (TRACP5b) and the bone formation marker such as serum total alkaline phosphatase (ALP), in comparison with whole body skeletal scintigraphy with Technetium99m MDP for the diagnosis of bone metastases (BM) in breast cancer (BC) patients. This study is intended to help the clinician to diagnose bone metastasis without resorting to radiological tools, as they are not cost effective and carry the risk of radiation. Experimental design: Four groups of samples were analysed. 1st group consists 52 normal female (cancer free women), 2nd group consists 38 BC patients without bone metastasis, 3rd group consists 27 breast cancer patients with limited bone metastasis (3 or less than 3 skeletal lesions) and 4th group consists 35 breast cancer patients with extensive bone metastasis (4 or more than 4 skeletal lesions), conformed by whole body skeletal scintigraphy with Technetium99m MDP. One way ANOVA was used to compare serum TRACP5b and serum ALP among these groups. Both serum TRACP5b and serum ALP are not markedly elevated in limited bone metastasis but are strongly elevated in extensive bone metastasis (p < 0.0001). As seen in this study the biochemical bone resorption marker, serum TRACP5b, abnormally increased in extensive bone metastasis of breast cancer patients and can be used as a specific marker for bone metastasis in lieu of radiological tools.  相似文献

The present study was carried out to explore the altered lipid, lipoprotein and apoprotein abnormalities along with lipoprotein (a) in chronic kidney disease patients with stage I to V which were further divided into group 1 (stage I and II), group 2 (stage III and IV) and group 3 (stage V). 50 chronic kidney disease patients with stage I to V and 20 healthy normal subjects as controls were recruited for this study. Among the various parameters tested triglyceride levels were high in group 1 and 2, whereas VLDL cholesterol, Lp (a) and apo B levels were significantly high in all the groups when compared to controls (P<0.05). However, LDL cholesterol level was significantly low in group 3 only as compared to control group (P<0.05). Apoprotein AI values also showed significant decrease in all groups as compared to controls (P<0.05). Though total cholesterol levels in group 1 and LDL levels in group 1 and 2 were higher than controls, but the values attained not statistically significant (P>0.05). In conclusion high levels of VLDL cholesterol, Lp (a), apo B and low levels of apoprotein AI as reported in this study are the major lipid disorders in the development of cardiovascular complications at all the stages in these patients.  相似文献

In the present study primary involution osteoporosis and vitamin D levels were studied in 60 subjects including thirty controls. The biochemical analysis of serum calcium, phosphorus, ALP, albumin and vitamin D3 (1,25-(OH)2 D3) levels were significantly decreased in osteoporotic patients when compared to non-osteoporotic control group. There was a significant correlation of magnitude of sun exposure with 1,25-(OH)2 D3 levels. Similarly, vitamin D intake in the diet has a significant correlation with 1,25-(OH)2 D3 concentration. However, no correlation could be obtained between vitamin D3 levels with increasing age. Among the biochemical markers serum calcium, phosphorus, ALP and albumin could not be correlated with 1,25-(OH)2 D3 levels.  相似文献

Matrix metalloproteinases (MMPs) play important role in the pathogenesis of coronary artery disease (CAD). 5A allele of -1612 5A/6A polymorphism of MMP-3 is associated with two fold higher activity than 6A allele. Present study was designed to analyse the association of this polymorphism with CAD in Indian population. Subjects included in the study were patients with stable angina (n=35), unstable angina (n=53), patients with recent event of myocardial infarction (MI) (MI Group-1, n=56) and patients at presentation of the acute MI (MI Group-2, n=49). Controls were healthy individuals (n=99). Genotyping of MMP-3 5A/6A polymorphism was carried out by PCR-based restriction digestion method. The genotype distribution of patient groups did not deviate from controls. Serum MMP-3 levels were significantly elevated at presentation of the acute MI by 36.8% (P=0.031) as compared to controls and more associated with 6A genotype suggesting discrepancy between in vitro transfection experiment and peripheral MMP-3 levels.  相似文献

The present study comprised 100 patients of essential hypertension who were screened for fasting hyperinsulinemia, which was detected in 77% cases. Twenty such hyperinsulinemic cases were subjected to 4 weeks of dietary control phase followed by 6 weeks of omega-3 fatty acids substitution [either 0.6 g/d (group 1) or 1.2 g/d (group 2)]. The mean basal fasting plasma insulin levels were significantly higher (p<0.001) in patients of hypertension when compared to normal controls (126.51±80.36 and 19.35±12.61 μU/ml respectively). At the end of 4 weeks of diet control only, no significant change was observed in any parameter. After substitution of omega-3 fatty acid, a significant reduction of fasting plasma insulin levels in both group 1 (29%) and group 2 (22.8%) was observed (p<0.001). Significant reduction of systolic and diastolic blood pressure (16.4% and 25% respectively), serum cholesterol, triglycerides and low density lipoprotein was also noted in both groups (p<0.001), while high density lipoprotein increased by ∼8 mg% in both groups. Thus omega-3 fatty acid substituion in low dose along-with curtailment of dietary omega-6 fatty acid may be used as an adjunctive measure in the management of essential hypertension.  相似文献

目的:比较中下段食管癌三维适形放射治疗常见的三野照射与四野照射方法的临床物理参数,优化放疗治疗计划。方法:随机选取中下段食管癌患者,采用VARIAN公司Eclipse治疗计划系统。按照ICRU50号报告指导原则勾画靶区,并分别设计三野与四野三维适形治疗计划,PTV所有方向上均放相同的边界5mm。然后利用体积剂量直方图DVH对两种布野方式的治疗计划靶区量分布,危及器官保护等参数进行分析和比较。处方剂量50Gy/25F。结果:两种照射方式靶区剂量分布情况相似,脊髓受照射剂量三野优于四野。在肺V20以及V5指标中,四野照射明显优于三野照射,在肺V30指标上无明显差异。结论:中下段食管癌三维适形放疗中,四野照射在肺V20、V5指标上优于三野照射计划,从预防严重放射性肺损伤角度上具有优势。  相似文献

本文作者首次观察了峨眉山凤仙花属Impatiens 13个野生种的花粉形态,发现3沟和4沟类型 花粉均出现在这一地区。其中3沟花粉与同科另一属水角属Hydrocera的花粉十分相似。同时注意到 花粉形态与其花的形态在某些方面具相关性,结合地理分布和生长习性等特性,得出以下结论:1.凤仙 花属3沟型花粉所指示的植物类群为一自然类群,并与水角属具有较密切的亲缘关系;2.凤仙花属4沟型花粉常具种水平上的差异,对属内类群划分具有系统学意义。  相似文献

用单克隆抗体MC_3在福尔马林固定的48例大肠癌、31例癌变腺瘤、42例伴有不典型增生腺瘤、40例腺瘤、13例息肉以及48例正常肠粘膜组织切片上用免疫组化(PAP)法检测,结果MC_3相应抗原的表达阳性率分别为93.8%(45/48)、80.65% (25/31)、47.62% (20/42)、37.5%(15/40),其阳性率差异有非常显著性(P<0.001).48例正常肠粘膜均为阴性.MC_3在大肠癌组织中的反应程度与患者3年生存率有关,差异有显著意义(P<0.05).MC_3反应强度在高分化肠癌组与中~低分化肠癌组比较差异有显著意义(P<0.05).在绒毛状腺瘤、混合性腺瘤和管状腺瘤中MC_3相应抗原表达阳性率分别为71.4%(25/35)、60.0%(9/15)、41.3(26/63),差异有显著性(p<0.05).绒毛状腺瘤、混合性腺瘤和管状腺瘤其癌变率分别为45.7%(16/35)、13.3%(2/15)、20.6%(13/63),其癌变率比较差异有显著性(P<0.05).  相似文献

Based on our demonstration earlier that ethanol extract, water extract and a compound purified from garlic possessedin vitro antitubercular activity against drug resistant and susceptibleMycobacterium tuberculosis, we tried the effect of garlic extract in 30 patients of tubercular lymphadenitis. For ethical considerations, two groups of patients, 30 each, were given antitubercular therapy (ATT) consisting of isoniazid, rifampicin, ethambutol and pyrazinamide for 30 days. For the next 15 days (31 to 45 days) group 1 patients received 3–6 garlic pearls per day in addition to ATT while group 2 patients received ATT only. From 46th day onwards both the groups received ATT only for 6–8 months. Antitubercular activity of the serum samples collected on 45th day was assessed by its effect on the growth ofM. tuberculois. The serum of group 1 patients showed significantly much higher antitubercular activity than that of group 2 patients. Further, there was relief of dyspeptic symptoms caused by ATT therapy in patients of group 1 with garlic plus ATT therapy but no change in group 2 patients with ATT only. Liver function and hematological tests were normal in both the groups after 6 months of therapy. Garlic extracts or compounds have a good potential as antitubercular(s) drug if given as a supplement to ATT.  相似文献

老鸦瓣群(Amana群)是百合科中的一个东亚特有类群,仅分布在中国中东部、日本和朝鲜半岛。该类群在形态上与广义郁金香属Tulipa L.相似,但也具有与子房近等长的花柱以及2-3(-4)个苞片等与郁金香属不同的形态特征。有关该类群是否应置于郁金香属中或是否应独立为属的问题长期以来存在争议。为了澄清该问题,我们对产于中国的Amana群进行了广泛的标本室和野外形态观察,并采用分支系统学方法,对该类群3个种以及郁金香属的其他15种植物(包括土耳其的3个种)的28个形态性状数据进行了分支分析。结果表明广义郁金香属并非是一个单系类群。Amana群与广义郁金香属的其他4个组sect. Orithyia、sect. Eriostemones、sect. Leiostemones、sect. Tulipanum以及Lloydia属的两个种共同构成一个大支的3个分支。我们对广义郁金香属5个组共19种植物的ITS以及trnL-F分子序列综合分析的结果也表明:sect. Tulipanum、sect. Leiostemones、sect. Eriostemones和sect. Orithyia构成一大支,Amana群和猪牙花属的Erythronium sibiricum构成另一大支。因此,我们认为Amana群应从广义郁金香属中独立出来,恢复其老鸦瓣属Amana Honda作为属的分类地位。  相似文献

Long-term exposure to organophosphate pesticides (OPs) without acute poisoning can lead to various OPs. Environmental exposure to organophosphate pesticides may be associated with depression and suicide attempts in a population living in a rural agricultural area. Patients (n = 149) suffering from major depressive disorder (with and without attempted suicide) and a control group of healthy individuals (n = 64) who had been living in the same rural district for at least 1 year were selected. Red blood cell acetylcholine esterase (RBC-AChE) activity was examined as the basis of evaluating the degree of chronic environmental exposure to OPs residues. There were negative association between RBC-AChE activity levels and suicide attempts, the number of past suicide attempts and hopelessness levels in the depressive patients. The results of the study may support the idea that environmental exposure to OPs may be associated with mental health in individuals living in agricultural districts who are not farmers or working in occupations with access to OPs.  相似文献

Introduction:

This study aimed to assess whether heart fatty acid-binding protein (H-FABP) and glycogen phosphorylase isoenzyme BB (GPBB) could be used for the accurate diagnosis of acute myocardial infarction (AMI) in acute coronary syndrome (ACS) patients.

Materials and methods:

The study included 108 ACS patients admitted to a coronary unit within 3 h after chest pain onset. AMI was distinguished from unstable angina (UA) using a classical cardiac troponin I (cTnI) assay. H-FABP and GPBB were measured by ELISA on admission (0 h) and at 3, 6, 12, and 24 h after admission; their accuracy to diagnose AMI was assessed using statistical methods.

Results:

From 92 patients with ACS; 71 had AMI. H-FABP and GPBB had higher peak value after 3 h from admission than cTnI (P = 0.001). Both markers normalized at 24 h. The area under the receiver operating characteristic curves was significantly greater for both markers in AMI patients than in UA patients at all time points tested, including admission (P < 0.001). At admission, the H-FABP (37%) and GPBB (40%) sensitivities were relatively low. They increased at 3 and 6 h after admission for both markers and decreased again after 24 h. It was 40% for H-FABP and approximately 2-times lower for GPBB (P < 0.01). In AMI patients, both biomarkers had similar specificities, positive- and negative-predictive values, positive and negative likelihood ratios, and risk ratios for AIM.

Conclusion:

H-FABP and GPBB can contribute to early AMI diagnosis and can distinguish AMI from UA.  相似文献

An audit of 2509 patient specimens analyzed for both total thyroxine (TT4) and free thyroxine (FT4) by the ACS-180 automated chemiluminescence immunoassay analyzer revealed that there were 219 discrepancies (8.7% of the total). A discrepancy was defined as one analyte within its reference range and the other outside. The discrepant results were divided into 4 groups: group A: normal TT4, but decreased FT4, 101 patients (4.0%); group B: increased TT4, normal FT4, 78 patients (3.1%); group C: decreased TT4, normal FT4, 34 patients (1.4%); and group D: normal TT4, increased FT4, 6 patients (0.2%). TSH measurements were available, by a 3rd generation chemiluminescent assay, in 142 of these patients, and were consistent with the FT4 result in 72 patients, with TT4 in 61 cases and with neither in 9 patients. The clinical diagnosis was investigated in a subgroup of 43 endocrine patients. Thirteen of the 20 endocrine patients in group A were diagnosed as hypothyroid, with a measured serum TSH, in 11 of them, of median 14.6, range 1.2 to 46.2 μlU/ml. Eleven of the 19 endocrine patients in group B were on thyroid replacement, with a measured serum TSH, in 7 of them of <0.01 μlU/ml. The audit of current laboratory practice led to a suggestion to replace the current thyroid function screening strategy of measuring both TT4 and FT4 by the combination of FT4 and TSH. The reasons for the discrepancies and the alternative strategies for screening of thyroid function are discussed.  相似文献

(1)Eichler(1958)在研究马来西亚铁线莲属植物时也深入地研究了中国、越南、泰国、缅甸及喜马拉雅山区的该属标本,认为根据我国台湾标本描述的Clematis taiwaniana Hayata可能为C.javana DC.的异名。本文作者在研究了有关的南洋群岛、中国台湾及日本琉球群岛的标本之后同意Eichler的归并意见;此外,C.grata Wall.var.ryukiuensis Tamura,C.sikkimensis(Hook.F.&; Thoms.) Drumm.ex Burkill,C.connata DC.var.bipinnata M.Y.Fang,C.kilungensis W.T. Wang &; M.Y. Fang等拉丁学名也被归并为异名。(2)对大叶铁线莲组sect.Tubulosae做出了新的分类系统。特产日本的Clematis takedana Makino 自发表后一直被认为是C.apiifolia DC.和 C.stans Sieb.&; Zucc.(大叶铁线莲C.heracleifolia DC.的近缘种之一)的杂交种。此种为木质藤本植物,具三出复叶,腋生花序有多数两性花,呈圆锥状,而相似于C.apiifolia DC.(萼片水平开展,雄蕊无毛,花药狭长圆形,药隔顶端不突出),但其萼片向斜上方开展(花萼宽钟状),花丝上部有疏柔毛,花药条形,药隔顶端有短突起而不同。笔者发现,特产我国河北西部和北京山地的羽叶铁线莲C.pinnata Maxim.(叶为具5小叶的羽状复叶)具有与C.takedana Makino相同的花构造,二者在亲缘关系上极为相近,组成一个自然小群。这二种与特产北京的细花铁线莲C.tatarinowii Maxim.(花两性,萼片直立,花丝上部有疏柔毛,花药狭长圆形,背面有疏柔毛,药隔顶端不突出)组成大叶铁线莲组sect.Tubulosae的原始群——羽叶铁线莲亚组subsect.Pinnatae。而原来的大叶铁线莲C.heracleifolia DC.等种组成此组的进化群——大叶铁线莲亚组subsect.Tubulosae,其特征为花通常单性,萼片直立,雄蕊常有疏柔毛,茎直立,草质至木质。此外,还对C. siamensis Drumm.ex Burkill的4变种和C.connata DC.的5变种进行了分类。(3)描述了3新系、3新种和4新变种。(4)做出了6新组合和3新等级。  相似文献

目的:对独活寄生汤联合来氟米特片治疗类风湿关节炎的临床疗效进行观察。方法:对独活寄生汤联合来氟米特片治疗类风湿关节炎(肝肾亏虚型)的临床疗效进行回顾性分析;应用随机数字表把纳入的120例患者随机分为治疗组和对照组,两组各60例,对照组给予来氟米特片治疗;治疗组予以独活寄生汤联合来氟米特片治疗,1疗程为6个月,分别于治疗前、治疗1个月、2个月、3个月对患者进行DAS-28评分并采用HAQ健康评估问卷进行生存质量评估。结果:治疗组总有效率、显效率分别为88.33%和80%;对照组总有效率、显效率分别为73.33%和61.61%。总有效率、显效率治疗组均显著优于对照组。两组患者HAQ评分、DAS-28评分在治疗1个月均有所改善,病情得到控制;两组患者HAQ评分、DAS-28评分在2个月后仍继续减少,病情初步缓解,较本组治疗前有显著性差异(P0.05),组间相比仍无显著性差异(P0.05);两组患者在治疗结束时HAQ评分、DAS-28评分明显减少,病情完全控制,较本组治疗前及治疗1个月有显著性差异(P0.05),组间相比均有显著性差异(P0.05)。结论:独活寄生汤联合来氟米特片可有效改善肝肾亏虚型型类风湿关节炎患者的临床症状及实验室指标,且无明显不良反应,提高生存质量,取得了满意效果。  相似文献

目的:观察逍遥散治疗慢性乙型病毒性肝炎的临床疗效。方法:100例慢性乙型病毒性肝炎患者随机分为两组,观察组50例在对照组基础上口服逍遥散,150ml/次,3次/d;对照组50例口服恩替卡韦分散片,0.5mg/次,1次/d。两组患者均治疗3个疗程共12周,观察两组患者治疗前后的临床症状、体征、HBV-DNA、肝功能的变化程度。结果:观察组患者临床疗效总有效率显著高于对照组(98%vs 84%),两组间比较差异有显著性意义(P0.05)。结论:加服逍遥散治疗慢性乙型病毒性肝炎有较好的疗效,值得临床进一步研究应用。  相似文献

Introduction:

The aim of the study was to identify biomarkers of alteration in bone mineral density (BMD) in patients on haemodialysis (HD) and peritoneal dialysis (PD).

Materials and methods:

In a cross-sectional, longitudinal study dual-energy X-ray absorptiometry scans were performed in 146 HD-patients and 28 PD-patients. Follow-up after 14 months (mean) was conducted in 73 patients. As potential biomarkers we investigated parathyroid hormone (PTH), 25-hydroxy vitamin-D, ionised calcium, albumin, phosphate, and total alkaline phosphatases (t-ALP).

Results:

Both groups of dialysis patients had lower BMD in the femoral neck (BMDneck) (P < 0.001) and forearm (BMDforearm) (P < 0.001) compared to healthy controls, but comparable BMD in the lumbar spine (BMDspine). BMD did not differ between dialysis types, but patients ever-treated with glucocorticoids had significantly lower BMD, while patients with polycystic kidney disease had higher BMD. BMD correlated with body weight, actual age, age at initiation of dialysis, duration of dialysis and levels of PTH and t-ALP. However, t-ALP only remained associated with low BMDspine after adjusting for other factors (P = 0.001). In the follow-up study all patients had decreased BMD in all three locations, but only for the lumbar spine there was a significant association between BMD and the bone markers t-ALP (P = 0.009) and PTH (P = 0.013).

Conclusions:

Both HD and PD patients have low BMD, and increased concentrations of t-ALP is associated BMDspine after adjustment, while PTH and t-ALP is associated with decrease in BMDspine over time. This substantiates the use of these biomarkers in both types of dialysis patients.  相似文献
