肝素治疗胎儿生长受限临床应用探讨   总被引:1,自引:0,他引:1  
目的:探讨肝素用于治疗胎儿生长受限(FGR)的临床疗效及安全性。方法:将74例胎儿生长受限患者随机分为3组,分别使用低分子右旋糖酐加复方丹参(对照组)、普通肝素(UFH)、低分子肝素(LMWH)治疗。治疗前、治疗后1周及终止妊娠前,行彩色超声检查,监测胎儿生长情况和脐血流变化,同时监测各项凝血指标,记录新生儿情况并进行随访。结果:(1)UFH组、LMWH组治疗后宫高、胎儿股骨长、头围、腹围增长值均高于对照组,差异有显著性(P<0.05),LMWH组胎儿双顶径增长显著高于对照组(P=0.031)。(2)两肝素组治疗后脐动脉收缩期最大血流速度(S)与舒张末期血流速度(D)的比值(S/D比值)、搏动指数(PI)、阻力指数(RI)与对照组比较均明显降低,差异有统计学意义(P<0.05),而两肝素组间比较无统计学差异(P>0.05)。(3)UFH组、LMWH组治疗后新生儿胎龄、出生体重、身长、胎盘重量均较对照组明显增加(P<0.05),而两肝素组间无显著差异(P>0.05);新生儿窒息、胎死宫内、新生儿死亡发生率、早产发生率和新生儿畸形发生情况各组间无统计学差异(P>0.05);对照组足月小样儿发生率明显高于两肝素组(P=0.005),而肝素组之间则无显著差异(P>0.05)。对照组与LMWH相比,新生儿转重症监护病房(NICU)几率明显升高(P=0.003)。(4)各组孕妇治疗前后各项凝血功能检测值比较,差异均无显著性(P>0.05),产后出血量及产后出血发生率相比差异亦无统计学意义。结论:肝素可改善胎盘血流,使胎儿体重增加,减少足月小样儿的发生率,且不增加产后出血发生机会,对母亲及胎儿都较安全。LWMH治疗FGR,新生儿转NICU几率明显低于对照组,效果优于普通肝素,且使用方便,产妇易于接受,更适于推广。  相似文献

Heparin was grafted onto polycarbonate urethane (PCU) surface via a three-step procedure utilizing α, ωdiamino-poly(ethylene glycol) (APEG, M n =2 000) as a spacer. In the first step, isocyanate functional groups were introduced onto PCU surface by the treatment of hexamethylene diisocyanate (HDI) in the presence of di-n-butyltin dilaurate (DBTDL) as a catalyst. In the second step, APEG was linked to the PCU surface to obtain the APEG conjugated PCU surface (PCU-APEG). In the third step, heparin was covalently coupled with PCU-APEG in the presence of N-hydroxysuccinimide (NHS) and 1-ethyl-3-(3-dimethylamidopropyl) carbodiimide (EDAC). The amount of heparin (1.639 μg/cm 2 ) covalently immobilized on the PCU-APEG surface was determined by the toluidine blue method. The modified surface was characterized by water contact angle, attenuated total reflection Fourier transform infrared spectroscopy (ATR-FTIR), X-ray photoelectron spectroscopy (XPS), and atomic force microscopy (AFM). The hemocompatibility was preliminarily studied by platelet adhesion test. The results indicated that heparin was successfully grafted onto the PCU surface, and meanwhile the hydrophilicity and hemocompatibility of the modified PCU surface were improved significantly compared with the blank PCU surface.  相似文献

The aim of this study was to explore the protective effect of basic fibroblast growth factor (bFGF) on brain injury following global ischemia reperfusion and its mechanisms. Brain injury following global ischemia was induced by four vessels occlusion and systemic hypotension. Twenty-four rabbits were randomized into three groups: group A, only dissection of vessels; group B, intravenous infusion of normal saline after reperfusion for 6 h; group C, 30 μg/kg bFGF injected intravenously at the onset of reperfusion, then infused with 10 μg/(kg·h) for 6 h. Serum neuron specific enolase (NSE), S-100B, tumor necrosis factor-α(TNF-α), interleukin-1 (IL-1), interleukin-8 (IL-8) were measured before ischemia, 30 min after ischemia, 0.5, 1, 3, 6 h after reperfusion. Brain water content was determined and cerebral histopathological damages were compared. NSE and S-100B were increased 1 h after reperfusion and reached their peaks 6 h after reperfusion, but were much higher in group B than those in group C 3, 6 h after reperfusion. In groups B and C, TNF-a was increased after ischemia and IL-1 and IL-8 were increased significantly 0.5 h after reperfusion, then reached their peaks 6 h, 3 h, 6 h after reperfusion respectively. TNF-a and IL-8 at the time points of 1 h and 3 h and IL-1 at 3 h and 6 h in group C were correspondingly lower than those in group B. These indices in group A were nearly unchanged. There were less severe cerebral histopathological damages in group C compared with group B, but no difference in brain water content. It could be concluded that bFGF alleviates brain injury following global ischemia and reperfusion by down-regulating expression of inflammatory factors and inhibiting their activities.  相似文献

Objective: To evaluate the efficacy and the indication of basic fibroblast growth factor (bFGF) in the treatment of exposure of orbital implants. Design: Retrospective and observational case series. Methods: We reviewed 41 patients (41 eyes) suffering exposure of orbital implants from Jan. 2000 to June 2006. The study group patients with mild exposure received combined treatment with bFGF and antibiotic drops, and while the control group patients with mild exposure were treated with antibiotic drops only. The study group patients with moderate and severe exposure received combined treatment with bFGF and antibiotic drops, and after 2 months they were subjected to amniotic membrane transplantation, while the control group patients with moderate and severe exposure underwent amniotic membrane transplantation after using antibiotic drops. Observation of the growth of conjunctival epithelium and comparison of the healing rate of the two groups. Results: The healing rates of the mild, moderate and severe exposure study group were 100% and 92.3%. The healing rates of the mild, moderate and severe exposure control group were 55.6% and 66.7% respectively. The difference of the healing rates of the mild exposure study group and the control group was significant (P=-0.033). And the difference of the healing rates of the moderate and severe exposure study group and the control group was not significant (P=-0.167). Conclusion: bFGF may promote obviously the healing of orbital implant exposure, particularly it can be the first choice for the treatment of mild degree exposure. For the moderate and severe cases, it can be administered before surgical repair to enhance neovascularization and will tend to increase the success rate of surgical repair.  相似文献

The Bacillus firmus was immobilized into Ca-alginate beads according to the different initial biomass concentration, calcification time and activation time.Three types of immobilized Bacillus firmus beads were packed respectively in trickling biofilter to purify xylene contained waste gases,and the performance of immobilized-cell biofilter was compared with traditional biofilm attached biofilter packed with two types of ceramic pellets.The results showed that three types of immobilized beads had different capabilities for removing xylene and life-spans.Higher initial biomass in immobilized beads resulted in better performance but shorter life-span.Activation process can remarkably enhance the activity of bacteria,and the removal efficiency of xylene can substantially be improved.Calcification time had influence on life-span of immobilized beads.Without acclimation,the cell-entrapped biofilter can obtain the maximum elimination capacity of 92.4 g/(m~3·h).However,compared with biofilm attached biofilter,it has a poorer intrinsic drawback in volatile organic compounds (VOCs) removal due to the existence of excess mass transfer resistance.  相似文献

目的:评价低分子肝素钠对重症肺心病患者的疗效和副作用。方法:将92例重症肺心病患者随机分为两组,观察组45例,给予低流量持续吸氧、抗感染、祛痰、止咳平喘、改善心肺功能等常规治疗;治疗组47例在常规治疗基础上加用低分子肝素钠5000IU,皮下注射,每日2次.连用7d。结果:治疗组与观察组治疗后血液流变学及动脉血氧分压比较差异有显著性(P〈0.05)。结论:低分子肝素钠治疗重症肺心病有显著疗效。  相似文献

To investigate inter- and intra-hemispheric electroencephalography (EEG) coherence at rest and during photic stimulation of patients with Alzheimer‘s disease (AD). Thirty-five patients (12 males, 23 females; 52-64 y) and 33 sex- and age-matched controls (12 males, 21 females; 56-65 y) were recruited in the present study. EEG signals from C3-C4, P3-P4, TS-T6 and O1-O2 electrode pairs resulted from the inter-hemispheric action, and EEG signals from C3-P3, C4-P4, P3-O1, P4-O2, C3-O1, C4-O2, TS-O1 and T6-O2 electrode pairs resulted from the intra-hemispheric action. The influence of inter- and intra-hemispheric coherence on EEG activity with eyes closed was examined, using fast Fourier transformation from the 16 sampled channels. The frequencies of photic stimulation were fixed at 5, l0 and 15 Hz, respectively. The general decrease of AD patients in inter- and intra-hemispheric EEG coherence was more significant than that of the normal controls at the resting EEG, with most striking decrease observed in the alpha-1 (8.0-9.0 Hz) and alpha-2 (9.5-12.5 Hz) bands. During photic stimulation, inter- and intra-hemispheric EEG coherences of the AD patients having lower values in the alpha (9.5-10.5 Hz) band than those of the control group. It suggests that under stimulated and non-stimulated conditions, AD patients had impaired inter- and intra-hemispheric functional connections, indicating failure of brain activation in alpha-related frequency.  相似文献

The optimal operation conditions in an anoxic sulfide oxidizing (ASO) bioreactor were investigated. The maximal removal rates for sulfide and nitrate were found to be 4.18 kg/(m3·d) and 1.73 kg/(m3·d), respectively. The volumetrical volumetric loading rates (LRs) observed through decreasing hydraulic retention time (HRT) at fixed substrate concentration are higher than those by increasing substrate concentration at fixed HRT. The sulfide oxidation in ASO reactor was partially producing both sulfate and sulfur; but the amount of sulfate produced was approximately one third that of sulfur. The process was able to tolerate high sulfide concentration, as the sulfide removal percentage always remained near 99% when influent concentration was up to 580 mg/L. It tolerated relatively lower nitrate concentration because the removal percentage dropped to 85% when influent con- centration was increased above 110 mg/L. The process can tolerate shorter HRT but careful operation is needed. Nitrate conversion was more sensitive to HRT than sulfide conversion since the process performance deteriorated abruptly when HRT was decreased from 3.12 h to 2.88 h. In order to avoid nitrite accumulation in the reactor, the influent sulfide and nitrate concentrations should be kept at 280 mg/L and 67.5 mg/L respectively. Present biotechnology is useful for removing sulfides from sewers and crude oil.  相似文献

The aim of the present study is to analyze sperm membrane changes and the spermicidal effect in treatment with the crude extract from Polygala tenuifolia Willd (PTW) in vitro. The root of PTW was extracted in distilled water. Normal human spermatozoa were used to assess the spermicidal activity (Sander-Cramer assay) of the extract from the PTW root. The hypo-osmotic swelling (HOS) test and the eosin Y (EY) staining were used to detect the integrity of sperm membrane and vitality. The sperm chromatin dispersion (SCD) test was performed to determine sperm DNA integrity. N-9 was used as a reference standard and semen added to physiological saline was used as the control. Semen samples were donated by 42 healthy fertile men. The crude extract from the root of PTW could immobilize and kill 100% spermatozoa within 20 s in vitro at the concentrations of 20.0 and 10.0 mg/ml; at the concentration of 5.0 mg/ml, spermatozoa were immobilized in (39.5±3.2) s. In the groups of the crude extract from the root of PTW and N-9 solution, the rate of the normal HOS (tails swollen) and the white head (unstained) was 0%, and the rate of the abnormal HOS (tails unswollen) and red head (stained) was 100%. Sperm DNA fragmentation showed no change in exposure to the crude extract from the root of PTW and N-9 solution. The sperm revival test did not show any spermatozoa that recovered their motilities. The rapid spermicidal activity of the crude extract from the root of PTW in vitro may occur by the disruption of the sperm membrane integrity.  相似文献

目的:探讨藻酸双脂钠(PSS)治疗急性脑梗死患者血液成分变化及疗效。方法:急性脑梗死患者158例,随机分为两组。对照组:78例,常规治疗;治疗组:80例,在常规治疗基础上,应用藻酸双脂钠注射液100mg加入0.9%氯化钠静脉滴注,每日一次,连续观察14d。对比分析二组治疗前后脂蛋白(a)、血脂、纤维蛋白原等血液流变学指标.并动态观察治疗前后临床神经功能缺损评分。结果:藻酸双脂钠可降低全血粘度、血浆粘度、血小板聚集率、纤维蛋白原及血清TC、TG、LDL,与治疗前比较有显著性改变(P〈0.01或P〈0.05);与对照组比较有显著性差异(P〈0.01或P〈0.05).但对脂蛋白(a)无影响;近期预后较常规治疗组好(P〈0.05).结论:藻酸双脂钠可改善血液高粘滞状态,起到降低纤维蛋白原、抗血小板聚集作用,可改善近期临床神经功能缺损程度。  相似文献

Objective: Angiogenic therapy is emerging as a potential strategy for the treatment of ischemic heart disease but is limited by a relatively short half-life of growth factors.Fibrin glue (FG) provides a reservoir for controlledrelease of growth factors.The aim of this study was to evaluate the effects of basic fibroblast growth factor (bFGF) incorporating FG on angiogenesis and cardiac performance in a canine infarct model.Methods: Acute myocardial infarction was induced by ligation of the left anterior descending coronary artery (LAD).Group I (n=6) underwent ligation of LAD alone.In Group II,transmural channels were created in the infarct area (n=6).In Group III,nontransmural channels were created to locate FG cylinders containing bFGF (n=6).Eight weeks after operation,myocardial perfusion was assessed by single photon emission computed tomography,cardiac function by echocardiography,and vascular development by immunohistochemical staining.Results: Total vascular density and the number of large vessels (internal diameter ≥50 μm) were dramatically higher in Group III than in Groups I and II at eight weeks.Only the controlled-release group exhibited an improvement in regional myocardial perfusion associated with lower defect score.Animals in Group III presented improved cardiac regional systolic and diastolic functions as well as global systolic function in comparison with the other two groups.Conclusions: Enhanced and sustained angiogenic response can be achieved by controlled-release bFGF incorporating FG within transmyocardial laser channels,thus enabling improvement in myocardial perfusion and cardiac function.  相似文献

Objective: To assess functional relationship by calculating inter- and intra-hemispheric electroencephalography (EEG) coherence at rest and during a working memory task of patients with mild cognitive impairment (MCI). Methods: The sample consisted of 69 subjects: 35 patients (n = 17 males, n = 18 females; 52-71 years old) and 34 normal controls (n = 17 males, n = 17 females; 51 -63 years old). Mini-mental state examination (MMSE) of two groups revealed that the scores of MCI patients did not differ significantly from those of normal controls (P〉0.05). In EEG recording, subjects were performed at rest and during working memory task. EEG signals from F3-F4, C3-C4, P3-P4, T5-T6 and O1-O2 electrode pairs are resulted from the inter-hemispheric action, and EEG signals from F3-C3, F4-C4, C3-P3, C4-P4, P3-O1, P4-O2, T5-C3, T6-C4, T5-P3 and T6-P4 electrode pairs are resulted from the intra-hemispheric action for delta (1.0-3.5 Hz), theta (4.0-7.5 Hz), alpha-1 (8.0-10.0 Hz), alpha-2 (10.5-13.0 Hz), beta-1 (13.5-18.0 Hz) and beta-2 (18.5-30.0 Hz) frequency bands. The influence of inter- and intra-hemispheric coherence on EEG activity with eyes closed was examined using fast Fourier transformation from the 16 sampled channels. Results: During working memory tasks, the inter- and intra-hemispheric EEG coherences in all bands were significantly higher in the MCI group in comparison with those in the control group (P〈0.05). However, there was no significant difference in inter- and intra-hemispheric EEG coherences between two groups at rest. Conclusion: Experimental results comprise evidence that MCI patients have higher degree of functional connectivity between hemispheres and in hemispheres during working condition, It suggests that MCI may be associated with compensatory processes during working memory tasks between hemispheres and in hemispheres. Moreover, failure of normal cortical connections may exist in MCI patients.  相似文献

The syntactic development of preschoolers who later became disabled readers was compared to that of children who were similar to the dyslexics in sex, socioeconomic status, and IQ, but who became normal readers. Expressive and receptive syntactic abilities were examined longitudinally from age 30 to 60 months. The dyslexic group was poorer than the control group on all measures until the age of five, at which time both groups exhibited similar syntactic proficiency. The etiological relation of language development to reading disabilities is discussed. Objective: To investigate if intra-aortic balloon pump(IABP) is contraindicated without anticoagulation therapy. Methods: Some 153 IABP patients in the King Abdulaziz Cardiac Center(KSA) were randomly assigned into two groups. Anticoagulation group (Group A) consisted of 71 patients who were given heparin intravenously with target aPTT50–70 seconds. Non-anticoagulation group (Group B) consisted of 82 patients without intravenous heparin during balloon pumping. Hematological parameters including platelet count, D-dimer, Plasminogen activator inhibitor-1 (PAI-1) and fibrinogen degradation products (FDP) were checked respectively at the point of baseline, 24 hours, 48 hours and 24 hours post IABP counterpulsation. Clot deposits on balloon surface, vascular complications from IABP including bleeding and limb ischemia were recorded. Results: Platelet count and PAI-1 level decreased at 24 hours and 48 hours in both groups (P<0.05). D-dimer dimer and FDP level increased at 24 hours and 48 hours in both groups(P<0.05), but returned to the baseline level 24 hours post IABP removal (P>0.05). Three patients in Group A and 2 patients in Group B developed minor limb ischemia (P>0.05). No major limb ischemia in either group. Two patients in Group A suffered major bleeding and required blood transfusion or surgical intervention, whereas no patient had major bleeding in Group B. Eight patients had minor bleeding in Group A, but only 2 patients in Group B (P<0.05). No clot deposit developed on IABP surface in either group. Conclusion: IABP is safe without routine anticoagulation therapy. Selecting appropriate artery approach and early detection intervention are key methods for preventing complications.  相似文献

Background: Sevoflurane and propofol are effective cardioprotective anaesthetic agents, though the cardioprotection of propofol has not been shown in humans. Their roles and underlying mechanisms in anesthetic postconditioning are unclear.Mitochondrial permeability transition pore (MPTP) opening is a major cause of ischemia-reperfusion injury. Here we investigated sevoflurane- and propofol-induced postconditioning and their relationship with MPTP. Methods: Isolated perfused rat hearts were exposed to 40 min of ischemia followed by 1 h of reperfusion. During the first 15 min of reperfusion, hearts were treated with either control buffer (CTRL group) or buffer containing 20 μmol/L atractyloside (ATR group), 3% (v/v) sevoflurane (SPC group),50 μmol/L propofol (PPC group), or the combination of atractyloside with respective anesthetics (SPC ATR and PPC ATR groups). Infarct size was determined by dividing the total necrotic area of the left ventricle by the total left ventricular slice area (percent necrotic area). Results: Hearts treated with sevoflurane or propofol showed significantly better recovery of coronary flow,end-diastolic pressures, left ventricular developed pressure and derivatives compared with controls. Sevoflurane resulted in more protective alteration of hemodynamics at most time point of reperfusion than propofol. These improvements were paralleled with the reduction of lactate dehydrogenase release and the decrease of infarct size (SPC vs CTRL: (17.48±2.70)% vs (48.47±6.03)%,P<0.05; PPC vs CTRL: (35.60±2.10)% vs (48.47±6.03)%, P<0.05). SPC group had less infarct size than PPC group (SPC vs PPC:(17.48±2.70)% vs (35.60±2.10)%, P<0.05). Atractyloside coadministration attenuated or completely blocked the cardioprotective effect of postconditioning of sevoflurane and propofol. Conclusion: Postconditioning of sevoflurane and propofol has cardioprotective effect against ischemia-reperfusion injury of heart, which is associated with inhibition of MPTP opening. Compared to propofol, sevoflurane provides superior protection of functional recovery and infarct size.  相似文献
