Sustained high levels of circulating catecholamines are reported to induce cardiotoxicity. Isoproterenol (ISP), a synthetic catecholamine has been widely employed to induce myocardial injury, though the role of inflammation and apoptosis is not well established. This study was designed to investigate the underlying mechanism of oxidative damage, inflammatory signaling, cell death in ISP induced myocardial infarction in rats. Wistar albino rats were divided in two groups: group I (sham control) and group II (ischemic control). ISP (85 mg/kg, s.c.) was administered at an interval of 24 h to group II for two consecutive days. On day third, after 48 h of the first injection of ISP, blood was collected from retro orbital plexus of rat eyes to estimate the biochemical parameters. Glutathione (GSH) and superoxide dismutase (SOD) were measured for antioxidant status. Similarly, malondialdehyde (MDA) was measured as an index of lipid peroxidation. Cardiac markers (SGOT, CK-MB, TropI and LDH) and pro-inflammatory cytokines (IL-6, CRP and TNF-α) were also estimated in ISP-induced rats. At the end of experiments animals were sacrificed for histopathological studies. GSH and SOD showed significant decrease after ISP challenge as compared to sham (control) group (p < 0.01) while MDA level, increased significantly (p < 0.01). ISP, also increased the level of cardiac markers and markers of inflammation significantly (p < 0.01), which was further verified by histopathological studies of the heart tissues. The study confirmed that ISP causes detrimental changes in the myocardium by altering cardiac and inflammatory markers, which leads to severe necrosis. The deleterious effects produced by ISP substantiate its suitability as a novel animal model for evaluation of cardioprotective agents/drugs.  相似文献

参麦注射液对急性心肌梗死大鼠心室重构的影响   总被引:3,自引:0,他引:3  
目的:探讨参麦注射液对急性心肌梗死(AMI)大鼠心室重构的影响。方法:通过结扎冠状动脉前降支建立AMI模型大鼠,AMI后24h存活的48只大鼠随机分为四组:AMI后1周模型组;AMI后参麦注射液治疗1周组;AMI后2周模型组;AMI后参麦注射液治疗2周组。另设1周、2周假手术组各8只。治疗组每日腹腔注射参麦注射液1次,AMI模型组和假手术组大鼠腹腔注射等量的生理盐水。测量各组心室重构参数。结果:与假手术组比较,AMI后1周时,心室腔已明显扩大(P〈0.05);到梗死后2周时,心室腔内径扩大更明显,与假手术组相比有显著性差异(P〈0.01),且心室重量指数(VWI)比假手术组明显增加(P〈0.05)。参麦注射液治疗2周后可改善心梗后心室重量指数增加、心室腔扩大的病理变化(P〈0.05)。结论:参麦注射液能改善大鼠AMI后心室重构。  相似文献

Objective: Malignant middle cerebral artery (MCA) infarction is characterized by mortality rate of up to 80%. The aim of this study was to determine the value of decompressive craniectomy in patients presenting malignant MCA infarction compared with those receiving medical treatment alone. Methods: Patients with malignant MCA infarction treated in our hospital between January 1996 and March 2004 were included in this retrospective analysis. The National Institute of Health Stroke Scale (NIHSS) was used to assess neurological status on admission and at one week after surgery. All patients were followed up for assessment of functional outcome by the Barthel index (BI) and modified Rankin Scale (RS) at 3 months after infarction. Results: Ten out of 24 patients underwent decompressive craniectomy. The mean interval between stroke onset and surgery was 62.10 h. The mortality was 10.0% compared with 64.2% in patients who received medical treatment alone (P<0.001). The mean NIHSS score before surgery was 26.0 and 15.4 after surgery (P<0.001). At follow up, patients who underwent surgery had significantly better outcome with mean BI of 53.3, RS of 3.3 as compared to only 16.0 and 4.60 in medically treated patients. Speech function also improved in patients with dominant hemispherical infarction. Conclusion: Decompressive craniectomy in patients with malignant MCA infarction improves both survival rates and functional outcomes compared with medical treatment alone. A randomized controlled trial is required to substantiate those findings.  相似文献

摘要:目的:探讨有氧运动对心梗大鼠心肌Bax、Bcl-2、p53和TGF-β1表达及心功能的影响。方法:选取Sprague Dawley雄性大鼠34只,体重为180~220 g,随机抽取10只为假手术组(A组),其余24只大鼠行左冠脉前降支(LAD) 结扎术制作心梗(MI)模型,术后存活20只随机分为心梗组(B组)和心梗+有氧运动组(C组),每组10只,C组进行8周跑台有氧运动。训练结束后进行心功能测定和Masson染色观察,免疫组化检测心肌Bax、Bcl-2、p53和TGF-β1表达。结果:与A组比较,B组心系数显著下降(P<0.01),血流动力学指标LVSP和±dp /dtmax均显著下降(P<0.01),LVEDP显著升高(P<0.01),Bax、p53和TGF-β1的MOD值均显著升高,且p53和TGF-β1的MOD值升高极其显著(P<0.01);与B组比较,C组心系数显著提高(p<0.05)、LVSP和±dp /dtmax均显著升高(P<0.01),LVEDP显著降低(P<0.01),心肌Bcl-2的MOD值显著升高(P<0.01),Bax、p53和TGF-β1的MOD值均降低,但p53显著降低(P<0.01)。结论:大鼠MI后心肌组织Bax、p53和 TGF-β1的表达均显著升高,Bcl-2的表达降低,心肌细胞趋于凋亡;8周有氧运动可使MI大鼠心肌组织Bax、p53 和TGF-β1的表达显著降低,Bcl-2的表达显著升高,心肌细胞凋亡抑制,TGF-β1可能通过影响Bax和Bcl-2的表达参与了心肌细胞凋亡;有氧运动可能通过抑制MI大鼠细胞凋亡,减少心肌细胞丢失进而提高心脏功能。  相似文献

目的观察血栓通联合银杏达莫治疗急性脑梗死的临床疗效。方法选择临床确诊为急性脑梗死的患者100例,随机分为两组,血栓通联合银杏达莫治疗组(治疗组)与血栓通治疗组(对照组),每组各50例患者,输液治疗每日1次,疗程均为14日,观察治疗后两组神经功能缺损评分比较及临床治疗效果比较。结果治疗后神经功能缺损评分均低于治疗前,差异有统计学意义(P〈0.05);治疗后神经功能缺损评分,治疗组评分低于对照组,差异有统计学意义(P〈0.05)。治疗后临床疗效比较,治疗组总有效率为96%,对照组总有效率为84%,两组总有效率比较有统计学意义(P〈0.05),治疗组疗效明显优于对照组,且无明显副反应。结论血栓通联合银杏达莫治疗急性脑梗死疗效显著。  相似文献

一个细胞在特定生理或病理状态下表达的所有种类蛋白质称为蛋白质组。应用蛋白质组学的研究方法和理论,可以从整体、动态的角度来研究运动对心肌蛋白质的差异表达的影响。本文在综述了近年来国内外运动心肌蛋白质组学研究发展现状的基础上,展望了未来这一专题的研究方向将主要集中在:运动心脏蛋白质组学数据库的建立、蛋白质点的进一步确证、蛋白质点之间的相互作用以及蛋白质的修饰;深入运动心脏蛋白质组学的研究,将为进一步阐明运动心脏的重塑机制的研究以及运动心脏重塑过程中的基因调控、蛋白质修饰、变化机理的研究提供新的理论依据和新的研究思路;将为促进全面健身运动的开展以及慢性心血管疾病的运动康复的研究带来新的视角。  相似文献

碎裂QRS波对急性心肌梗死患者的短期预后评估(英文)   总被引:2,自引:1,他引:1  
This study is aimed to investigate the clinical significance and the short-term prognostic value of frag- mented QRS (fQRS) for patients with acute myocardial infarction (AMI). Three hundred patients with AMI were tested with retrospective analysis on the patients' clinical information, hospitalized treatment, fQRS onset time, location of lesions, and other relevant data, in order to assess the relationship between the presence of fQRS and its prognosis. The rates of malignant cardiac arrhythmia, left ventricular systolic dysfunction (LVSD), and mortality in the positive fQRS group were 13.6%, 29.2%, and 23.7%, respectively, with all showing a p value 〈0.05. For the ST segment elevation myocardial infarction (STEMI) subgroup, all the rates showed significant differences with a p value 〈0.01, while for the non-STEMI (NSTEMI) subgroup showed no significant differences. In patients with a positive fQRS, there were no differences in malignant cardiac arrhythmia between patients with and without percutaneous coronary in- tervention (PCI) (p〉0.05). As for the LVSD and mortality, the p values between patients with and without PCI were 0.031 and 0.000, respectively, suggesting statistical significance. The results imply that AMI patients with positive fQRS especially for the patients with STEMI had higher rates of malignant cardiac arrhythmia, LVSD, and mortality than the non-fQRS group. Patients of AMI with positive fQRS, who underwent early revascularization, could lower the incidence of the cardiovascular event. In addition, the presence of fQRS could be used as an indication of early in- tervention treatment for patients.  相似文献

研究目的:心脏破裂是急性ST段抬高型心肌梗死最严重的并发症之一,病情凶险,死亡率高。通过明确心肌梗死后出现心脏破裂并发症的危险因素,早期识别心脏破裂高危人群,有利于在急性心肌梗死的早期采取干预措施以减少心脏破裂的发生。创新要点:既往缺乏对中国sT段抬高型心肌梗死继发心脏破裂的系统风险评估,本研究采用回顾性队列研究的方法,完善并健全中国多中心的急性心肌梗死数据库,样本总量达到9798例,为国内同类研究中样本量最多。既往的国外文献报道:女性、高龄、心梗后就诊时间延迟、前壁心肌梗死、溶栓治疗等是心肌梗死后心脏破裂的独立危险因素。本研究在中国人群验证以上危险因素的基础上首次提出就诊时血色素下降及白细胞计数的升高与随后发生的心脏破裂密切相关。新的心脏破裂预测因素的发现有利于对心脏破裂更精细化的危险分层,早期识别心脏破裂高危人群。研究方法:建立中国多中心心肌梗死数据库,比较发生心脏破裂与未发生心脏破裂患者临床特点的差异,筛选敏感特异的心脏破裂的独立预测因素。重要结论:中国人群急性ST段抬高型心肌梗死后心脏破裂的发生率是1.82%,独立预测因素主要包括:高龄、女性、就诊时静息心率增快、前壁心肌梗死、心功能不全、就诊时间延迟、肾功能不全、贫血与白细胞计数升高。本文第一次提出就诊时贫血状态及白细胞计数的升高时与心脏破裂的发生直接相关。  相似文献

摘要:目的:探讨间歇运动及间歇运动联合TNF-α抑制剂Etanercept对心梗大鼠骨骼肌MuRF1和Smyd1表征的影响及炎症水平与骨骼肌功能改善的关系。方法:雄性3月龄SD大鼠70只,随机分为假心梗组(S)、心梗组(MI)、心梗+间歇运动组(ME)、心梗+ Etanercept组(MIT)、心梗+生理盐水组(MIS)、心梗+间歇运动+ Etanercept组(MET)、心梗+间歇运动+生理盐水组(MES),每组10只。采用左冠状动脉结扎法制备心梗模型,S组只在心脏相同位置穿线不结扎。ME组、MET组和MES组术后1周采用大鼠跑台进行间歇运动干预。其中MIT组和MET组于术前3 d腹腔注射Etanercept(1.2 μg/g)1次,术后i.p. 1次/3 d×4周,MIS组和MES组注射定量生理盐水作为参照。各组训练结束后次日腹腔麻醉,测定心脏功能,之后冰浴条件下快速剥离并切取胫骨前肌,入甲醛固定或液氮冷藏备用。胫骨前肌重量/胫骨长度比值测定骨骼肌质量变化,免疫荧光测定骨骼肌HSP70和骨架蛋白Myosin表达变化及细胞横截面积,RT-qPCR测定骨骼肌MuRF1和MAFbx mRNA基因表达变化,Western Blot测定骨骼肌MuRF1、MAFbx、Smyd1、HSP90、TNF-α、NF-κB(pp65)、IL-6、IL-10、HSP70、Myosin和α-actinin蛋白表达变化,糜蛋白酶活性试剂盒检测骨骼肌蛋白酶体活性变化。结果:心梗导致大鼠骨骼肌质量系数、细胞横截面积、细胞骨架蛋白Myosin和α-actinin及肌纤维生成关键因子Smyd1表达显著降低(P<0.01),MuRF1、MAFbx、TNF-α、IL-6、NF-κB(pp65)和伴侣分子HSP90表达显著升高(P<0.01),间歇运动或Etanercept干预以及二者联合干预均可显著提高心梗大鼠骨骼肌质量系数、细胞横截面积、IL-10、Myosin、α-actinin和Smyd1蛋白表达(P<0.05, P<0.01),降低MuRF1、MAFbx、TNF-α、IL-6、NF-κB(pp65)和HSP90水平(P<0.05, P<0.01);心梗大鼠骨骼肌的细胞浆和细胞核MuRF1蛋白表达显著升高(P<0.05, P<0.01),糜蛋白酶活性增加(P<0.01),间歇运动可进一步升高其胞浆和胞核HSP70表达(P<0.01),降低MuRF1蛋白表达、细胞浆/细胞核HSP70比值和糜蛋白酶活性(P<0.05, P<0.01),且机体炎症水平与细胞浆/细胞核HSP70比值呈显著正相关(P<0.01)。结论:间歇运动或Etanercept干预或二者的共同干预均可显著提高心梗大鼠骨骼肌质量、骨架蛋白和Smyd1的表达,增加细胞横截面积,降低MuRF1、MAFbx和HSP90的表达,改善骨骼肌功能。  相似文献
