Role of plasma amino acids and gaba in alcoholic and non-alcoholic fatty liver disease-a pilot study

Alcohol appears to affect brain function, primarily by interfering with the action of gamma-aminobutyric acid (GABA) and other
neurotransmitters. As alcohol is mainly metabolized in the liver, therefore we undertook this pilot study to monitor the patterns
of changes in plasma amino-acid concentrations due to alcoholic and nonalcohol fatty liver disease and their relation with
plasma GABA level. Plasma amino-acid concentrations were measured in 25 alcoholic liver disease (ALD) patients, 18 non-alcoholic
fatty liver disease (NAFLD) patients, and 24 age and sex matched control subjects by HPLC. GABA concentration was elevated,
while isoleucine and leucine levels reduced significantly in ALD patients compared to the control subjects. Methionine and
phenylalanine levels elevated and valine content reduced significantly in ALD patients compared to other two groups, and GABA
level was significantly correlated with methionine and phenylalanine. Plasma concentration of lysine was significantly reduced
in both groups of liver disease patients compared to the control group, but was not correlated with GABA level. Glycine and
tyrosine levels reduced significantly in NAFLD patients compared to other two groups and were significantly correlated with
GABA. Interestingly, though amino acids such as alanine, histidine, proline and serine were not affected by liver diseases,
but were significantly correlated with GABA level. This pilot study indicated that alcoholic liver disease presented a more
deranged plasma amino acid pattern than nonalcoholic, and the amino acid imbalances. More studies are necessary to identify
the role of any particular amino acid on brain function and on neurotransmitter(s). 相似文献

Alcoholic liver disease (ALD) is due to excessive alcohol intake for long duration. Distinguishing ALD from non-ALD (non-alcoholic steatohepatitis, hepatitis of viral origin) is difficult as patient may deny alcohol abuse. Clinical examination, histology and serology may not differentiate these conditions. Accurate diagnosis is important as management of ALD differs from non-ALD patients. The aim of our study was (1) To evaluate the patients of ALD and non-ALD by biochemical parameters compared to controls, (2) To assess whether these parameters can differentiate ALD from non-ALD. Study was carried out on 50 patients of ALD in group I and 35 patients of NASH (non-alcoholic steatohepatitis) and acute viral hepatitis each in group II. Age matched healthy controls n = 50. Selection criteria—history of alcohol intake (amount and duration), clinical examination, sonography of abdomen, serum alanine transaminase (ALT) and bilirubin levels. Blood samples were analyzed for bilirubin, aspartate transaminase (AST), ALT, alkaline phosphatase (ALP), gamma glutamyl transferase (GGT) by kinetic method. Statistical analysis was done by Student unpaired ‘t’ test. Patients of ALD have raised AST/ALT ratio (De Ritis ratio) (>2), ALP and GGT compared to controls (P < 0.01).There is significant difference in AST/ALT ratio, serum GGT and ALP in ALD group compared to that in NASH and acute viral hepatitis (P < 0.05). This study suggests that De Ritis ratio >2 in ALD patients may be due to alcohol induced hepatic mitochondrial injury and pyridoxine deficiency. High GGT and ALP values may indicate enzyme induction by alcohol and mild cholestasis. Thus ALD patients have severe hepatic damage. De Ritis ratio <1 and normal to mild elevation in GGT level in NASH and acute viral hepatitis suggest mild hepatic injury of non-alcoholic origin. Our study concludes that ALD patients can be differentiated from NASH and acute viral hepatitis with certainty by measuring serum AST/ALT ratio, GGT and ALP. These biochemical parameters may help clinicians to support the diagnosis of ALD and non-ALD. 相似文献

Ethanol-induced liver injury may be linked, at least partly, to an oxidative stress resulting from increased free radical
production and/or decreased antioxidant defence. Distinguishing alcoholic and non-alcoholic liver disease has important implications.
This study looked at the possible changes between alcoholic and non-alcoholic liver diseases by examining the presence of
oxidative damage, as monitored by several parameters relating to oxidative stress. Lipid peroxides concentration, superoxide
dismutase activity and glutathione S-transferase activity increased, where as glutathione content, glutathione peroxidase
activity and glutathione reductase activity decreased among the tested subjects in comparison to normal healthy group. Determination
of these parameters may be valuable in the evaluation of liver disease. However, oxidative stress related enzymes and non-enzymes
can not be utilized as a marker for alcoholic liver diseases, as these parameters responded in the same way after liver is
damaged irrespective of their cause. Their level may help in determining the degree of liver damage. Degree of oxidative injury
was similar in patients with non-alcoholic liver disease and in moderate drinkers; while significantly higher in heavy drinkers.
The differences between the groups might be based on the type of liver pathological condition rather than its etiology (i.e.
alcohol and non alcohol related causes). 相似文献

The present study was conducted to evaluate some of the components of antioxidant defense system and oxidative damage in 20
male patients of alcoholic liver disease (ALD). The results were compared with 20 healthy male smokers and 20 healthy male
non-smokers volunteers. Patients were subjected to detailed clinical examination and laboratory investigations. Blood samples
were collected for estimating reduced glutathione (GSH), total thiols (T-SH) malondialdehyde (MDA), transaminases (AST, ALT),
glutathione-S-transferease (GST) and gammaglutamyl transferase (GGT). Serum aspartate amino transferase (AST)/alanine amino
transferase (ALT) ratio was significantly (p<0.01) reduced in ALD patients as compared to the controls. However, the core
of utility of MDA and GST was found to be significantly (p<0.01) increased in ALD patients compared to controls. There was
a significant negative correlation of MDA with both GSH and TSH. Plasma GGT levels were significantly (p<0.01) increased in
alcoholics and the enzyme showed a significant positive correlation with MDA. These results give enough evidence of increased
oxidative stress and compromised antioxidant defense system in patients with ALD. 相似文献

The effect of Emblica officinalis fruit extract (EFE) against alcohol-induced hepatic damage in rats was investigated in the present study. In vitro studies showed that EFE possesses antioxidant as well nitric oxide (NO) scavenging activity. In vivo administration of alcohol (5 g/kg b.wt/day) for 60 days resulted increased liver lipid peroxidation, protein carbonyls, nitrite plus nitrate levels. Alcohol administration also significantly lowers the activities of superoxide dismutase, catalase, glutathione peroxidase, glutathione S-transferase and reduced glutathione as compared with control rats. Administration of EFE (250 mg/kg body weight) to alcoholic rats significantly brought the plasma enzymes towards near normal level and also significantly reduced the levels of lipid peroxidation, protein carbonyls and restored the enzymic and non-enzymatic antioxidants level. This observation was supplemented by histopathological examination in liver. Our data indicate that the tannoid, flavonoid and NO scavenging compounds present in EFE may offer protection against free radical mediated oxidative stress in rat hepatocytes of animals with alcohol-induced liver injury. 相似文献

Serum alkaline phosphatase (ALP) (EC 3.1.3.1), 5′nucleotidase (5′NT) (EC 3.1.3.5), aldolase (ALD) (EC 4.1.2.13) and sorbitol
dehydrogenase (SDH) (EC 1.1.1.14) were estimated in infective hepatitis, alcoholic hepatitis, chronic active hepatitis, obstructive
jaundice, cirrhosis of liver and amoebic liver abscess. It was observed that serum ALP and 5′NT were significantly increased
in all cases of chronic active hepatitis and obstructive hepatic disease. However, the elevation observed in the latter was
much higher than the former. Serum SDH and ALD levels were elevated in all cases of infective hepatitis, studied though increase
in the former was much higher than the latter, suggesting its significance in the diagnostic confirmation of this disease.
Results presented suggest 5′NT and SDH as more reliable diagnostic test compared to ALP and ALD for obstructive jaundice and

The severity of alcoholic cirrhosis depends on the presence of liver inflammation and fibrogenesis. Previous studies have hypothesized that carbohydrate deficient transferrin can be used as marker of liver impairment in alcoholic liver disease patients. The present study was designed to assess whether carbohydrate deficient transferrin is associated with procollagen III peptide and predict fibrosis in alcohol cirrhosis patients. We enrolled 48 patients with alcoholic cirrhosis and 38 healthy controls. Serum carbohydrate deficient transferrin, procollagen III peptide and interleukin-6 levels were estimated in both groups. Serum carbohydrate deficient transferrin, procollagen III peptide and interleukin-6 were significantly increased in alcoholic cirrhosis patients compared to controls. Stepwise regression analysis showed that carbohydrate deficient transferrin (adjusted R2 = 0.313, β = 0.362, p = 0.003) and interleukin-6 (adjusted R2 = 0.194, β = 0.459, p = 0.001) were positively associated with procollagen III peptide when age, duration and amount of alcohol consumption were considered as covariates. We conclude that elevated carbohydrate deficient transferrin and interleukin-6 act as predictors of fibrosis in alcoholic cirrhosis. 相似文献

A number of factors are linked with non-alcoholic fatty liver diseases (NAFLD), a condition that ranges from clinically benign
fatty liver to its more severe form, non alcoholic steatohepatitis (NASH). In this study, we evaluated the role of cytokines
secreted from adipose tissue in the pathogenesis and progression of NAFLD. We also compared anthropometric profile, lipid
profile and insulin resistance data in 105 NAFLD patients with 77 normal subjects. These subjects showed a normal serum albumin
level, prothrombin time and renal function but elevated aminotransferases. Predisposing factors were diabetes mellitus (35%),
overweight (56%) and hyperlipidemia (44%). Insulin resistance (IR), determined by homeostasis model assessment (HOMA) was
confirmed in 70% patients with NAFLD and 42% patients fulfilled the minimum criteria for insulin resistance syndrome (IRS).
NAFLD patients showed elevated levels of pro-inflammatory cytokines tumor necrosis factor (TNF)-α, and interleukin (IL)-6,
while anti-inflammatory cytokines IL-4 level decreased and IL-10 level remain unchanged; however, TGF-β1 level elevated significantly
compared to normal subjects. While insulin level and HOMA-IR both were significantly positively correlated with BMI, waist-to-hip
ratio, total cholesterol, VLDL-cholesterol, triglyceride and TGF-β1; glucose, IL-6 and TNF-α levels were significantly positively
correlated with HOMA-IR only. In conclusion, pro-inflammatory cytokines play an important link between metabolic and liver
disorders in the fat accumulation, and thereby cause IR, inflammation and liver fibrosis. 相似文献

The effect of acute hypoxic hypoxia on the profile of plasma amino acids in rats was studied and compared to that resulting
from acute liver injury induced by giving carbon tetrachloride. In hypoxic rats exposed to 45% air in N2 for 5 h, the concentrations of branched chain amino acids, including valine, leucine and isoleucine, and aromatic amino acids
such as phenylalanine and tyrosine were significantly increased as compared to those in normoxic rats. The ratio of branched-chain
to aromatic amino acids (Fischer’s ratio) was significantly decreased. The levels of arginine and citrulline, which are related
to the urea cycle, were also depressed. Furthermore, plasma proline level was reduced in hypoxic rats. The activities of plasma
marker enzymes for tissue damage remained unchanged during hypoxia, indicating that tissue injury was not induced by exposure
to hypoxic conditions. We suggest that the characteristic profile of plasma amino acids and the Fischer ratio are valuable
tools for understanding the pathology of acute hypoxia in the absence of systemic tissue damage. 相似文献

This work was designated to monitor the coagulation abnormalities associated with the gradual progression of liver diseases. The study included fifty patients; forty were diagnosed with liver cirrhosis with different stages categorized according to the Childs-Pugh classification and another ten patients were diagnosed with hepatocellular carcinoma (HCC). Haemostatic variables including fibrinogen (FI), calcium (FIV), transglutaminase (FXIII), prothrombin time (PT) and platelet count were estimated in patients and compared with the baseline levels of healthy subjects (n = 10). The results demonstrated that the fibrinogen level was progressively decreased, whereas PT was progressively prolonged in Child A, Child B and Child C groups. The maximum deterioration was observed in HCC patients. Calcium significantly increased in mild (Child A) and moderate (Child B) but not in Child C cirrhosis and HCC patients. FXIII level did not show any significant changes in cirrhotic patients compared to healthy group. Some of the haemostatic variables we investigated were correlated with serum albumin and bilirubin but not with aminotransferases (ALT and AST). The results indicated that the haemostatic abnormalities in fibrinogen, calcium and PT (but not FXIII) were deteriorated in parallel with the gradual regression of the constitutional function of liver. 相似文献

Total amylase activity, its isoenzymes and pancreatic to salivary ratio were studied in 33 healthy subjects and 52 patients with hepatobiliary and pancreatic malignancies of which 12 were with hepatocellular carcinoma, 12 with liver metastases, 9 with carcinoma of gall bladder, 6 with obstructive jaundice, 7 with periampullary carcinoma and 6 cases with carcinoma of pancreas. Total amylase was found to be elevated in carcinoma of pancreas and periampullary carcinoma compared to normal whereas in obstructive jaundice the values were decreased significantly (p<0.001). P (Pancreatic) fraction was found to be elevated significantly in all except in carcinoma of pancreas and obstructive jaundice. S (Salivary) fraction was decreased significantly in all except in carcinoma of pancreas. P/S ratio was elevated in hepatocellular carcinoma, obstructive jaundice, metastatic liver disease, gall bladder carcinoma and periampullary carcinoma (p<0.001) but in carcinoma of pancreas P/S ratio was low as compared to normal. This simple test of total serum amylase, P fraction and S fraction, P/S ratio can be utilised in differentiating various hepatobiliary and pancreatic malignancies. 相似文献

Various metabolic and biological changes follow burn injury. Serum Thio-barbituric acid reactive substances (TBARS), transaminases,
alkaline phosphatase and amylase were measured in 43 patients with thermal injury over the first 10 days of post burn period.
No clear correlation between elevated serum enzymes except amylase and the burn size was observed on admission. Mean serum
TBARS were significantly increased in the burn patients. Transaminases values increased till 5th day then declined on 10th day, whereas alkaline phosphatase and amylase activities continued to rise till day 10. It is concluded that functional disturbances
occur in liver and pancreas around a week after thermal injury. Monitoring serum ALP and amylase in postburn period has valuable

Alcohol consumption and health outcomes are complex and multidimensional. Ethanol (1.6g / kg body weight/ day) exposure initially
affects liver function followed by renal function of 16–18 week-old male albino rats of Wistar strain weighing 200–220 g.
Chronic ethanol ingestion increased in thiobarbituric acid reactive substances level and glutathione s-transferase activity;
while decreased reduced gluatathione content and activities of catalase, glutathione peroxidase and glutathione reductase
in a time dependent manner in the hemolysate. Though superoxide dismutase activity increased initially might be due to adaptive
response, but decreased later. Elevation of serum nitrite level and transforming growth factor-b1 activity indicated that long-term ethanol consumption may cause hepatic fibrosis and can elicit pro-angiogenic factors. However,
no alteration in vascular endothelial growth factor-C activity indicated that ethanol consumption is not associated with lymphangiogenesis.
Therefore, we conclude that long-term ethanol-induced toxicity is linked to an oxidative stress, which may aggravate to fibrosis
and elevate pro-angiogenic factors, but not associated with lymphangiogenesis. 相似文献

Osteoporosis becomes a serious health threat for older postmenopausal women by predisposing them to an increased risk of fracture.
Osteoporosis and associated fractures are an important cause of morbidity and mortality. Special attention is being paid to
early detection, management, and treatment of postmenopausal osteoporosis in women. Biochemical markers can enable dynamic
and rapid measurement of total body skeletal metabolism and will be clinically useful in the management of postmenopausal
osteoporosis women (PMO) and also for assessing the effects of antiresorptive therapy. With this view, we planned to assess
osteoclastic activity by determining urinary hydroxyproline in osteoporotic women. The aim of this study is to measure urinary
hydroxyproline (expressed as mg of hydroxyproline/g of creatinine) and serum ascorbic acid in postmenopausal women with osteoporosis
and without osteoporosis. These biochemical parameters were determined 3 months post antiresorptive therapy (alendronate + calcium + vitamin
D) in postmenopausal osteoporosis patients. 60 postmenopausal women with osteoporosis in the age group 45–60 years and 60
healthy postmenopausal women (normal bone mineral density) in the same age group were included in the study. Urinary hydroxyproline
levels were significantly increased (P < 0.001) in PMO at baseline level as compared to control group. These levels were decreased significantly (P < 0.001) post therapy in PMO patients. Serum vitamin C levels were significantly decreased (P < 0.001) in PMO patients at baseline level as compared to controls. No significant change occurred of serum vitamin C level
post therapy. Raised excretion of hydroxyproline at the baseline level might be due to increased degradation of collagen type
I from the bone matrix in osteoporosis. Breakdown of collagen seems to be lowered as reflected by lowering of hydroxyproline
excretion post antiresorptive therapy. Alteration in the concentration of this marker can be very well utilized to monitor
the effectiveness of therapy. Thus simple, direct urinary assay to measure bone resorption is very useful in monitoring the
therapy in PMO and may become an integral part of the management of osteoporosis. 相似文献

The present study was carried out to investigate the protective role of Triphala (a combination in equal proportions by weight
of fruit powder of Terminalia belerica, Terminalia chebula and Emblica officinalis) against 1,2-dimethylhydrazinedihydrochloride (DMH) induced Endoplasmic reticulum stress (ER stress) in mouse liver. An oral
dose of 3 mg/kg body wt in drinking water for 5 weeks significantly (P < 0.001) increased the levels of serum glutamate oxaloacetate transaminase (SGOT), serum glutamate pyruvate transaminase
(SGPT), serum Alkaline phosphatase (ALP) and total bilirubin thus suggesting damage to mouse liver and biliary dysfunction.
The DMH administration invariably led to increase in the liver microsomal proteins of molecular weight of about 29 (ERp29)
and 53 kDa (ERp53) and decrease in the protein of molecular weight of 36 kDa (ERp36) thereby suggesting the interference of
DMH and its metabolites with normal protein biosynthesis and folding, in the reticular membranes of the liver cells thus developing
ER stress. Histological studies show necrosis, large sized hepatocytes with increased N:C ratio, aberrant mitotic figures
and prominent nucleoli in the liver of DMH treated mice. In animals fed 5% Triphala in diet (w/w) during DMH administration,
there was significant decrease in the above changes in the liver suggesting the suppression of DMH induced ER stress in liver.
Triphala significantly (P < 0.05) decreased lipid peroxidation and also the activity of lactate dehydrogenase (LDH) in mouse liver. It simultaneously
increased the level of reduced glutathione (GSH) and the activity of glutathione-S-transferase (GST) thereby suggesting that
it prevents peroxidative damage and also diverts the active metabolites (electrophiles) of DMH from their interactions with
critical cellular bio-molecules which could be responsible for its protective action against DMH. 相似文献

This retrospective study was conducted to assess the serum lipase / amylase ratio in acute pancreatitis for South Indian population attending the clinics of Gastroenterology and Emergency medicine in the last five years. One thousand one hundred and thirty two patients (768 males and 364 females) with acute pancreatitis (AP) were selected for the study. The diagnosis of AP was based on clinical evaluation, Computed Tomography (CT) findings and biochemical parameters such as serum lipase and amylase. Based on the etiology, the groups were divided into alcoholic, biliary and miscellaneous AP. Based on CT findings, groups were divided into mild, moderate and severe AP. Serum lipase, amylase and lipase / amylase ratio were calculated and statistically analyzed. Serum lipase levels in alcoholic group (3226.3 ± 1384.7) were significantly lower in comparison to biliary (6064.5 ± 1640.8) group though the levels were not significantly different from miscellaneous group (2132.3 ± 1210). Serum amylase values were significantly lower in alcoholic group (923.4 ± 557.5) in comparison to biliary (1736.2 ± 390.7) and there was significant difference between alcoholic and miscellaneous group (535.8 ± 477.6). The serum lipase / amylase ratio > 4 occurred in alcoholic group than with biliary and miscellaneous group. The sensitivity and specificity to predict alcoholic AP with lipase / amylase ratio at >4.0 was 84 % and 59 % respectively. In conclusion the serum lipase to amylase ratio greater than 3.0 could be used to differentiate but keeping the cut off at 4.0 would be of higher sensitivity without much change in specificity. The serum lipase to amylase ratio with a cut off of 3.0 or greater is not useful to differentiate the severe AP from milder AP. Hence, serum amylase and lipase are important for evaluation of pancreatitis though it is not a gold standard for the diagnosis or assessment of severity of acute pancreatitis. 相似文献

(18–20 mg/kg) did not significantly change the gross body, weight and tissue weights of brain and liver. Packed cell volume
(PCV) and hemoglobin concentration remained unaltered. However, non-heme iron content in liver and brain decreased significantly
(p<0.001). The activities of glutamate dehydrogenase, glutamic acid decarboxylase, and GABA-transaminase (GABA-T) in brain
decreased by 15%, 11.4% and 25.7% respectively. However, this decrease was not statistically significant. Binding of3H Muscimol at pH 7.5 and 1 mg protein/assay increased by 143% (p<0.001) in synaptic vesicular membranes from iron-deficient
rats as compared to the controls.3H glutamate binding to the synaptic vesicles was also carried out under similar condition. However, the L-glutamate binding
was reduced by 63% in the vesicular membranes of iron deficient animals. These studies indicate that iron plays important

Introduction
Sparse data are available about the effect of therapy methods on antibody levels in patients with liver failure. The aim of this study was to determine serum immunoglobulin concentrations in patients with chronic hepatic failure (CHF), acute- (ALF), or acute-on-chronic liver failure (ACLF) and to evaluate the impact of MARS treatment or liver transplantation (LT) on antibody levels.Materials and methods
We followed ten patients with ALF, twelve with ACLF and 18 with CHF. Eight patients with ALF and seven with ACLF underwent MARS therapy, whereas the rest received LT. 13 healthy volunteers served as controls. Serum antibody concentrations were measured using ELISA-technique.Results
Median serum levels of IgA, IgG and IgM were significantly increased in patients with CHF compared to ALF or controls (P < 0.02, P < 0.01, and P < 0.01). IgM and IgG concentrations were also significantly elevated in patients with CHF compared to ACLF (IgM, 3.7 vs. 1 g/L, P < 0.001; IgG, 8.7 vs. 3.1 g/L, P = 0.004). Immediately after LT a significant decrease of IgA (6.9 vs. 3.1 g/L, P = 0.004), IgG (8.7 vs. 5.1 g/L, P = 0.02) and IgM (3.7 vs. 1.8 g/L, P = 0.001) was detected in patients with CHF and antibody levels further decreased the days after LT reaching levels comparable to healthy individuals. MARS treatment had no apparent effect on the immunoglobulin profile in patients with ALF or ACLF.Conclusion
We provide evidence that LT reverses hypergammaglobulinemia in patients suffering from CHF within one day, which could be explained to a reconstituted hepatic antibody clearance, whereas MARS treatment has no immediate effect on immunoglobulin levels.Key words: antibodies, immunoglobulins, liver failure, liver transplantation, artificial liver support system, molecular adsorbent recirculating system 相似文献20.
Caffeic acid is a well-known phenolic compound widely present in plant kingdom. The aim of this study was to investigate the
possible protective effect of caffeic acid (CA) against oxytetracycline (OXT) induced hepatotoxicity in male Albino Wistar
rats. A total of 30 rats weighing 150–170 g were randomly divided into five groups of six rats in each group. Oral administration
of OXT (200 mg/kg body weight/day) for 15 days produced hepatic damage as manifested by a significant increase in serum hepatic
markers namely aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), lactate dehydrogenase
(LDH), bilirubin and increased plasma and hepatic lipid peroxidation indices (TBARS and hydroperoxide). The present finding
shows that the levels of enzymatic antioxidants namely superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase
(GPx) were significantly decreased in OXT intoxicated rats. Upon oral administration of caffeic acid (40 mg/kg body weight/day)
there were decreased hepatic marker activities, bilirubin and lipid peroxidation and increased enzymatic antioxidants in OXT + Caffeic
acid group compared to Normal + OXT group(P < 0.05). Our study suggests that caffeic acid has antioxidant property and hepatoprotective ability against OXT induced toxicity. 相似文献
